Ridgeway House is a purpose-built residential home for 42 older people, some of whom may have dementia. Built in the 1970`s, it is one of a number of homes provided by the Orders of St John Care Trust in Wiltshire and elsewhere. Accommodation is all in single rooms, located on two floors, with a passenger lift to the first floor. All bedrooms have wash hand basins, and one has en-suite facilities. Toilets and bathrooms are located conveniently. There are sitting rooms on each floor, whilst behind the home are secure gardens, including a landscaped patio area. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information we have received about the service, or asked for from it, since the previous annual service review. This included: The Annual Quality Assurance Assessment [AQAA] that the service sent to us. This is a self-assessment that focuses on how well outcomes are being met for people that use the service, and how the service intends to develop. It also contains some statistical information. Surveys returned to us by people who live and work at Ridgeway House. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection report [November 2007], and the previous annual service review [January 2009]. Inspections we have carried out at other services provided by The Orders of St John Care Trust. What has this told us about the service? The service sent us their annual quality assurance assessment [AQAA] when we asked for it. The AQAA was very detailed and gave all the required information. It showed the management maintained a focus on how the service is provided and how it may be further developed. They take account of feedback that they obtain directly from the people that receive a service. There is evidence they identify and work with peoples diverse needs. The provider maintains oversight of the service by routine audit and monitoring. The AQAA gave examples of how changes have been made within the home in response to gaining views and opinions from people who live there. After creative attention to improving peoples dining experience last year, there have been further developments in this important aspect of peoples every day lives. For example, the lunch service period has been extended and, at residents request, tea and coffee are served in the dining room after lunch has finished. A project to establish the meaning of personalisation, and how to incorporate it into how the home functions, has been shared between management, staff and people living in the home. The resultant report has been shared with us. Evidence has been provided of increased staffing levels in the home, especially at peak times. The activity co-ordinator hours have also been extended. The description of activities provision in the AQAA demonstrates an imaginative and individualised approach, based on person-centred principles. There is a strong emphasis on staff training, so that not only care staff but also housekeepers and the administrator have embraced National Vocational Qualifications. All staff undertake training in dementia care and there has also been provision of training in end of life care. The manager has Annual Service Review Page 4 of 7 sought to further her own development by embarking on a diploma in dementia care. Notifications of incidents and events have shown us that the home responds promptly to matters arising. Other professionals are consulted as appropriate. New risk assessments, and safeguarding referrals, are instituted when indicated. The Orders of St John Care Trust have introduced changes to their care planning systems and related documentation. This has resulted in a consistent approach to assessment and review, within and between homes in the Trust. Seven people who live at Ridgeway House [or their close relatives in some cases] responded to our postal survey. Four indicated that the staff were always available when needed, three said they usually were. All respondents said they received the care and support they needed. One person added that they would like to see staff having more time to spend one-to-one with residents. All respondents indicated that the home always arranged activities that they could take part in if they wished, and there were two specific comments of appreciation for the meals in the home. There were no negative comments, but a number of respondents considered the home was usually or sometimes fresh and clean, rather than always or usually. One person wrote additionally that the cleaning could be improved - they dont do behind the doors. Six members of staff responded to our survey. Of these, only one considered there were always enough staff to meet the individual needs of people living in the home. Two staff specifically recommended the home would benefit from a head of care role, as has been introduced successfully into several Orders of St John Care Trust homes. One person considered a senior role within the housekeeping team would serve to raise standards there. A number of respondents referred to the age of the building and the impact of small room sizes and lack of en suite facilities on residents, in particular those with more complex needs. Suggestions of how the service might be improved were counterbalanced by confidence expressed that the staff team and management worked together. Staff valued their training and qualifications, and considered residents benefit from a happy atmosphere and hard work on the part of all departments. A health professional who visits the home regularly returned a survey form. They wrote the staff seem friendly and helpful. The clients seem to live contented lives in this homes environment. The service has shown since the previous inspection that matters arising have been managed well. They share openly with us and have shown us their service continues to provide good outcomes for the people that use it. There has been clear commitment to address requirements and recommendations we made at the previous inspection, and to further improve peoples quality of life year on year. What are we going to do as a result of this annual service review? Subject to any changes in registration and inspection practice following implementation of the Health and Social Care Act 2008, our current plan is to do a key inspection by 26th November 2010. However, we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
