What the care home does well We saw that the manager makes sure that all altercations between service users are reported appropriately through local safeguarding procedures. She also maintains a monthly log of all incidents. The manager explained that whenever people who live at the home have been involved in an altercation, they are taken to either their own room or the sensory room so that staff can spend some time with them offering reassurance and support to make sure that the person has not been injured or upset by the incident. What the care home could do better: The manager told us that charts are completed to assess what may have led up to an incident, what happened and what happened afterwards. These are known as ABC charts. We did not see these charts as they are not kept within the care files. We looked at the care files of three of the people who have been involved in recent altercations. Two of the files were for people have a tendency to display aggressive behaviour toward others. We saw that the files contained a number of care plans but none had been developed relating specifically to their aggressive behaviour. The registered manager should make sure that ABC charts are completed and then used to form the basis of a care plan. This means that staff will know what situations may lead to the individual becoming aggressive and what actions they should take to avoid this. The care plan should also outline what staff should do to provide the person with the support they need following any incident. The registered manager agreed that the development of such care plans is needed and said that she would make sure that this is done. The ground floor provides accommodation for up to twenty-four people needing general nursing, and the first floor is for up to twenty-six people suffering from dementia. The home is situated behind the shops in Almondbury village. It is approximately two miles from Huddersfield but within a couple of minutes walk of all local amenities, including the bus stop. There is level access to the entrance of the home. In the home, access to the first floor is gained either by stairs or a passenger lift. There is a garden including a secure area where service users may sit. There is ample parking at the front of the home. The fees range from £398 to £630 per week (July 2009). We did this random inspection to look at what systems the home has in place to try to reduce the number of incidents and how they support people when the incidents have occurred.
