What the care home does well

The home has a particularly happy and relaxed atmosphere, and benefits from a really committed staff team. Residents are given a good degree of choice and freedom, and staff are given good support to develop their skills. Residents were evidently extremely happy with the service they receive. They described flexible support from staff and were enthusiastic about the level and type of activities available. Most of the outings and activities are free of charge. Residents said that the food is generally very good and that they get good support with their personal care needs. Comments residents made about the staff included, "They`re all so friendly", "It`s lovely here - they`re all very nice", and "They`re very attentive".

What has improved since the last inspection?

The home has a rolling programme of redecoration and refurbishment, which helps to ensure that everywhere looks welcoming and fresh.

What the care home could do better:

Although the environment is generally very well maintained, the lock on the door of the top floor toilet and bathroom was not working and needs to be repaired. The system for carrying out staff checks needs to be tightened up. There was no evidence on file that the pre-employment criminal record check had been carried out on a new member of staff. The manager designed a checklist during this inspection to be kept on the front of all staff files as a prompt to follow up any outstanding issues before the person starts work in the home.

This condition is specific to one person and will lapse when the person reaches 65 or leaves the home. 20th July 2005 Date of last inspection Brief Description of the Service: Wyvern Lodge is registered for up to 16 older people. It is situated in a residential area, near accessible local amenities. The bus stop to the town centre and sea front is just outside the home. Wyvern Lodge provides day care for up to 5 people each day. Most of the accommodation is in the original part of the building but some rooms are in a new ground floor wing at the rear. The home has a passenger lift. There is a small, secluded garden with seating. This unannounced inspection was carried out over the course of one morning and focused on spending time with the residents, discussing their quality of life at the home. The inspector also spent time with some of the staff on duty, and with the manager and her deputy, and sampled some of the records relating to residents personal care and to staff pre-employment checks. The inspector spoke to several of the residents individually and in the privacy of their own rooms, but also spent time in the lounges with different groups of residents. Everybody was really positive about the home, and residents were warm in their praise of staff. Several people gave the inspector examples of the way staff have gone above and beyond the call of duty to make sure they are happy and comfortable. Residents are regularly consulted about all sorts of aspects of their lives, and the management of the home is particularly transparent. There is some sort of in-house activity every weekday, and regular trips out are also organized. Staffing levels allow staff to spend time chatting with residents and to work creatively. The environment is very comfortable and homely, and basic provisions such as food and toiletries are high quality. The records seen were generally up-to-date, clear and informative. Care plans and task lists give clear guidance to staff on what they should be doing. Staff employment records need more detail, and the manager took action during this inspection to address this. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 10 Residents care needs are well documented and well met. EVIDENCE: Care plans and associated documentation are excellent, giving really clear information on each aspect of the persons life, and helping to promote good and consistent practice among the staff team. All of this documentation is being reviewed every month, and any updates are clearly noted. Residents are fully involved in drawing up their care plans and agreeing how their needs will be met. A good range of risk assessments is in place for each person. Most of those sampled indicate low risk. Where a higher level of risk is identified, the assessments included the actions to be taken to reduce the risk to an acceptable level. The more dependent people all looked well groomed and comfortable. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12-15 Residents are well supported to follow the lifestyle of their choice, and get good opportunities to join in a wide range of interesting activities. EVIDENCE: Personal care support is given at times and in ways that suit residents own preferred routines as far as practicable. One person described how staff had stepped up the level of support offered during a period of illness, tactfully ensuring she had all she needed without making her feel her independence was being compromised. Many of the residents and staff with whom the inspector spoke said how happy they are at the home. People feel valued and treated as individuals. The inspector was given several examples of how the management staff have worked with individual staff to boost their confidence and skill. A good range of activities and outings is laid on, and residents are kept informed by means of regular meetings and colourful posters on the noticeboard. The activities and outings organized by the home are free of charge: residents are only occasionally asked to contribute towards the costs of any additional trips they request. Exceptionally thorough records are kept of all the activities offered. Residents were very positive about the choice and quality of the meals offered. A late evening sherry is routinely offered to those residents who are still up. Food supplies in the fridge and cupboard showed that good quality provisions are used. The linen store and household goods stores showed that other essentials are also high quality. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19-21, 25-26 The environment is homely and well suited to residents needs. EVIDENCE: The whole home has been redecorated over the past couple of years, and all areas seen were fresh and welcoming. A regular programme of maintenance and improvements helps to keep the home environment pleasant and comfortable. There is a pleasant main lounge, leading onto a sun lounge and dining room. As one resident said, its nice to have the two lounges - you can sitting here in chat if you dont want to watch the telly. There is a toilet and bathroom on each level of the home, and three of the bedrooms have an ensuite toilet. Each bedroom is above the minimum size of 10 square metres and is fitted with a safety lock. There is a small patio garden towards the back of the home with ample seating and shade. Wyvern Lodge - WSM DS0000055608.V277987.R01.S.doc Version 5.1 Page 14 All bathroom and toilet locks worked with the exception of the one on the top floor. The manager said that she would have this repaired promptly. The staff sleeping-in room doubles as the hair dressing room so that residents can have their hair done away from the public areas of the home. Low surface temperature radiator covers are fitted to all radiators, and every window is fitted with a restrictor. The hot water outlets that are accessible to residents are fitted with temperature regulators. All areas of the home were very clean and fresh, and several residents confirmed to the inspector that this is the normal standard. The cleaner was well aware of how to use cleaning chemicals safely, and a copy of essential information on the cleaning products in use was kept accessible in the firstfloor linen cupboard. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 & 29 Staff levels are ample to meet residents’ needs but recruitment practice is not sufficiently thorough to ensure residents’ safety. EVIDENCE: Staffing levels at the time of this inspection reflected those on the rotas. There were two senior staff on duty, a carer, a cleaner and a cook. Residents did not have to wait for help, and no-one was under pressure to get tasks done in a hurry. Staff went about their tasks efficiently but taking time to pause and chat with residents. The carer engaged residents in a game of catch after morning coffee, and managed to skilfully make people feel included. Everyone enjoyed the game and the amusement it provided. Most of the pre-employment checks required on prospective staff are being carried out and recorded but there was no evidence on file that the preemployment criminal record check had been carried out on a new member of staff. Although the deputy manager said that she had phoned to request the PoVA First check, no note had been made of this, and there had been no follow-up when the check was not returned. This staff member had been allowed to start work in the home without this vital check being satisfactorily completed. The inspector advised today that a record is kept of the dates all checks are sent for and returned, and reminded the manager that staff must not work in the home without all pre-employment checks being satisfactorily completed.
