Little Trefewha is situated in the village of Praze-an-Beeble, close to the towns of Camborne and Helston. It is set in its own spacious, very well tended grounds and has reasonable parking space for staff and visitors. The home is part of a group of homes owned by a locally based company and provides residential care for up to twenty elderly people. The premises consists of a two storey detached building with a ground floor extension at the back. There are offices accommodated by the Company`s General Manager andAnnual Service Review 32009administration staff included. The upper floor is accessible by a stair lift. all the rooms are single. Five rooms have en suite facilities. The home provides ample shared space including a large lounge and a smaller `quiet` lounge. Meals are prepared in the kitchen on the ground floor and served in the dining room, or individual bedroom if preferred. Outside there is a patio and extensive lawns with seating and tables accessible to residents. Suitably experienced care staff provide personal care within a relaxed, friendly, welcoming atmosphere. Annual Service Review Page 3 of 8 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information we have received, or requested, since the last key inspection. This included the annual quality assurance assessment (AQAA) that was sent to us by this service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gives us some numerical information about the service. We looked at surveys returned to us by staff, healthcare professionals and people using the service. We have considered things that the manager has told us about what has happened within the service, these are called notifications and are a legal requirement. We have also looked at any other information from external organisations we may have received. The previous key inspection report was taken into consideration as well. What has this told us about the service? This service sent us their annual quality assurance assessment (AQAA) when we requested it. It was clear and concise. We looked at the information in the AQAA and our judgement is that the care home is still providing a good service and that they are continually making improvements to maintain this. The AQAA told us that this home gives good value for money, occupancy levels run at around 95 and people using the respite service return as they enjoy the time they spend there. The management are hoping to improve the residents guide in the coming year to include a section on The Company. Work is continually being done to improve the care plans to ensure a person centred approach and to keep the policies and procedures up to date. The AQAA told us that nutritional assessments are to be undertaken for all residents. Independence is promoted in the home and residents are encouraged to be involved in the running of the home via the regular residents meetings. A range of activities are on offer and entertainers visit monthly. It is hoped that more trips out will be arranged in future and that the extensive gardens around the home can be put to better use. The homes complaints policy and procedure is available for all to see and a copy is contained within the homes residents guide. A complete record of any complaints received is maintained along with the outcomes. Staff are aware of the safeguarding procedures to protect residents from abuse and have received appropriate training in this area. There have been no complaints or safeguarding issues in the past year. Little Trefewha offers residents a pleasant homely place to live in that is well maintained and safe. Re-decoration of the home is ongoing to maintain its light and airy feel. The home is kept clean and hygienic and staff receive regular training in respect of infection control. It is recognised that the access to the grounds could be improved for the residents and ramped access is planned for the inner courtyard. Annual Service Review Page 4 of 8 Over the past year new dining room chairs with arms, cushioned seats and backs have been provided and new chairs for the main lounge are planned. The re-decoration programme in the corridors and general areas has continued and the laundry facilities have been re-organised to make them easier to use and reduce the risk of cross infection. A new stair lift has been provided to give full stair assistance to the upper rooms and the car parking areas have been improved to allow a greater number of cars to park on the premises. Recruitment procedures are robust and prospective employees are fully vetted prior to employment. All staff have received an updated contract with a job description and staff handbook. Name badges for all staff have been introduced so that residents can identify staff members attending to them. The AQAA told us that 60 of care staff are qualified to at least NVQ level 2 in care and training for staff is promoted at this home. Over the past twelve months the management have provided Deprivation of Liberties, Protection of Vulnerable Adults, Mental Capacity Act, Parkinsons and Dementia training for the staff. The management have undertaken more indepth training on these subjects to ensure a higher level of understanding to support the staff. Further training plans include palliative care, nutritional assessment and assistance with feeding along with further Stoma Care training. The manager feels that they need to investigate alternative ways for staff to complete training including distance learning, internet and in house. The management run the home in the best interest of the residents. There is a clear organisational structure for