Margaret Allen House is a purpose built care home, comprising part of the original property and two newer extensions to the building. The home provides accommodation and personal care for up to 15 older people in single rooms. In October 2007 Devon Community Housing Society and Guinness Care and Support92009 amalgamated with Guinness Care and Support being registered as the providers of this service in December 2007. Margaret Allen House is situated within level walking distance of the shopping centre of the market town of Tiverton and close to the public park. There is a communal lounge with a conservatory attached. Meals are taken in a separate dining room or may be served in individual rooms. One staircase is equipped with a stair lift and a passenger lift is also provided for access to the first floor. A call bell system operates throughout the home. At the rear of the building there is a small raised garden and limited car parking area. The range of fees can be obtained directly from the home. Additional costs, not covered in the fees, include chiropody, hairdressing and personal items such as toiletries and newspapers. What the care home does well Margaret Allen House provides people with a clean safe and homely environment. The home has been extended and adapted to meet the needs of frail elderly. The registered providers have showed us that they wish to work with the Commission and have met all requirements in a timely fashion. People are cared for by a staff group who know and understand their individual needs. Individuals who live at the home said `Care could not be better.` `I can`t fault them, everything is very good, the care, the food, the cleanliness.` `I am very happy here.` People are treated with respect and dignity and their rights upheld. People we spoke to said that staff always knock on their door before entering and that they are supported in a kind and respectful way. The home ensures that there is a good variety and choice of meals, drinks and snacks. People can choose where they eat their meals and any special diets can be catered for. Everyone we spoke to was very complimentary about the food. The home has a clear complaints procedure and people feel confident to use this. We noted that the CQC contact details need to be changed to reflect our central postal address. What the care home could do better: Plans of care should include a mental capacity assessment. This will help staff to better understand what decisions people are able to make themselves and what they may need some support and guidance in. Risk assessments should be reviewed as part of the monthly review of the plan of care. This will ensure that risk assessments are kept up to date and take into consideration any changes to the individuals needs. Care Homes for Older People Page 3 of 9 What we found: This random inspection took place during a week day in May 2010. The main focus of this inspection was to check compliance of the requirements made during the previous key inspection and to review the quality outcomes for people living at the home. We spent time talking to the registered manager, two care staff and four individuals who live at the home. We saw some of the individual bedrooms and all of the communal areas. We looked at some of the key documents including pre admission assessments, plans of care, medication storage and recording of administering of medications. We also saw the staff recruitment file for the newest person employed. We checked how the home record any accident incidents and complaints and concerns. We checked how the home manages monies on behalf of individuals to ensure there is a clear audit trail. The home had been notified that a review of the home was due and had been asked to complete and return an AQAA (Annual Quality Assurance Assessment). This shows us how the home has managed the quality of the service provided over the previous year. It also confirms the dates of maintenance of equipment and what policies and procedures are in place. Information from this document was used to write this report. During the inspection 2 people were case tracked. This involves looking at peoples individual plans of care, and speaking with the person and staff who care for them. This enables the Commission to better understand the experience of everyone living at the home. As part of the inspection process we try ask as many people as possible for their opinion on how the home is run. We sent questionnaires out to people living at the home;to date two have been received. Their comments and views have been included in this report and helped us to make a judgment about the service provided. We found that the registered manager and provider had acted upon all requirements made during the previous inspection. The environment was now much safer. They had ensured that there is suitable hand washing equipment available to help prevent the risk of any cross infection. They have secured all large free standing furniture to the wall so that they no longer a risk of being a tip hazard. The one window in the staff room that had no window restrictor now is fitted with a restrictor and they are ensuring that all substances that contain chemicals are kept securely locked away. We also found that following requirements made during the previous inspection, the home have made their medication procedures and practices more robust. They ensure that all hand written entries are double signed. They also ensure that where a variable dose is prescribed, they make clear on their records what dose has been given and how the remaining medication has been disposed of. We saw that plans of care are now being shared with and agreed with the individuals they concern where possible. We saw that monthly reviews are taking place and these give good details of any changes in individuals personal, health or emotional well being. We did find that although risk assessments were in place for self medicating for example, these were not always up to date. We have asked the home to ensure that these are Care Homes for Older People Page 4 of 9 reviewed monthly as part of the care plan monthly review and update. We have also recommended that the home includes a mental capacity assessment as part of the plan of care for each individual. The staff we spoke to said that they had good opportunities for training. The registered manager showed us a list of training that has been planned for staff to attend over the next five months. We saw that for the most recent person employed the home has only confirmed them in post once all relevant checks and reference had been obtained. We saw that the home keep a record of any concerns or complaints with details of how these have been resolved. Individuals we spoke to said that they felt confident in making any concerns known and that issues would be dealt with. One person said Maggie is very good, she always makes time to listen. Both people who returned surveys said they knew how to make a complaint. Overall we found that the outcomes for people in the key areas we looked at were good. The home had responded and acted upon all requirements made from the previous inspection.
