What the care home does well

Service users described good relationships with the staff and said they were all polite and helpful. Staff were observed to be friendly and relaxed and showed a good understanding of their needs.Arrangements for service users to maintain contact with their family and friends are good. Visitors confirmed that they are always made welcome and kept informed and involved. A variety of social activities were available providing service users with varied and interesting days. Meals are varied, well balanced and nicely presented offering good choice and nutritious food at all meals. All of those spoken to were pleased with the quality and choice available. Mealtimes were seen as a relaxed and social occasion. Hygiene practices were good protecting the health of service users and staff. The home is well staffed with a skilled, consistent and trained staff team giving security to service users. Staff recruitment and training records were clear and concise and contained all relevant information. The vetting process helps protect service users. The staff had a good understanding of service users individual needs. Sixty eight percent of staff is qualified to National Vocational Qualification in Care level 2 (NVQ) or above providing residents with a trained, skilled staff team. The service users were very complimentary about the staff. Seventeen relatives feedback cards were received, the majority were very positive about the care provided, and these included comments such as: "I find the care given to my Aunt is wonderful". "The home is very well managed. The staff are pleasant, caring and any concerns about my Mother is recorded and dealt with fairly quickly". "I feel my sister could not be cared for by a more dedicated staff". " There are regular meetings, and concerns are brought up and discussed at meetings".

What has improved since the last inspection?

The service users care records have continued to improve, and these are regularly evaluated and reviewed. The requirements from the last inspection visit have been met. Several areas throughout the home have been decorated, and new flooring laid, and several bedrooms have been refurbished.

Date of last inspection 21st November 2006 Brief Description of the Service: Wansbeck Care Home comprises of a purpose built residential unit for older people adjoined to an older building, which was originally a vicarage. The older part of the Home contains the ancillary services such as the laundry, kitchen, staff rooms and office facilities. The home is situated close to local shops and facilities in West Sleekburn. There is level access to the front entrance of the home, which is located beside the car parking area. Wansbeck Residential Care Home caters for 40 service users, 30 of whom may have a dementia, 5 of whom may have dementia but are aged under 65 years of age and 10 older people not falling into any other category of need. Fees range from £383.52 to £436.06 per week. There is a Statement of Purpose and Service User Guide in place to inform prospective residents and their relatives about the service provided at Wansbeck Care Home. The Visit: An unannounced visit was made on 16.11.07. During the visit we: • • • • • • Talked with people who use the service, relatives, staff, the manager & visitors. Looked at information about the people who use the service & how well their needs are met, Looked at other records which must be kept, Checked that staff had the knowledge, skills & training to meet the needs of the people they care for, Looked around the building/parts of the building to make sure it was clean, safe & comfortable, Checked what improvements had been made since the last visit. We told the manager/provider what we found. What the service does well: Service users described good relationships with the staff and said they were all polite and helpful. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. Prospective service users are provided with enough information about the service to enable them to make a choice about where they want to live. All service users are appropriately assessed prior to admission into the home. All are provided with a written contract explaining their terms and conditions with the home. EVIDENCE: Details of the extra charges and what these are for, are in the contract given to service users and are agreed prior to their admission. Three service users’ files were checked and each included a full needs assessment completed by the referring care manager. These contained a range of appropriate information, and the service users are involved in drawing up both these initial assessments and the home’s subsequent service user plans. The service user plans checked by the inspector were comprehensive, and listed details of service user’s needs and actions taken by the staff to meet these needs. The service users spoken to said their needs were met and they were happy with the care offered to them. One said, “The staff are excellent, very caring, and respectful, I have no worries living here, all my needs are taken care of”. Staff interviewed had had a range of relevant training and experience. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. This judgement has been made using available evidence including a visit to this service. The care planning system is clear enough to ensure that staff has the information they need to meet the assessed needs of the service users. There are good systems in place to ensure service users medication needs are met. Service users are supported to make decisions about their lives, and take risks to promote their independence. Privacy and dignity are respected at all times. EVIDENCE: There are comprehensive assessments in the service users’ care plans. There is also a comprehensive risk assessment of service users. Each service user has an allocated key worker. Care plans are drawn up with service users and plans are amended and reviewed on a regular basis. There are systems in place that will ensure that the placement and the service users plans are reviewed six monthly and annually. These involve the care managers, service users and their representatives. Service users’ spoken to said that they are able to make decisions for themselves, and that they are happy with all aspects of the care that they receive. Three visitors said that they were extremely happy with the care their relatives were receiving. One said, “my wife is very well cared for here, the staff are very good, I visit daily and I have no complaints”. Another said, “ My Husband really likes it here, he receives very good care. He could have transferred to another Southern Cross home very near to where I live, but he refused because he likes it here so much”. Staff who have completed relevant training administers medication. A sample of medication records was examined. These include service users photographs for identification purposes. Clear directions were recorded and each dose of medication was signed for, or a code entered to verify the reason not given. The Controlled Drugs are not currently prescribed, should this change appropriate procedures are in place. Privacy and dignity issues are built into the home’s policies and procedures and staff training. All personal care and medical examination/treatment is carried out in private. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12 13 14 15. People who use the service experience good quality outcomes in this area. This judgement has been made using available evidence including a visit to this service. Service users are supported to live a normal life in the community. They have regular access to a wide range of community activities, and maintain contact with their family and friends. They receive support and encouragement to enable them to be in control of their own lives. EVIDENCE: Each service user has daily life skills assessment carried out. This is reviewed and updated on a regular basis. All service users participate in this process. The care records clearly identified each service users social care needs, and their interests and hobbies. They are supported and encouraged to be in control of their own lives, and to enjoy their own hobbies and interests. The home has an activities co-ordinator, and the care staff supports her in providing a diverse range of activities both inside and outside the home. All have the opportunity to go out on shopping trips and outings to various places of interest. Last week several attended a tea dance in Ashington. There was evidence around the home that staff continue to work hard to ensure service users have enough stimulation. The home also produces a monthly newsletter and service users are involved in its production. Mind Active are involved in the home, and they provide a range of activities these include: reminiscence sessions, arts and crafts and tai chi classes. All service users are supported to maintain very close links with their families. They can choose who they want to see and when. Daily routines promote independence, choice and freedom of movement. Some service users are involved in housekeeping tasks. At least three hot meals are provided each day. The nutmeg diet has been introduced, and each recipe used is nutritionally well balanced, and it promotes healthy eating for older people. All meals are home made, and each recipe includes appropriate levels and amounts of: fat, fibre, protein, calcium, sugar, vitamins, and iron content. The lunchtime meal appeared appetising and was well presented. Without exception, all service users said that the food was always very good, and that they were always provided with a choice. This judgement has been made using available evidence including a visit to this service. Procedures are in place to protect service users from abuse or harm. All staff has undertaken Safeguarding Adults training. Service users are confident that their concerns/complaints are listened to and acted upon. EVIDENCE: There is a complaints procedure. It contains details of how to contact the CSCI to make a complaint, if complainants are not happy with the homes investigation and response. The procedure is written in a way that ensures service users fully understand its contents. Four service users said that they had been given copies of the procedure and that staff always listened to their concerns and dealt with them fairly. The home keeps a record of complaints. Since the last inspection visit there have been no complaints received. Wansbeck Care Home DS0000000531.V351473.R01.S.doc Version 5.2 Page 16 There has been two safeguarding adult referrals made by the manager, one was regarding a member of staff, and the other relating to conflict between two service users. Both were fully investigated and appropriately resolved. The home has a Whistle Blowing policy and the Local Authorities Vulnerable Adults procedures. Staff interviewed were aware of these, and they were able to describe the action they should take if