12032009Stratford Court was originally purpose built by the local authority as a care home for up to 48 older people. The home is also registered for up to 14 older people with dementia and up to 6 older people with a mental disorder. One of the beds is offered to older people in the community for periods of respite care. The registered providers are The Orders of St John Care Trust. Mrs Teresa Baldwin was registered as manager in December 2009. The accommodation is arranged over two floors, with additional day care facilities located on the lower floor. The home is opposite Victoria Park in Salisbury, close to the city centre. The staffing rota provides a minimum of 5 care staff and a care leader during the morning, 4 care staff and a care leader during the afternoons and evenings and three waking night staff. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received and asked for, since the last key inspection of 12th March 2009. We looked at their annual quality assurance assessment (AQAA); this is their own assessment of how they are doing, that focuses on how well outcomes are being met for people using the service. We looked at surveys returned to us by people using the service and from other people with an interest in the service. We looked at how the service has managed any complaints. We looked at what the service has told us about things that have happened in the service; notifications that are a legal requirement. We also looked at our last report on the service, relevant information from other organisations and what other people have told us about the service. What has this told us about the service? The home sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and gave us all the information we asked for. Mrs Baldwin told us that as a result of listening to people the following changes had been made: mealtimes have been brought forward by fifteen minutes, activities are planned by asking people what they want to do, some sinks in bedrooms have been adjusted so they are more accessible, the laundry system has been changed, the chef has discussed the meals with people and a male chiropodist now visits the home. Mrs Baldwin told us she plans to monitor the laundry service and the meals. She also told us she planned to provide an area where families and visitors can make hot drinks. We looked at the information in the AQAA and our judgement is that the home is still providing a good service and that they know what further improvements they need to make. People living in the home always tell us that they are happy there. One comment was Clean and fresh. Pleasant people. Comfortable and settled. Well looked after. Another person told us Caring. Everything is beautiful, nothing to be made better. A further person told us Very friendly, helpful. Always there when I need help. I would like all my washing back from laundry. Another comment was the company is good. Staff are friendly. We received comments from relatives of people living in the home. All continue to be very satisfied with the quality of the care. One comment was The staff generally appear pleasant and kind with the residents and there is a nice atmosphere in the home. My mother is obviously well cared for and looks healthy and content. They have handled the occasional behaviour of my mother with sensitivity and understanding. They are also very kind to my father when he visits her and he is much happier and reassured that she is in good hands. Another relative told us I have been very impressed as to how she has been cared for both physically and mentally. I especially commend the activities and entertainment that are provided. I have also found all the staff I have dealt with caring and pleasant to deal with. [Do better] Do something about the smell. Annual Service Review Page 4 of 6 A further relative told us They have got to know my mother well and have an understanding of her everyday needs. On a few occasions though when my mother has said she is feeling OK they have just accepted this even though she may have looked or been feeling under the weather. I feel some things usually small complaints could be nipped in the bud earlier. My mother is generally very happy at Stratford Court and I feel happy she is in safe hands. Love the activities they have and I know she enjoys the food. Another relative told us My mother loves joining in all the activities. She loved the holiday she had this year, the first. Loves going out and I think the service does well. I have always found the staff very cheerful, helpful and caring. My mother has settled in and feels happy and secure. [Do better] I know my mother would like more baths if possible. Sometimes the laundry is a little slow. Two relatives said that visitors should be encouraged to use hand gel to stop cross infection. One relative told us they felt their mother should have more support with personal care and have more baths. One staff thought there should be more trips out. Another staff member told us Be able to spend time with all the residents, not just the ones that need the attention. In the AQAA Mrs Baldwin told us We have a suggestion box which allows residents, staff, families or any other visitor to make suggestions on someones behalf if they are unable to make their own voice heard. One staff member told us I have recently been encouraged by the fact of the new manager and the way the trust thinks with involving the residents by doing life history and ongoing activities. To see residents enjoy themselves and the atmosphere has changed to a home not a residential dwelling. Two staff told us the training provided was good. One comment from a healthcare professional was I have confidence in Teresa Baldwin, manager but not all staff. Teresa always responds quickly to crisis and is supportive to our client group. [Do better] Improve training of staff in meeting the needs of clients with dementia and understand the differing manifestations of the illness. The home continues to let us know about things that have happened since our last key inspection and they have shown that they have managed issues well. They work well with us and have shown us that their service continues to provide good outcomes for the people who use it. What are we going to do as a result of this annual service review? Subject to any changes in registration and inspection practice following implementation of the Health and Social Care Act 2008, our current plan is to do a key inspection by 12th March 2012. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
