It is registered to provide nursing and residential care for up to 30 older persons. The home offers single and double room accommodation, with communal sitting/dining areas. Bathrooms are located throughout the home, some of which have aids to support service users during personal care. West House is situated in Chester-le street, a town near to Durham and Newcastle. This is an overview of what the inspector found during the inspection. CSCI has a statutory duty to inspect all care homes at least twice a year. This unannounced inspection was carried out in accordance with this duty. The inspection took place on the 28th September between the hours of 9.45am and 2.45 pm. During the inspection, time was spent talking to seven residents, four staff and two relatives. Records were examined and a tour of the building took place. In line with current CSCI policy on Proportionality, the inspection focused upon a number of key standard outcomes for service users. The key standard outcomes not inspected on this occasion will be raised during the next inspection of the home. Issues raised in the last inspection were also examined. Six service user comment cards and four relative comment cards were received, feedback from which is included within the inspection report. What the service does well:
Each service users has an individual plan of care, which provides staff with the information they require to meet the service users needs. Medication is administered to service users by a registered nurse, although service users would be supported in administering their own medication where they were able. There were no odours present and the home was clean and tidy. Plans have been submitted to improve the premises. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 A contract is required to tell service users what the home is providing within the fees paid. EVIDENCE: There is no contract, which will tell prospective service users what the home is providing, what the fees are and what is included within the fees. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8 9 and 10 There is a care planning system in place, which provides staff with the information they need to satisfactorily meet service users’ needs. There is some evidence of service users’ involvement within these plans. Medication systems and practice supports service users. Service users privacy is maintained. EVIDENCE: Discussion with staff working within the home confirmed that a high standard of care was being provided for service users, some of the care staff spoken to felt that the care had improved and that more time was now available to spend with service users. The home is trying to involve service users/relatives within care planning; this is being evidenced within the individual care plan. Comments from service users included “I’ve nothing to grumble at – I’m well looked after, no problem” and “They look after you well here – no improvements that I can think of”. One relative commented, “The care is high, and mum is clean neat and tidy, changed immediately if she spills something, she gets well fed”.

A qualified nurse administers all medication. All service users spoken with confirmed that they were treated with dignity and respect, staff knock on doors prior to entering and staff were observed throughout the inspection speaking to service users in a respectful and caring manner. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 13 14 & 15 Activities should be provided which are based on service users likes and dislikes. Menus are varied and comments regarding the food provided within the home were positive. EVIDENCE: Comments regarding the activities being provided were mixed; and included “there’s not much to do at all”, the activity co-ordinator post is currently vacant. Relatives commented, “I am made welcome and can visit at any time, I am able to have a meal”. Service users spoken with confirmed that they were able to make choices in all aspects of daily living, comments included “I get up and go to bed when I like, staff are very helpful” and “I can make choices in all aspects of my care”. “The food is usually pretty good, the chef will cook something else if I don’t fancy what is on the menu”, “it’s good food” and “I like the cooking here, it’s better than mine”. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Systems are in place to protect people from abuse. Policies to protect service users need updating in some areas. EVIDENCE: Service users spoken with during the inspection said that they had no complaints. The adult protection procedure needs to be updated to ensure that it links with the local authority procedure in the event of any allegations being made. The current policy makes reference to the home carrying out its own investigation; it does not reflect that the home must refer allegations of abuse to Social Services. All staff receive in-house training on adult protection. The home has a policy on making a referral under the protection of vulnerable adults (POVA) list. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 There are plans in place to extend and improve the environment. The home was clean tidy and free from odour during the inspection. EVIDENCE: The home has submitted plans to the council to extend the premises; the owner should provide CSCI with a detailed plan of redecoration, which will take place over the next two years. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 & 30. Staffing numbers appear to be sufficient to meet the assessed needs of service users. Training is provided for all staff. EVIDENCE: Ten of the seventeen care staff working within West House had achieved the NVQ level 2 award. The staff are given a copy of the General Social Care Council’s (GSCC) code of conduct. There is a vacancy for one full time carer, and a night duty nurse as well as an activities co-ordinator. One carer stated, “ Things have improved loads, supervision and training are taking place and morale is high, the staff numbers are ok, they have improved”. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): EVIDENCE: None of these standard outcomes were formally assessed on this occasion, however concerns were raised regarding the smoking arrangements within the home. The home should have a smoking area for service users, which does not pose a risk to non – smokers. One visitor has an oxygen cylinder, which is being used, in an area where people smoke.
