Ascot Lodge is a Care Home providing nursing care and accommodation for eighteen older residents. Care Homes for Older People Page 3 of 8 What we found: We conducted a site visit to the home on 24th May 2010. The reason for this visit was in light of concerns that have come to our attention. These have been in respect of the overall management of the service. This includes the use of CCTV cameras in the home, the lack of social activities activities and how shifts would be covered, as a number of staff have left. We were also informed that a number of residents have taken up accommodation in other homes. People accommodated at the home like to be called residents. This term is therefore used in this report, as a mark of respect. At the time of our visit we spoke with the nurse in charge and also the new manager, Mrs Tracy Thomas, about the management of the home. Mrs Thomas started her employment on 16th May 2010. We looked at some care, service and staff records and also met with staff and residents to gain their views, as to what it is like to work and stay at the home. We viewed certain areas, as we had been advised that a few of the bedrooms had been affected by damp. We were informed that the CCTV cameras have been disabled and are not in use. We found this to be the case. Following our site visit, the manager informed us that they had been removed. The staffing rota showed us that there were sufficient numbers of staff on duty to provide care and support to the residents. At the time of the site visit, there were six residents with a registered nurse and two care staff on duty. We were told that there are no planned admissions. The number of people accommodated remains very low. The nurse in charge was undertaking kitchen duties and cooking the lunch time meal for the residents. This was in conjunction with her duties as a registered nurse. The nurse was undertaking this work, as the home requires a part time cook for two days a week. A full time cook works the remaining five days a week and has the necessary food hygiene certificate. The nurse currently requires a certificate in food hygiene if she is to continue working in the kitchen, until a replacement cook can be found. This is to help ensure she has the necessary skills and knowledge to handle food safely. We asked to look at Regulation 26 reports. These must be completed by the owner or a person is who not in day to day charge following a formal visit to the home. These visits should include meeting with staff and residents, viewing different areas of the home and looking at number of care, service and staff records. This helps to ensure the home is running efficiently and effectively, so that people are well cared for and the staff are supported. We were only shown reports up to March 2010, however the owner subsequently informed us that he has visited the home and produced a report. These are now being sent to us for our information; this will help us to monitor the service. We discussed the social arrangements in the home. The managed confirmed that this needed to be improved as there does not appear to be any formal programme in place for future events. There was a record of in house entertainment to early May 2010. A member of the care staff was previously helping to sort out the entertainment, however Care Homes for Older People Page 4 of 8 with staffing and resident numbers being reduced this has temporarily stopped. We were advised that DVDs have been purchased for staff training purposes. We discussed their use with the manager who confirmed that they would be used in conjunction with practical demonstrations. The manager has noted that staff now require training in safe working practices, for example, moving and handling. She confirmed that to date there is no formal training plan in respect of this. We saw evidence that one new member of staff had not received moving and handling and another persons certificate had expired. The manager said that she would be arranging fire prevention training for the summer as this was due shortly. We discussed with the manager how the residents care is recorded. When looking at the two care files we found limited information in respect of how the staff were reporting on wound care. The manager agreed that this needed to be improved to help evidence the treatment being given. The manager is in the process of completing care reviews to update the information recorded. We had been advised that two bedrooms were affected by damp. We found that the wallpaper was peeling off the wall in one of bedrooms, however the room is currently not in use. The manager said that the owner is aware and that the room is going to be redecorated once the necessary has been carried out. We spoke with two residents and they confirmed that they were happy with the care they were receiving.