they had any concerns about service users welfare. The home also has a copy of the Department of Health’s document, “NO SECRETS”. The Home keeps detailed financial records on behalf of the service users, and receipts of personal spending are kept. This judgement has been made using available evidence including a visit to this service. The home provides a comfortable and safe environment for those living there. The standard and decoration within the home is generally good. Some areas still need to be improved. Communal areas and bedrooms are large enough to meet the service users needs. All areas within the home are well maintained, clean, tidy and free from offensive odours. EVIDENCE: Wansbeck Care Home DS0000000531.V351473.R01.S.doc Version 5.2 Page 18 The home was clean, well decorated and well maintained. The home is in a residential location and very close to the village Pub. The grounds were tidy, safe, attractive and accessible. The fire service and the environmental health department had made visits to the home. Requirements made by these organisations had been met. The home has an appropriate amount of sitting, recreational and dining space. There are enough rooms for a variety of activities to take place, including an activities room. Service users confirmed that they are able to see visitors in private in their own rooms. The home is smoke-free, however there are two service users that smoke, and this means that they have to smoke outside, with the dark winter nights approaching, it might be worthwhile considering a designated smoking area within the home. Furnishings and fittings throughout the home were domestic in design and in good condition. Lighting in all the corridors is rather dim, and this may hamper some service users co-ordination, orientation and may also increase the risk falls in the home. Therefore it is recommended that the lighting in all the corridors is improved, additional wall lights might enhance the lighting in these areas. Room sizes meet the minimum required. There is space on either side of beds when necessary, to enable access for carers and specialist equipment. All doors have privacy locks, and staff were observed to knock on doors and wait for an answer before entering. Service users’ bedrooms have opening windows. The rooms were centrally heated and the heating level could be controlled within each bedroom. The bedrooms viewed were nicely decorated and highly personalised. There was emergency lighting throughout the home. The home was clean and free from offensive odours. The laundry facilities are well organised and the washing machines have the specified programme to meet disinfection standards. This judgement has been made using available evidence including a visit to this service. There is a good match of well-qualified staff offering consistency of care within the home. There are robust procedures in place for the recruitment and selection of new staff, which helps to protect service users. The staff receives supervision and this provides them with a good understanding of the service users support needs. EVIDENCE: Staff levels on the day of the inspection met the agreed level. Samples of 4 weeks’ rotas showed the required numbers of staff were on duty: 7 staff between 8am and 9pm with 4 between 9pm and 8am. Staff said that staffing levels were appropriate. All the staff were over 18 years of age and those left in charge were at least 21. Staff interviewed said that they were happy with level of training available to them. As part of the staff induction-training programme, all are provided with dementia care awareness training and managing challenging behaviours, this is good practice. The training programme meets The National Training Organisation requirements for the first six months, and detailed training records are kept. Staff said they receive three days paid training. Two staff files were examined; the service has a rigorous staff recruitment and selection process to ensure that all appropriate checks and references are in place prior to employment. Over 68 of the staff team have achieved NVQ level 2 or above. The manager is supported by the organisation in providing clear leadership throughout the home with all staff demonstrating an awareness of their roles and responsibilities. Wansbeck Care Home DS0000000531.V351473.R01.S.doc Version 5.2 Page 22 EVIDENCE: The manager has many years experience in senior management, she has been employed since January, she is not yet the approved registered manager, this is currently being processed, however she has successfully managed two other care homes prior to this one. Staff were clear about their responsibilities and had very good knowledge of the service users care needs. Staff spoke positively about the manager saying she had encouraged both staff and service users to contribute to the development of the service. One said that, “the care provided is very person centred and that this was promoted throughout the home”. Service users are told when inspections take place and they are shown inspection reports. These are also summarised and discussed in service user/relatives meetings. Copies are available for relatives and others to see. The records inspected were found to be appropriately completed. These included the fire log book, accident records, personal allowance records and Health and Safey manual. There are appropriate maintenance contracts for the home. Water storage tanks, gas and electrics are checked annually.
