Conditions of registration: 1 A maximum of 28 service users may be acommodated at any time. 2 A maximum of 3 service users over the age of 65 years in the DE(E) Dementia service user category may be accommodated. 3 A maximum of 3 service users over the age of 65 years in the MD(E) Mental Disorder service user category may be accommodated. Date of last inspection 7th October 2004 Brief Description of the Service: Tripletrees is a care home, registered to provide personal care for a maximum of twenty eight older people to include a maximum of three service users with dementia/mental disorder. Tripletrees is a large detached and extended property, situated in Burgess Hill, being close to shops, local amenities and transport links. The accommodation is arranged on three floors and comprises of twenty four single bedrooms and two double rooms which are served by a passenger lift and stair lift. Communal rooms include a large lounge and a dining room on the ground floor. The service is privately owned by Follet Care and is managed by Mrs M Follet. This is an overview of what the inspector found during the inspection. This unannounced inspection was done as part of the inspections required to be carried out over each inspection year by CSCI. The inspector talked to residents and carried out a tour of the plus talked to staff on duty. Records were looked at. Four residents were happy to talk to the inspector. The inspector spent three and a half hours at the home. Pre inspection planning included reading through the last inspection report, and a discussion with the inspector who carried out that inspection. There have been no complaints or concerns raised to CSCI about this service since the last inspection. What the service does well: What has improved since the last inspection?
The external path that was uneven has been roped off and is awaiting renewal. This will maximize safety for residents. The manager and deputy manager attended a training day on Adult Abuse on the day of the inspection and will be providing in house training to staff following the course.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3and 5. Residents who live at this establishment receive clear information to enable them to make a choice about whether or not they wish to live in the home. EVIDENCE: The home Statement of Purpose and Service Users Guide clearly states the facilities and care the home can provide to enable residents to make a choice as to whether the home can meet their needs. Three residents confirmed they and their families were given the opportunity to visit the home before making a decision and were in possession of written information about the home. Records show that the home carries out pre admission assessments on prospective residents to ensure the home can meet their care and social needs. Resident stated that they felt all their needs were met by very pleasant staff who always have a smile. The last inspection report was accessible for any one to read. The home need to ensure all contacts stipulate the room numbers residents are to be accommodated in, as recommended in The National Minimum Standards for Older People, standard 2. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9,10. Residents who live at this establishment have their health and personal care needs met. Residents are treated with respect and dignity. Medication administration is sound. EVIDENCE: Care plans set out health, personal and social care needs ensuring staff deliver the correct support and care required by individual residents. Plans seen were very comprehensive and well maintained, being reviewed regularly to keep up to date with changing care needs. Residents spoken with stated they felt they were well looked after and received the care they needed. One resident described the staff as “lovely and caring, always smiling when they help you”. Another resident stated “nothings ever too much for the staff, if you ask for something they always do their best to help”. Medication records showed all to be in order. Records are clear and well maintained ensuring correct procedures are carried out. Records show that risk assessments are in place to limit the risk of areas such as pressure sores, falls and poor nutrition. Residents stated that staff knock on their bedroom doors before entering and communicate with them in a respectful manner. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13, 15 Residents who live in this establishment are provided with a variety of activities. Contact with family and friends are encouraged. Food provision is good. EVIDENCE: All residents spoken with stated their friends and family are able to visit at any time ensuring important contact is maintained. On the day of the inspection the activities co-ordinator was seen to be present and just finishing the mornings organised activities. The activities co-ordinator works Monday to Friday between 10.30am and 12 midday providing a wide variety of activities, from sewing to games and general chat, for residents to participate in, if they wish. Afternoon activities are generally board games or time to rest. Staff spoken with were very positive about providing afternoon activities and stated board games such as scrabble were very popular. The regular hairdresser was also on the premises during the inspection and residents were very complimentary about the service she provides. The food provision is of a good standard. On the day of the inspection the cook had provided a lunch of Pork, parsnips, cauliflower, broccoli and mashed potato followed by rice pudding and jam. Residents stated they had enjoyed their lunch and confirmed they had an alternative choice if they did not want the menu on offer. Residents also confirmed that staff ask them daily what they would like for lunch and supper, from the choice on offer. Records are kept of food intake and closely monitored if there are concerns. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18 Residents who live at this establishment are protected from abuse. There are clear processes for complaints doe residents and visitors to activate if necessary. EVIDENCE: The senior carers on duty during the inspection were not aware of any complaints being addressed at present. The complaints procedure was seen on display in the entrance hall of the home. Residents stated they would feel happy to complain to staff if they had any concerns. No resident voiced any concern during the inspection. The home has an adult protection policy and records showed no incidents to be recorded. CSCI has not received any complaints or adult abuse enquiries since the last inspection. Staff confirmed that they had covered adult abuse whilst completing their NVQ qualifications. The manager and deputy manager were attending a training course on the day of the inspection relating to adult abuse and staff indicated that they would be receiving in house training following the managers and deputy managers completion of the course. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 23, 24, and 26 Residents who live at this establishment live in a well maintained environment which is safe and clean and meets the needs of current residents. Personalisation of bedrooms is encouraged. EVIDENCE: A tour of the home showed evidence of maintenance work being carried out on a regular basis. Aids and adaptations are in place to assist residents getting around the home and accessing toilet and bathing facilities. All bedrooms were very clean and fresh smelling, pleasantly decorated with many personal possessions. Some rooms are small but furnishings have been sensibly installed to maximize room space. One or two carpets are in need of re stretching as they present as a health and safety risk. The last inspection noted the uneven path and recommended a risk assessment be in place to lessen the risk of falls. The senior carers showed the inspector the path that was roped off and awaiting work to make it safe. Room 7 has had difficulties with a fire door leading off and comment was made at the last inspection. The fire door is now closed with ventilation via a window above the door resulting in the resident be able to get fresh air as they wish and to no detriment to the fire exit. The homes laundry room does not have hand-washing facilities, and although the home does not have much foul laundry, the provision of a hand washing facility in the laundry room is advised. Current facility is in the staff room located a short distance form the laundry room. Best practice shows the provision of hand washing facilities in the laundry room should be seriously considered to maximize the protection for both staff and residents from cross infection. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 Residents who live at this establishment are cared for by a staffing team led by a registered manager. Staffing levels suit the needs of residents. EVIDENCE: The inspection was carried out in the presence of two senior carers, one experienced as a senior and one less experienced. The less experienced senior was receiving support from the more experienced carer and appeared to be working well together. The manager and deputy manager were attending a training course on the day of inspection. The home was seen to be running smoothly and efficiently in the care of the two senior staff. A staff rota showed staffing levels to be consistent with no gaps. Residents spoken with were very complimentary about the staff with many stating “they are very kind and always happy”. No resident commented on a lack of staff at any time. The inspector noted very good communication between staff and residents during the inspection with residents feeling at ease with staff. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 38 The home is very resident focused. Residents and staffs welfare is promoted and protected. EVIDENCE: The residents are very much the focus of the home with records evidencing the care needed and provided, and activities provided to meet the residents requests. Residents have the freedom to move about the house as they wish. There were no health and safety concerns, other than a few carpets requiring re stretching, and all staff, including the domestic staff were seen to be working in accordance with health and safety procedures. Due to the manager not being present during the inspection the inspector has not assessed many of these standards. This will be addressed at the next inspection.
