What the care home does well

Feedback from service users and their carers continue to indicate that the service provided by Treefields is highly valued and is found to be `excellent` One of the strengths of the service is evident in the way activities are provided to service users. Staff ensures that such activities are interlinked with those taking place in the community. There is excellent communication among the staff, service users and their carers. There is also evidence of good consultation and collaborative work among them and other professionals from other services. Service users are provided with appropriate care and support through wellpersonalised support programmes, whilst they are in respite care.

What has improved since the last inspection?

The physical environment has been improved and made more pleasant. The health and safety aspects have been reinforced for the benefit of both service users and staff. New entertainment facilities have been provided for the benefit of the client group. A new care planning document has been produced and has been successfully implemented. Staff training has progressed well.

What the care home could do better:

Although the information package relating to Treefields has been revised to include criteria for access to the respite service, there is a need to improve the statement of purpose. Guidance on this matter has been provided and a recommendation is made for this document to be improved. The service provider needs to make sure that the registered manager is given the opportunity to commence the required training in order to obtain the qualification he needs, as laid out in the relevant National Minimum Standard.

It is a detached two-storey house, situated on the Wingfield estate, about three miles from Rotherham town centre. It is accessible by public transport and is within reach of local amenities. It is owned and managed by Rotherham Local Authority’s Social Services Department. The accommodation is offered in single bedrooms, five on the first floor and one on the ground floor. Access to the first floor is only by stairs. There is a lounge, a dining room, a kitchen and a small laundry area on the ground floor. There is an office at the side of the main entrance, which also serves as a reception point. There are garden areas at the front and back of the house. The side and rear grounds are fenced off. The back garden provides a patio area with garden furniture. This inspection was carried out on 26 January 2006, starting at 11.30 hours and finished at 15.30 hours. The inspection included a tour of the premises, conversation with three members of staff and the manager. Care documentation and other records were checked. Five service users were expected to start their respite stays on the afternoon of this inspection and arrangements for their admission were well organised. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 Although Treefields provides a good range of information about its services, in appropriate formats, it has not yet fully clarified what arrangements are in place for meeting some of its aims and objectives. EVIDENCE: The statement of purpose and service user guide have been developed and revised. The service user guide is well designed, with pictures and symbols so that it can be accessible to the client group. The criteria for admission, including the management of ‘emergency admissions’ have been outlined. However, these criteria were not contained in the statement of purpose. Advice has been given and a recommendation is made for improving the statement of purpose. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6 Care plans are appropriately developed to reflect the needs of individual service users. EVIDENCE: A new service user plan has been developed. It allows for service users and carers’ involvement in the planning of the respite stays and the care that is required. Care staff commented that they would telephone the main carers of the clients before admission, in order to discuss any changes that may have taken place in relation to the service user, and which could affect their care. These arrangements had been followed in preparation for the admission of service users, on the afternoon of this inspection, for their respite stays. A sample of care plans were checked and showed evidence of person-centred planning. Relatives and service users were satisfied with the way care and support is provided at Treefields. The ageing, illness and death of a service user are handled with respect and as the individual would wish. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24. Treefields continues to provide a good standard of accommodation, which is homely, comfortable and safe for its service users. EVIDENCE: The inspector, accompanied by a senior member of staff, undertook a tour of the communal parts of the Centre. They were found to be in a good decorative state. The entrance hall has recently been decorated. The laundry room has been tidied to improve access and its safe use. The external gate to the garden area has been provided with an electronic keypad and made more secure. The manager stated that the amenities fund of the centre had been used to purchase new entertainment facilities for the lounge and for use by service users. The manager also explained that he would be making a business case for continuing improvement in the physical environment of the Centre. Treefields continues to be a well organised and well managed respite service, which meets the needs of its client group. EVIDENCE: The registered manager has the necessary experience and skills to manage the Centre and to oversee the welfare of the resident group. However, he has not yet started the course of training to enable him to have the required qualification as laid down by the regulations. Urgent action must be taken to ensure the registered manager acquires the necessary qualification. There is a good staff team at the Centre. There was evidence of collaborative work to ensure the wellbeing of service users and their carers. Staff spoken to, stated that they had received training on a range of health and safety issues. This included fire safety and moving and handling. The office lay out has also been reorganised in order to meet health and safety guidelines. Staff have also undertaken training on adult protection issues.
