What the care home does well The home is run by a committed and competent manager and deputy manager who have created an open and positive atmosphere whereby residents` feel confident and well cared for. Health and safety is generally taken seriously to make sure that the people who live and work at Coneygar House are safe. However, risk assessments must be reviewed in a timely fashion. The people who live at the home inform us that staff are kind to them and treat them well. They said that their individual rooms meet their needs and that the food is home cooked and of good quality. People important to those who live at the home say they can visit at any time within reason and consider that the staff at the home keep them informed of significant events as appropriate. What the care home could do better: The registered manager needs to look at improving the approach to care planning, on going assessment and review of the needs of those who live at the home so that people do not have unmet needs. The home must improve its medication practices with regards to the use of medication on a " when required basis" so as not to put people at risk of harm. The registered manager must ensure that all risk assessments are kept up to date and that systems are introduced to ensure that people are not put at risk through poor health and safety procedures, systems and risk assessments. The home provides personal care for up to 22 people aged 65 years and over. Residents accommodation is provided in four buildings grouped around a courtyard with a separate building for the laundry and kitchen. The Lodge has 9 bedrooms, (including 8 with en suite hygiene facilities) a bathroom, and a communal lounge/dining room. All the rooms and facilities are on the ground floor. The Stable Block has 3 en suite hygiene facility ground floor bedrooms. The House has 6 bedrooms, including 4 with en suite facilities; a stair lift has been recently installed. Care Homes for Older People Page 2 of 8 Brief description of the care home There are 2 separate bathrooms and a communal conservatory/dining room. 3 bedrooms are on the ground floor and are fully accessible. A further 2 first floor bedrooms known as the Flats have en suite facilities and are accessed via an outdoor ramp or indoors by stairs. The property and gardens are well maintained and the garden has seating for the use of residents and visitors. Much of the garden is fully accessible to wheelchair users. Weekly fees range from £462 - £525 with additional charges for hairdressing & chiropody plus market price for other commodities i.e. newspapers. The home has a suitably equipped laundry. Coneygar Lodge is a short walk from the centre of Bridport where major bus routes and local amenities are available. The home provides suitable transport to take small groups of residents out for afternoon tea or visits to local places of interest. Care Homes for Older People Page 3 of 8 What we found: The inspection took place over the course of one day and was undertaken by one compliance Inspector . The purpose of the visit was to ensure that the National Minimum Standards were being maintained at the home. We looked at the care files in relation to 3 residents including their Medication Administration Record charts(MAR). In relation to the care files the most recent residents file contained a good pre assessment of need together with a number of risk assessments. We spoke with them about their experience of moving into the home . They informed us that they felt that they had been given good information as to what the home could offer. They told us they were settling in well and had started to make friends. They also told us that staff had been helpful which made the transition into the home easier. The staff who we talked with were able to tell us about the persons needs and how they should be met. The other two care files we looked at contained a working care plan which was based on the individuals original initial assessment. As they had been at the home for over a year we would have expected to have seen a full review of the persons care plan so it reflected their current needs and aspirations. Although the full review had not taken place the staff were fully aware of the persons changing needs and how they should be met. However one person had complex behaviours that were not fully documented. Again the staff were able to articulate how to care for the person but the lack of detailed records and action plan may mean that the person does not receive a consistent approach when having their needs met. The MAR charts were kept in good order, all handwritten entries were double signed and a good audit trial exists with regards to the medication administered. In order to improve in the area of medication administration the home need to ensure more detail is included in instructions to staff with regards to administering medication on a when required basis. Our findings showed that whilst staff appeared to have knowledge of individual issues and when it was appropriate to administer medication when required it was unclear how this was being robustly managed. We looked the environment that people live in. Coneygar lodge is split into several small houses with a central house and lounge should people wish to congregate as a group. The people who live at Conygar lodge expressed satisfaction with their living arrangements. The rooms we saw were highly personalised, some residents are able to bring pets in with them. All people who live at the home have a personal evacuation plan so that in the event of a fire they can be assisted to safety in a way that does not cause unnecessary harm to either them or those that are assisting. During our tour of the buildings we noted three issues that will require attention. Not all radiators were covered, not all windows had the openings restricted (when looking at risk assessments later these were found to be out of date) and infection control policies were not being adhered to, for example cloth towels were found in the bathrooms. Whilst paper hand towels were available to prevent any cross infection people should bring their own bath towels to the bathroom and take them out when they have finished their bath. Care Homes for Older People Page 4 of 8 The final area we looked at was in relation to staff recruitment. We found that prospective staff undergo a series of checks to establish their fitness to work at the home. Once employed new staff undergo a detailed induction and have opportunities to train and develop their skills.
