Aden House is a purpose built home situated in Clayton West close to the main Wakefield Road. The home offers personal and nursing care for a maximum of 60 older people; it is also registered to accommodate up to 4 adults with physical disabilities and has a 20 bedded unit caring for elderly people with dementia. This Unit is known within the home as The Butterfly Unit. Aden House is situated on the same site as its sister home Aden Lodge, and is within a short walking distance of the main bus route on the Wakefield Road. What the care home does well Feedback from people living in the home spoken to on the day of this visit was positive. During this visit staff were observed to maintain the privacy and dignity of people. Staff approached and spoke to people in an appropriate manner. The atmosphere in the home was calm and relaxed with friendly staff going about their work in a competent manner. Staff are working towards The Gold Standards Framework Award, which will give the staff the necessary knowledge and skills to look after people who need end of life care. As a tribute to the standard of food provided the home has received a four stars, "Very Good" award in association with Kirklees Council`s Health Choice Award. "Scores on the doors". The award is a five star rating scheme providing information about the standards of hygiene and compliance with legislation in food premises within Kirklees. What has improved since the last inspection? The care plans identify the level of support and care that each person requires ensuring their health and welfare needs are been met. Staff that are responsible for the administration of medications are trained to administer medication safely. To ensure that the home is run in the best interest of people who live there, satisfaction surveys are now sent out each year. The recent surveys are in the process of being collated, and when completed will be displayed on the notice board in the entrance of the home. What the care home could do better: There was an odour detected on the Butterfly Unit which must be removed. The lighting on the unit is rather gloomy and would benefit from additional lighting. The quality rating for this service is two star. This means the people who use this service experience good quality outcomes. This report refers to an inspection, part of which included an unannounced visit by two inspectors on the 22nd July 2008, commencing at 8.30am, and the length of the inspection was 7 hours. There were 56 people living at the home on the day of the visit. As part of the inspection in order to provide information to help us form judgments about how the service is run, the manager was asked to complete an annual quality assessment document. This she did, and the document provided the Commission for Social Care Inspection (CSCI) with a lot of information about the way the home is run, and what they hope to achieve in the future. During the visit we spoke with members of staff and people who receive care to obtain their views. We also looked at a sample of care records, staff recruitment, and training records, quality assurance audits and looked around the home. To enable people who use the service to comment on the care it provides, we sent ten surveys to people living at the home, and nine were returned, ten to relatives/advocate/friends, three of which were returned, and five to local doctors and health care workers (social workers, community nurses), none of which were returned by the time of the inspection. We would like to thank all the people who gave feedback about this home, and would like to thank the manager and staff for their co-operation throughout the inspection process. People are assessed prior to them moving into the home and are able to visit the home to establish whether or not it is the right place for them. EVIDENCE: The information received from the home prior to inspection said that pre admission visits are encourage, and where this is no possible, then photographs of the home are provided. The information also said that prior to admission all people are assessed by the Home Manager and nursing staff to ensure that the home is able to meet their needs and this was confirmed by the Manager at the time of the visit. People living in the home said that they received enough information about the home before they moved in so they could decide if it was the right place for them. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7 - 10 People who use this service experience good outcomes in this area. This judgement has been made using available evidence including a visit to this service. People’s care plans outline the level of support and care each person requires in ensuring their health and welfare needs will be met. People are treated with respect. The recording and administration of medication is satisfactory. EVIDENCE: Surveys sent to relatives asked if the care home meets the needs of their relative/ friend. One responded, ‘always’ and two responded, ‘usually’. Relatives were also asked if their friend/ relative received the care and support they needed. Four people said that they did and five people said that they usually did. Everyone said that the staff listen to what they say and that they receive the medical support that they need. Four people’s care records were looked at in detail and the documentation clearly identified the care needs, and the level of support the person requires. There were also movement and handling assessments, nutritional assessment, skin integrity, social interests, religious and cultural needs, and people’s likes and dislikes recorded. There was evidence that the care plans are updated monthly or as the needs of the person change. The healthcare needs of people are met. Evidence was seen of involvement from people’s doctors, physiotherapists, opticians, dentists and other healthcare professionals where needed. Three relatives said that they were kept up to date with important issues affecting their friend/ relative i.e. an admission to hospital. People who use the service confirmed that they receive the medical support they need. In relation to privacy, we were shown evidence that the Manager intends to use door handle signs showing when the person does, or does not wish to be disturbed. Staff are working towards The Gold Standards Framework Award, which will give the staff the necessary knowledge and skills to look after people who need end of life care. People who live at the home were asked in the surveys, ‘Do you receive the care and support you need?’ Six people said that they did receive the care and support they needed, and four people said that they usually did. In February 2008 the CSCI Pharmacist inspector visited, and she recorded examples of good practice for people who self administer their medication however, there were poor systems for the accurate administration and recording of medication. On the day of this inspection, a sample of three people’s medication was checked, and the storage, administration and recording of the medication were done correctly. There was also evidence that a weekly audit of medication take place to ensure that the medication is being given correctly. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12 – 15 People who use this service experience good outcomes in this area. This judgement has been made using available evidence including a visit to this service. People living at the home are offered a range of activities and are able to make their own choices about how they spend their time. Meals provided are good, varied and served in a pleasant environment. EVIDENCE: The home has a full time activities person who works Monday to Friday each week. The home is a member of the National Association for Providers of Activities for older people, which is about raising awareness of activities and understanding in care settings. The activities person has also attended an activities training event where they were provided with some activities equipment for the home. There was evidence in the records to suggest that the services offered to people ensure that they can get involved in activities and events that match their preferences, and this satisfies their social, cultural, religious and recreational interests and needs. Records were seen of the activities that take place and include, Skittles, card games, dominos, reading, karaoke, quiz’s, ball games, listening to singing. Entertainers visit the home on a regular basis and there was evidence of this in the documentation seen. There are areas around the home where people can sit and relax, and on the unit that specialises in dementia care, for the relaxation of people living there, the quiet lounge has been changed to a relaxation room. The Manager said that newspapers and magazines can be ordered upon request and that the home also has three daily papers delivered. The library visits monthly, and they deliver large print books, and for those people who have sight impairment, taking books are delivered. The menus were displayed on a notice board in the entrance of the home, and included the main dish of the day and a healthy option. The carers ask people on a daily basis what they would like for their meals that day, and the manager said that should someone wish to have a diet specific to his or her culture then one would be provided. Food preferences and diets are also taken into consideration when planning the menus. Birthdays are celebrated, and on the day of the visit one lady was celebrating her birthday with a cake that had been provided by the home. This judgement has been made using available evidence including a visit to this service. People who use the service can be confident that their complaints will be listened to and taken seriously. People are protected from abuse. EVIDENCE: There is a complaints procedure and the Manger said that it is usually displayed in the entrance of the home however, on the day of the inspection the procedure had been removed from its usual position. The Manager said that she would ensure that the procedure is replaced. People, who use the service and their relatives, said that they knew how to make a complaint and knew who to speak with if they were not happy. The manager keeps a log of any complaints received and there was evidence that people’s concerns had been taken seriously and the appropriate action had been taken to resolve the matter wherever possible. According to the information received from the home prior to the inspection, they had received two complaints in the last twelve months. The home has a whistle blowing procedure, and staff that were spoken with were aware of the procedure to follow. Records showed that all staff have had safeguarding (adult protection) training, which provides information on how to protect people from abuse. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26 People who use this service experience good outcomes in this area. This judgement has been made using available evidence including a visit to this service. The home offers people a homely and comfortable environment. EVIDENCE: As part of the inspection a tour of the home was conducted which included the communal areas, a number of people’s bedrooms, and the laundry. Five people who use the service said that the home is always fresh and clean and four people said usually. There was an odour detected on the Butterfly Unit which could be rectified by more appropriate flooring. The lighting on the corridor was rather dim and would benefit from additional lighting. The atmosphere on the day of the visit was warm and friendly and people looked comfortable whilst sitting in various parts of the home. People said that they were satisfied with their rooms and had everything that they needed. This judgement has been made using available evidence including a visit to this service. People are supported by qualified, competent and experienced staff in sufficient numbers and that has had all the necessary checks before working with people so that they are kept safe. EVIDENCE: The information received from the home stated they are staffed efficiently to meet the needs of the people in their care, and evidence confirmed this when we looked at the duty rotas and spoke with staff. People who use the service also said that there were staff around when they need them. Evidence was seen to suggest that care staff are encouraged to have an NVQ level 2 in care, and the manager confirmed that sixty five percent of care staff have the qualification. Staff recruitment files of three members of staff were looked at in detail and they contained the required information and employment checks. These checks are necessary to help protect people from potentially unsuitable staff. The staff that were spoken with confirmed that they had received induction training when they came to work at the home, and training in dementia care. The home is run in the best interest of people who live there. EVIDENCE: The manager has a number of years experience in the care of older people, and she has achieved the Registered Managers Award.
