The Conifers provides personal care and accommodation for a maximum of twelve older people, who may also have a physical disability or a diagnosis of dementia. The Conifers can also provide additional day care for up to three people.22009 The home is owned and managed by Dr & Mrs Trehan and is located in an established residential area of Old Trafford close to the village of Chorlton, shops, bus and train routes and other amenities. The home is a two storey Victorian property set in its own grounds and consists of 10 bedrooms (eight single and two double), one dining room and two lounges. Stair lifts are installed on both sets of stairs. The home has gardens to front and rear of the property, which are well maintained. Parking is available to the rear of the premises.

What the care home does well We had conversations with three people using this service. They told us that their care was good and that they got on well with staff. People sitting in the lounge appeared to be relaxed and were enjoying a mid-morning cup of tea. We observed staff interacting well with people living in the home. People were addressed in a respectful and dignified manner. We observed a member of staff taking care to explain to a person with a visual impairment, exactly where they had positioned a cup of tea. This was evidence of good practice in minimising the risk of accidental scalding to the person concerned. Staff had received training in recognising abuse and the action they must take if they suspected a person using the service was at risk of harm. What the care home could do better: People using this service consistently tell us that the care and support they receive is good and staff appear to know what they must do to keep people safe from harm. However, written information in risk assessments, reviews and care plans must accurately reflect the action that staff take to minimise the risk of accidental injury to the people accommodated. Monthly reviews must detail significant events and use the information to update risk assessments and care plans. Clear written guidance must be provided to ensure that staff are aware of the action they must take to manage the risk of accidental harm to people living in the home. We recommend that staff receive training and competency assessments in undertaking the assessment and management of risk. Regulation 37 notifications help us to monitor the conduct of the home and outcomes for people using the service. The registered manager must consistently notify us about significant incidents occurring in the home. This includes accidental injuries that require treatment from a medical practitioner. Care Homes for Older People Page 3 of 9 What we found: On 5th February 2010 we were notified by Trafford Social Services that a person accommodated in The Conifers had sustained a serious injury during a fall on 16th January. The incident had been referred to Trafford for investigation under their Safeguarding Adults Procedures. The investigation raised concerns relating to the homes failure to assess this persons needs prior to admission, to implement a care plan and to undertake a risk assessment based on a history of falls. Care homes are required to let us know, without delay, about serious injuries occurring to the people accommodated. These are called Regulation 37 notifications. The home did not send us notification about this incident until 24th February. Our records showed that no other Regulation 37 notifications had been received from The Conifers since our last visit. We visited the home on 6th May 2010 to look at these issues. Our visit was unannounced, which means that the manager was not informed that we were coming to visit. The registered manager was on holiday on the day of our visit, but we were joined by a member of senior management from a nearby nursing home owned by the provider. We examined the Conifers accident records and found that five incidents had been recorded between October 2009 and March 2010 for injuries sustained two people living in the home. Accidental injuries should trigger a review of the persons risk assessment and care plan. This had not been undertaken appropriately for the two people concerned. For example, one person sustained three cuts to her hand on 25th February, from contact with an item of furniture in her room. Her review dated 14th March made no mention of this incident and no risk assessment had been undertaken to minimise the risk of this happening again. The second person had attended hospital in January for treatment to a cut on her back. A review conducted two weeks after the incident failed to highlight the risk. Additionally, no specific risk assessment had been undertaken and there was no clear guidance in place to tell staff what they must do to keep this person safe from further accidental harm. We looked at the initial assessments undertaken for both these people. It was evident that suitable in-house assessments of need had been undertaken prior to their admission.