this home and across the group of homes. Monthly managers meetings are held, with the managers from the other homes in the group, the director and the general manager. The general manager has a monthly meeting with the homes manager to discuss accounts and budgets etc. There are plans to link the home into the companys computer system with access to the internet. The manager has attended training as previously mentioned to ensure a higher level of awareness to support the staff. She has also attended advanced medication training, a swallowing awareness workshop and completed her dementia training. Monthly working budgets are now in place and the general manager has organised better purchasing for the home by using the strength of the Trecare group. Plans for the future include encouraging better attendance at the monthly staff meetings by trying different times, days etc. Also to ensure that staff supervisions are maintained and are more frequent. We received eight surveys from people using the service and they were positive about the home and the services provided. Four people told us the food is always to their liking and four said usually. Three people told us that there are always enough staff and that they listen and act on what the residents say, the other four said usually. When asked what the home does well the comments included, A very friendly home from home, Supplies my special needs as far as diet goes extremely well, Everything that a home should do, Excellent and cooperative staff, Manager is extremely helpful and competent, The home is very clean and there is a happy atmosphere and For me everything I request. Annual Service Review Page 5 of 8 When asked what the home could do better the comments included Nothing, Bit warmer in winter and Access for wheelchairs into the home a bit easier, from the conservatory into the front hallway. Other comments included Mums very happy there and the family are more than satisfied by the way that it is run, I am a comparative newcomer, I came to Cornwall to be near my daughter. I received such a warm and friendly welcome I felt immediately at home and still do. Everyone, staff and residents are happy - at least Ive heard no complaints, so things couldnt be better from the residents point of view. Very lucky to have a room at Little Trefewha only 2 miles from my family and I get visits from my family most days. We received five surveys from staff who work in the home and these were also positive. However only one person said there were always enough staff to meet the needs of people using the service, the others said usually. Two staff said the way that information is shared always works well the other three said usually. Three staff said they always have the knowledge and skills they need to do their job, the other two said usually. When asked what the home does well the comments included, Being a small home I think we tend to give a more friendly, homely environment than some bigger establishments, I think that general cleanliness and good housekeeping are above average, Provides a safe, homely atmosphere for the residents, Good communication between residents and staff, Ongoing training courses, 1:1 supervision with staff, Clear and precise care plans/medicines sheets, Delivering excellent quality of service to the residents, from meals to entertainment, A good friendly relaxed atmosphere and Relatives and friends are always encouraged to get involved. When asked what the home could do better comments included, Q4 asked if we are given training that gives us enough knowledge about certain aspects. I dont think we have enough training that covers medicines. I think we would benefit from knowing more about the medication we administer as well as safe handling and good practice, In an ideal world it would be nice if staff had more time to engage with the residents. When the home is full, staff can be stretched so socialising with the residents is difficult, Most things seem to boil down to money and budget restraints, for example when we are full we are really busy just doing basic care work and there just isnt enough time to sit and talk with our residents (we are rushing about too much). If there was more money we could do with an extra person who could be dedicated to doing this, In the afternoons there are just 2 of us plus 1 in the office to look after 20 residents plus 1 respite and There is plenty of outside entertainment, trips etc but Im not sure this is the same as having a familiar face fulfilling this role. Other comments included, I enjoy working at Little Trefewha and if I were elderly and in need of care I would be happy to be a resident, I feel our clients and their families are well served in general, For many years I had heard about the very good reputation of Little Trefewha where the care of the elderly residents was excellent and the staff turnover very low. I have to say that I have found this to be the case since starting work here in Oct 2009, The manager is committed to high standards of care, is very professional and fair with her staff and In my view this home is an inspiration to other homes. Annual Service Review Page 6 of 8 We received surveys from two healthcare professionals and one stated that the questions were not all relevant. The responses were variable one said always to nearly all questions the other usually. There was only one comment, The management and nursing staff always appear caring and conscientious. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do an Key inspection by 30 March 2011. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
