This inspection was carried out on 20th February 2006.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The service continues to provide a safe and very comfortable home with a committed staff team. The needs and wishes of service users come first and every effort is made to ensure that they lead full interesting lives. Service users enjoy leisure and social activities. During the morning a general knowledge quiz was being played in the main lounge. Both service users and staff were enjoying the activity. Staff were observed to provide care in a supporting yet sensitive manner and were respectful at all times. Service users spoke positively about the home. One service user confirmed that when anything is raised at the residents meetings it is always dealt with. Another service user who had not lived at the home very long said that "staff are very kind and helpful" and this has gone towards helping her settle. There were no requirements or recommendations made at this inspection.

What has improved since the last inspection?

The home continues to improve through the strong leadership and management approach of the home. It is evident that staff are given direction and are supported by the homes management team. This was clear in the discussions held with staff. Service users spoken with said that all staff are approachable and helpful and that they are able to do as they wish, and that they were fortunate to live in such a good home.

What the care home could do better:

The home should continue to maintain the high standard of care it provides as it continues to meet service users expectations and aspirations.

The home is a detached property that is purpose built which also adjoins a sheltered housing scheme. The accommodation is spaced over two floors and all areas are accessed by a vertical passenger lift. The home is set in its own grounds with a large car park to the front of the home. Townend Close is in easy access of local amenities in Crosshills and the towns of Skipton and Keighley are readily accessible. This report relates to an unannounced inspection carried out on the 20th February 2006, which started at 9.45hrs and finished at 12.15hrs. A tour of the home was carried out which included the service users private accommodation, a selection of records were looked at and time was spent observing activity in the home, talking and listening to service users and staff. The focus of the inspection was a number of key standards, inspecting the care records of four service users in detail to establish if they corresponded with service users experiences in the home. The registered manager was not on duty at the time of inspection. Margaret Bishop Senior Care assisted throughout the morning. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,5 and 6 Prospective service users can be confident before moving into the home that their needs can be met. EVIDENCE: There have been no amendments made to the Statement of Purpose or the Service Users Guide since the last inspection. All the required information is given to all prospective service users, which was confirmed by one new service user who had recently moved into the home. All service users are given Terms and Conditions of Residence and a copy is held on file. Service users are given the opportunity prior to their admission to visit the home and stay for the day, come for tea or a overnight stop, whatever they wish or are able to do. The senior carer stated that the home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 11 Service users are well supported to make choices about all aspects of their health, personal and social care needs. EVIDENCE: Four service users case files were inspected, which contained comprehensive plans of care. Risk assessments covering areas of daily living were available and up to date. The health care needs of service users were recorded on their individual care plans. Risk assessments covering areas of daily living were available and up to date. Copies of Terms and Conditions of Residence were also held on individual files. Service users files continue to be well documented. The organisations policies regarding the death of a service user are in place. The senior carer was able to demonstrate that the home has recently dealt with a death in a sensitive manner. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 Service users dietary needs are well met. EVIDENCE: Mealtimes at Townend Court continue to be flexible. Choices are available each mealtime and all service users spoken with commented that the food at the home was very good. Special diets such as diabetics are catered for. Menus were inspected and on a four weekly cycle. Seasonal changes are made accordingly. The kitchen was clean and maintained well throughout. In discussions with the main cook, the environmental health had recently inspected the home. Three recommendations were made, which are currently being completed. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Service users are confident that any concerns they may have are acted upon. EVIDENCE: The home’s complaints procedure is available to service users and visitors. Service users commented that if they had a complaint or a concern that they would speak directly with staff on duty or to the manager. It then would be acted upon. Service users were also confident that the registered manager would always put things right. There has been one informal concern raised which has been dealt with appropriately by the home. Residents meeting are held regularly. This was seen as being a good forum for raising concerns and was confirmed by service users. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 and 26 Service users are provided with a clean and homely environment, which continues to be maintained to a high standard. EVIDENCE: The home continues to be maintained to a high standard throughout. All areas of the home inspected were all clean and no unpleasant odours were present. The home was warm, well lit and ventilated. Service users rooms have been personalised with their own furniture and belongings. Service users who showed me their rooms were proud and said that they felt that they were in their own flat than it being a care home. They also commented on how well the home is kept clean. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 Service users benefit sufficient numbers of staff on duty that are well informed. EVIDENCE: The staff duty roster was inspected. This reflected eight care staff including two senior carers on duty in the morning. There is two waking night staff on duty each night. There is also one administrator, one activities co-ordinator. The ancillary staff includes three kitchen staff, four domestic staff and one maintenance man. JUDGEMENT – we looked at outcomes for the following standard(s): 38 Service users’ health safety and welfare continues to be promoted and protected. EVIDENCE: Throughout the morning and from discussions held with service users and staff and through observation, Townend Close continues to be managed well with a committed staff team. The organisations health and safety policies and procedures are in place. A number of health and safety records were inspected all of which were up to date and accurately maintained.
