The home must at all times employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. 9th August 2005 Date of last inspection Brief Description of the Service: The home is a two storey purpose built care home in the centre of the town of Garstang, where there is easy access to local transport links. There are attractive gardens at the rear of the property, with lawns, patio area, flower beds and a water feature, where residents can sit and enjoy the sunshine. There is a small car park at the front of the home, and easy parking in the roads alongside the home. The home provides personal care for older people, and is equipped to suit the needs of its residents. For example, a stair lift has been installed, and ramps are provided for wheelchair access. There are fifteen single and two double rooms, most of which are ensuite. Toilets and bathrooms are conveniently located. There is sufficient communal space, made up of a lounge, dining room, and a conservatory. The home has a no smoking policy. Staffing is provided over 24 hours, every day of the year. The inspection took place over 2 hours, and was one of the two inspections which must be made each year. Additional inspections may be made if necessary. The inspection was unannounced, in that the owners were not aware that the inspection was to take place. A tour of the home was made, and rooms were inspected at random. Staff and some records were viewed. The manager and three staff on duty were spoken to. What the service does well: What has improved since the last inspection? Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 The medication at this home is well managed, promoting good health EVIDENCE: All medication is stored securely in a dedicated trolley, which is attached to the wall in the dining room. A locked cupboard is also used in the staff office. Medication records were available for all residents, and were seen to be clear and up to date, with photographs of residents identifying these records further. The manager stated that she regularly monitors the records to ensure they are complete. Only named, trained, senior staff administer medication, and the records listed the staff members who were to do this. Those spoken to were able to discuss the correct way to administer and record medication. Patient information leaflets are kept for staff information on each individual’s medication, and there is regular liaison with the local pharmacist. Westwards House Residential Care Home DS0000061814.V275587.R01.S.doc Version 5.1 Page 10 It was evidenced with signatures that new medication arriving at the home is always checked on receipt, and any medication which needed returning to the pharmacist was listed and signed for. A risk assessment would be carried out on any resident wishing to self medicate, and they would need to sign a disclaimer. One resident looks after their own medication at present, keeping it in a secure place in their own room. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 The standard of the environment in this home is good. It provides residents with an attractive, homely and comfortable place to live. EVIDENCE: There is a well-maintained and protected garden area at the rear of the home, with lawns, seating and a water feature, making it an extremely pleasant spot to sit in the warm weather. Photographs were seen of the garden party held there earlier in the year, with residents and visitors enjoying the summer sun. The environment is very homely, comfortable and warm. The front lounge, which overlooks the road into the town centre, has comfortable supportive seating, with bookshelves holding books of a variety of subjects for anyone to read. Residents in there were chatting or reading quietly. The second lounge holds a large settee and easy chairs, and a television and video recorder, along with a large selection of videotapes to choose from. A computer stands ready for any resident to have a go. The large conservatory overlooking the garden Westwards House Residential Care Home DS0000061814.V275587.R01.S.doc Version 5.1 Page 14 gives another alternative place to sit and enjoy the view, or the company. The fish tank in there is always a topic of conversation. The bedrooms were very cosy and personalised, with matching décor and bed linen making a final touch. Residents own ornaments, pictures, and in some cases small pieces of furniture, made each room special to its owner. “They keep me nice and tidy”, commented one resident, “I do my best as well. I can’t think of anything I’d like done differently. I’m happy here”. Staff record any maintenance needs they find, and the maintenance man deals with them as soon as possible. The records viewed showed the date the request was recorded, and the task was generally signed as dealt with in the next couple of days. The manager stated that it was planned to start a rolling programme to refresh the home this year, in order to keep up the high standard of decoration the home has had. The laundry area is away from food storage and preparation, and was clean and tidy, with laundered, named, clothing hanging ready to be returned to its owners. This area is close to the dining area, but staff spoken to were aware of the correct way to work to prevent and control infection. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 There is a clear training plan for all staff in the home, ensuring they have the knowledge and skills to care for residents in a safe way. EVIDENCE: The records were viewed, and showed that there is a commitment to training staff, with ongoing training for staff in relevant areas, such as moving and handling, food hygiene, nutrition, and dementia care. Staff are enrolled on National Vocational Qualification (NVQ) in care training, and presently of the fifteen staff, two hold the award, three have nearly finished the course, and two more are due to start. Two overseas staff hold care qualifications. The home continually meets the standard required of 50 of staff NVQ (or equivalent) qualified, and purely through trained staff occasionally leaving the employ of the home, does the level drop, until new staff are trained. Presently this figure stands at 26 , rising to 60 once all have completed their training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 The manager provides clear leadership, demonstrating an awareness of her role and responsibilities in the home. EVIDENCE: The manager has many years experience of the provision of care, both in a care home environment, and in people’s own homes. She has achieved National Vocational Qualification Level 4 in care and management, and is also an NVQ Assessor for care staff undertaking NVQ qualifications. She stated that she likes to keep up to date with information, and makes sure she attends relevant courses regularly. She clearly makes herself accessible to staff, residents and visitors alike. She assisted with the lunchtime medication, chatting and encouraging staff, Westwards House Residential Care Home DS0000061814.V275587.R01.S.doc Version 5.1 Page 17 praising the cooks lunchtime sweet of home made treacle sponge. She helped a member of staff who was trying to adjust a resident’s hearing aid. Residents approached her easily and informally with requests, and were listened and responded to in the same manner.
