What the care home does well

This is a home where older people are respected and their needs are met. Residents spoke very highly of the care provided at Tripletrees. An experienced and caring manager communicates a clear sense of leadership. Staff are caring and attentive to the needs of residents. The manager, staff and residents have a good rapport and there is a jolly atmosphere. A varied activity programme is available for residents.

What has improved since the last inspection?

Carpets that needed stretching have been replaced. Contracts include details of the room to be occupied by residents.

What the care home could do better:

Continue with plans to install a wash hand basin in the laundry room. Continue with plans to make a garden path safe.Contact the insurance company for the replacement employer`s liability certificate. A requirement has been made for a copy of the certificate to be sent to the Commission.

Date of last inspection 24th May 2005 Brief Description of the Service: Tripletrees is a care home, registered to provide personal care for a maximum of twenty eight older people to include a maximum of three service users with dementia/mental disorder. Tripletrees is a large detached and extended property, situated in Burgess Hill, being close to shops, local amenities and transport links. The accommodation is arranged on three floors and comprises of twenty four single bedrooms and two double rooms which are served by a passenger lift and stair lift. Residents benefit from a large lounge and dining room on the ground floor. The home is owned by Follett Care Limited and is managed by Mrs M Follett. One inspector started the announced inspection at 10.00 am, which lasted a total of four and a quarter hours. The inspector spent time talking with residents, the manager and staff, walked around the home, and read paperwork. All residents spoken to during the inspection were happy with the care and services provided at Tripletrees. The home was seen to be clean, tidy, warm and homely. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2, 6 Contracts are provided for residents, which provide details of the terms and conditions of residency. No intermediate care is provided at Tripletrees. EVIDENCE: The inspector sampled the home’s ‘admission agreements’ or contracts which contained the terms and conditions of residency. They included the fees to be paid and room numbers. Residents and the registered manager had signed contracts. Intermediate care is not provided at the home. Residents come to the home for permanent, long-term care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 10, 11 Residents are treated with respect and dignity. Residents’ welfare is closely monitored and their needs are well met. EVIDENCE: All residents spoken with told the inspector they were treated with respect and dignity. Any personal care required by residents is given in private and staff are sensitive to the needs of residents. One resident said, “I just ring the bell and they come pretty soon. I think it’s excellent.” Every consideration is given to service users who are dying and changes in condition are monitored throughout the day and night. Relatives are assisted to participate in whatever aspect of care they would find helpful, according to the wishes of the service user. One resident had recently suffered a bereavement and stated that the staff were being very supportive. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15 Residents are respected and treated as individuals. Their individual, social, cultural, religious and recreational interests are respected and promoted. Residents are encouraged to maintain contact with family and friends. A wholesome diet is provided for residents. EVIDENCE: Residents are encouraged to take control over as much of their lives as possible. Examples include food, times for rising and retiring, whether to join in activities and support to attend local clubs. Two residents were looking forward to going out to the local Age Concern day centre on the following day, for which community transport had been booked. One resident spoke of how her daughter and grandchildren are made very welcome when they visit. The activities co-ordinator was facilitating a group discussion about television programmes, e.g. The Two Ronnie’s, Mastermind, Starsky and Hutch. Residents enjoyed naming the actors and the group leader did a very good job including residents in the discussions. Other residents not wishing to join in were happy to sit in the other part of the lounge, or in their rooms. Residents were complimentary about the food provided and spoke of how they were able to choose what to eat. The inspector joined residents for lunch, which was steak in gravy, mashed potato and vegetables, followed by rice pudding. One service user chose to eat soup and one was seen to prefer yoghurt instead of rice pudding. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 There are clear processes for complaints that residents and visitors can use if necessary. EVIDENCE: Residents stated they would feel happy to complain to staff if they had any concerns. No resident voiced any concern during the inspection. One recent complaint was dealt with by the registered manager, which was found to be unsubstantiated. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22, 23, 24, 25, 26 Residents are able to access comfortable communal facilities. Bedrooms are comfortable, clean and tidy, offering accommodation to meet residents’ needs. EVIDENCE: The inspector walked around the home and noted the lounges and bedrooms were comfortably furnished and homely. Some bedrooms are small but residents spoken with were happy with their rooms and took pride in showing the inspector some of their personal possessions that were on display. Carpets identified at the last inspection as in need of stretching were replaced a few days later. The last inspection recommended that the laundry room had hand-washing facilities installed. Mrs Follett confirmed that a wash hand basin was on the list of jobs awaiting the plumber. A garden path has not yet been made safe, but has been cordoned off so that residents cannot use it. Residents are supported to obtain and use any equipment that is appropriate to their needs. Examples include walking sticks, frames, wheelchairs and mattresses. The manager confirmed that the home has a very good working relationship with local district nurses and doctors. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29, 30 A caring manager and staff team support residents. An appropriate number of staff are working at the home. EVIDENCE: The home was seen to be running smoothly and efficiently. The staff rotas were read and the inspector felt that staffing levels were satisfactory, with sufficient staff on duty at different times of the day to meet the needs of the residents. Staff spoken to were clear about what was expected of them whilst on shift. The inspector noted very good interaction between staff and residents during the inspection. Four staff files were sampled and contained the required information. The manager maintains a list of staff with their Criminal Records Bureau details. The inspector noted that the manager maintains a list of CRB checks that are held on staff members. The training folder was read by the inspector. Mrs Follett is responsible for delivering induction training to new staff members. Recent staff training has included incontinence, catheter care and adult abuse/protection. Another resident commented, “The staff are good “ and “Everyone’s friendly.” Attention to meeting residents’ needs is promoted by the caring and attentive staff team. One member of staff had written out a ‘soap rota‘ for a resident, to enable her to easily identify the days, channels and times her favourite programmes are shown on television. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 34 An experienced manager runs the home. The leadership, guidance and direction to staff ensures continuity of care. The home is very resident focused. EVIDENCE: The home is run by an experienced, professional and dedicated manager, who has run the home for over thirty years. Mrs Follett is currently undertaking the Registered Manager’s Award. Staff said that they receive clear direction and consider that communication between management and the staff team is good. They were clear about what is expected of them. The employer’s liability insurance certificate expired on 30/9/05 and the home has not yet received a replacement. Mrs Follett assured the inspector that this had been renewed and she was awaiting the new certificate. A requirement has been made for Mrs Follett to forward to the Commission a copy of the new certificate.
