Porthgwara is a detached well maintained care home set in landscaped grounds in the village of Coverack, near the town of Helston. The home boasts a wonderful panoramic view of the sea from the front of the property, which is appreciated by residents, staff and visitors to the home. Porthgwara provides nursing and personal care for up to thirty-two elderly people. The registration also allows for people with a physical disability. Accommodation is provided on four floors and consists of the original house with modern day extensions. The most recent extension provides six rooms with en-suite facilities. There are six double rooms most have single occupancy. There are spacious communal areas and adequate toilet and assisted bathing facilities. There are two shaft lifts to access the upper floors. The extensive gardens, lawn and very large patio are extremely well maintained. There are large plant tubs on the patio. Access for residents is restricted in certain parts of the grounds for safety reasons. A quiet area of the garden has been set-aside for visitors and staff to utilise. There is limited car parking space at the front of the home. Qualified nurses and care staff provide care in a relaxed friendly atmosphere. There is a flexible visiting policy and visitors are actively encouraged. Suitable activities take place each day organised by an activities co-ordinator and these are very much appreciated by the residents. What the care home does well The home has a suitable statement of purpose, service user`s guide and colour brochure that are issued to people using the service and are available in the entrance hall of the home. All prospective residents are assessed prior to admission to ensure that the home can meet their needs. From the assessment written care plans are compiled based on the activities of daily living and relevant risk assessments are undertaken. The care plans inform and direct staff on the care to be provided; they are reviewed each month with the resident and their representative as appropriate. Residents told us they are very well cared for at Porthgwara by staff that are kind and respectful. There is a suitable system for the storage, administration and disposal of medicines and relevant guidelines are available for the staff. The home provides appropriate activities and entertainment for people using the service and volunteers also visit and work with groups of residents There are several clubs that residents can attend and residents told us they have a say in what they do via their resident association meetings. People using the service live in a safe well-maintained home that is well decorated, clean, homely and comfortable with no unpleasant odours. The home is situated in a beautiful setting, which is appreciated by residents, staff and visitors. The grounds are lovely andkept very tidy, there is a large patio overlooking the sea a quiet area of the garden is set aside for staff or relatives use. Recruitment procedures are robust and the checks required by law are undertaken. There is an open style of management and staff, residents and visitors told us the home is very well run. What the care home could do better: Two signatures should be recorded for the administration of Temazepam tablets. Care Homes for Older People Page 3 of 10 What we found: A Regulatory Inspector visited Porthgwara Nursing Home on 06 May 2010 and spent six hours at the home. In accordance with the amendments to the Care Standards Act inspection programme this was a random inspection and an unannounced visit. The purpose of the inspection was to ensure that residents needs are properly met, in accordance with good care practices and the laws regulating care homes. The focus was on ensuring that residents placements result in good outcomes for them. On the day of inspection 30 people were living in the home and a daycare service was provided for 2 people. The inspector met with a number of residents, relatives, staff and the management team to gain their views on the services offered by the home. Records, policies and procedures were examined and the inspector toured the building. This report summarises the findings of this inspection. We received a completed Annual Quality Assurance Assessment (AQAA) from the registered manager, 9 surveys completed by people using the service and 8 from staff, prior to this inspection; the information has been used to inform this inspection. No requirements or recommendations have been notified following this inspection. Residents and relatives expressed extreme satisfaction with the care and services provided at the home. The home continues to provide an excellent service and a very high standard of care to the people placed there. We were given updated copies of the statement of purpose and service users guide that include all of the information required by legislation. We looked at the care files and medication records for the two most recent admissions to the home. Concise pre-admission assessment information was seen for both people that included information from the Department of Adult Social Care where appropriate. Both people had written care plans in place, based on the activities of daily living. These were signed as agreed by the resident or representative as applicable. The plans were informative and provided good guidance and direction for staff on the care and support required to meet their individual needs. Concise care plans listing personal care needs were kept in the residents rooms for care staff to refer to. Day and night records were seen, completed by the nurses, with separate records completed by the care staff. The registered manager said she hoped to have a combined system eventually. The records were informative and referred to the care plans, social and emotional information was included. Residents told us they were very well cared for in the home and the surveys we received from residents were very positive. Comments included Maintains a high standard of care for its residents, It