The home is situated in the village of Kington St Michael in a large building, parts of which date back to 1660. The home is registered for older people and older people who have had a diagnosis of dementia. The home is adjacent to the pub and the village provides many amenities. This is one of two homes in Wiltshire owned and run by independent providers, Mr and Mrs Tirbhowan. Mrs Tirbhowan is the registered manager. During the waking day thereAnnual Service Review is a minimum of two staff who are involved in cooking and cleaning as well as care. There are two members of staff sleeping in at night. There is are steps from the pavement to the front door although there is level access to the rear of the property. All the bedrooms are single accommodation and have ensuite facilities except one, which has its own nearby private toilet. On the ground floor there is a large sitting room, separate dining room, kitchen, toilet, laundry facilities, one of the bedrooms and an office. The first floor is accessed by two sets of stairs, one with a stair lift. To the rear of the home there is a garden with a large level patio. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received and asked for, since the last key inspection of 11th January 2008 and the Annual Service Review of 5th January 2009. We looked at their annual quality assurance assessment (AQAA); this is their own assessment of how they are doing, that focuses on how well outcomes are being met for people using the service. We looked at surveys returned to us by people using the service and from other people with an interest in the service. We looked at how the service has managed any complaints. We looked at what the service has told us about things that have happened in the service; notifications that are a legal requirement. We also looked at our last report on the service, relevant information from other organisations and what other people have told us about the service. What has this told us about the service? The home sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and gave us all the information we asked for. The home told us about changes they had made as a result of listening to people who use the service: communal areas and bedrooms had been redecorated; board games, card games and a dart board had been provided; a wireless internet connection installed and the main bath had been upgraded. Further improvements to the environment included: new carpets to bedrooms, all radiators were guarded to reduce the risk of scalding, a new call alarm system was installed and the flooring to the kitchen and laundry room had been replaced. Other improvements in the last year included: two staff members had nearly completed NVQ Level 3 in care. All staff had completed training in dementia care, nutrition and health. Staff were undertaking a course in palliative care. We looked at the information in the AQAA and our judgement is that the home is still providing a good service and that they know what further improvements they need to make. People living in the home always tell us that they are happy there. One comment was Look after us well. The home is well run. Another person told us Best food. Best care. A third person told us I would not be here if I didnt enjoy living here. I get all the support I need. I love the food. I have quiet time when I need it. I have plenty of opportunities to go out if I want to. One of the relatives told us Sashtee and her staff always have time to spend with mum especially during the periods when she was most anxious. Kington House has been a godsend to all of us (myself, my brother and most importantly my mum). Another relative told us Monitors mums health issues promptly. Also recognises health issues immediately. Appropriately communicates with mum who increasingly speaks her native language. Able to understand mums needs even though mum cannot say it in English at times. Provides plenty of fluids and food, provides specific preferences for mum. Another relative told us They all take into consideration mums special needs in communication. All the staff are very patient with her. Just to say what a great service the staff provide for mum as we all know she needs constant care. A further relative said They are very caring and has a family feel which is ideal Annual Service Review Page 4 of 6 for mum who is normally shy. Very accommodating. Of all the homes we looked at this is by far the best for mums situation. Some comments were about communication in a different language and diet from a different country. We asked Mrs Tirbhowan about these comments. She told us about difficulties in obtaining an interpreter with a particular dialect. Mrs Tirbhowan told us that the home had a list of key words and a dictionary, which staff used to communicate. The home had incorporated different dishes into the menu. Mrs Tirbhowan told us about cooking dishes associated with the country as an activity which also involved family. Staff members told us that they all worked well as a team. One of the healthcare professionals told us The service has a good understanding of the service users needs. They have built a trust relationship with the service user and are able to respond effectively to their needs through their person centred approach to care. They actively support social inclusion. They are always very welcoming and easy to work with and supportive in keeping me informed. Sashtee and her team are a pleasure to work with. Another healthcare professional told us when we asked what the home did well Person centred care. Liaison with statuary services. Maintain a calm home environment. Flexible I have no problems with the care standards at this establishment. The home continues to let us know about things that have happened since our last key inspection and they have shown that they have managed issues well. They work well with us and have shown us that their service continues to provide good outcomes for the people who use it. What are we going to do as a result of this annual service review? Subject to any changes in registration and inspection practice following implementation of the Health and Social Care Act 2008, our current plan is to do a key inspection by 11th January 2011. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