looks after me in all respects, Caring for my individual needs, I am very well looked after with a stable group of carers who are always friendly and cheerful and Staff stay cheerful, respond to and respect residents. We spoke with three relatives who told us that the care at Porthgwara is exceptional and they were more than happy with the services provided for their family members living in the home. One relative said she could go on holiday and be assured that her mother was Care Homes for Older People Page 4 of 10 happy and in safe hands. We saw that a robust medicines system was in place that included the receipt, administration, disposal and secure storage arrangements. We examined the medicines records for the two most recent admissions and found them to be completed appropriately and up to date. We were shown the system and storage arrangements for controlled drugs and this again was appropriate. We discussed the records kept for the administration of Temazepam and were assured that two signatures would be recorded from now on. Surveys received from residents and staff, in February 2010, indicated that activities provided by the home could be improved. Comments included I think social activities for the men could be better, Have more entertainment for the people using the service and More personal one to one attention and interaction throughout the day. The manager told us that there have already been improvements in this area and there is now an activities team of three staff. Volunteers also come in to the home and work mainly with groups of residents. We talked with residents who told us they were satisfied with the activities in the home. One resident said that some people would like to go out into the community a bit more and that the management were aware and were trying to arrange this. Some residents told us they had had a cream tea at a local cafe and this was enjoyable. We saw plenty of good interaction between staff and residents during this inspection. We saw residents doing crossword puzzles, reading books, flower arranging and knitting. The knit wit club were busy knitting teddy bear puppets to be sent to Kenya. They were very enthusiastic, chatting together and enjoying themselves. A carer told us she had started a gardening club which was fairly new but residents were keen and some work had commenced. The activities co-ordinator was on duty and she was taking residents out to vote in the general election. A resident showed us the activities programme for the month displayed on a notice board along with photographs of residents involved in activities. We also saw some art work that had been done by residents. We saw the records maintained for residents, in respect of activities. The records were variable in content but generally included the amount of participation and what was gained from the activity, emotional experiences were also included. The Inspector toured the building with the registered manager who pointed out any improvements that had been made since the last inspection. The home was warm, clean, well maintained and there were no offensive odours. We were told that a re-decoration and refurbishment programme was in place for the coming year and that improvements over the past year had been mainly things that people may not be aware of as they could not be easily seen, for example the installation of thermostatic controls on hot water outlets, replacement of the emergency lighting systems, provision of external power points for charging residents powered scooters, a new computer network to the managers office and refurbishment of the treatment room. Other improvements included the installation of a Parker bath re-roofing the flat roof areas, the purchase of new dining chairs with arms and the purchase of new lounge chairs. We were told the laundry system was as before and that infection control training had been undertaken by staff. We were shown a letter from the Primary Care Trusts infection control team who were going to undertake an audit at the home. The grounds were seen to be tidy and very well maintained. A living willow arbour had been erected in the garden with a seating area for residents use. A carer told us she had built the arbour, with some assistance, and was very proud of the overall result. Care Homes for Older People Page 5 of 10 We examined the personnel files for the three newest employees; they all contained the documents required by legislation. We saw interview records and job descriptions included. We also saw certificates of training in the files and we were shown completed induction records. The manager told us that all new care staff undertake an induction programme in line with the Skills for Care standards and those who do not have a qualification are enrolled on an NVQ course. Staff told us during this inspection that they had a very good induction to the home and that training provision in general is very good. Staff, residents and relatives told us there are generally enough staff on duty and surveys told us there are usually enough staff to meet the needs of residents. Surveys from staff were positive about the home and the training provision, comments include Induction and support of new staff is good, There is a good deal of support and supervision, especially when you are new to the team, There is a good clinical base negating and reducing the need for clinic/hospital attendance, The home has a dedicated, hard working team in every department, Its a great place to work and the people are friendly and Having a nurse manager and a matron ensures a strong leadership for staff. The accident records were examined and showed minimal accidents occur in the home. The numbers of serious injuries and deaths at the home corresponded with the number reported to the Commission. The registered provider, registered manager and deputy manager were very helpful and open during this inspection and have obviously worked hard to maintain high standards in every aspect of this service.
