The Dene Lodge is a care home proving care and support for up to 33 people over the age of 65 years, with a wide range of support needs. It is close to local shops and services and public transport links. Accommodation is arranged on three floors with the communal areas located on the ground floor. What the care home does well This inspection was carried out to look at progress being made to meet the requirements set at the last inspection. What the care home could do better: No new requirements have been made as a result of this inspection. 4 requirements have been met, two have been partially met and 1 has not been met. The three unmet requirements will remain in place and will be looked at again at the next inspection. The home has a specialist unit which cares for people who have a dementia. Some people living within this unit may lack capacity to make some decisions for themselves. It is therefore strongly recommended that all staff receive comprehensive training on the Mental Capacity Act 2005 and its implications for practice. Care Homes for Older People Page 3 of 10 What we found: This random inspection was carried out by one inspector over a period of 2.5 hours. The purpose of the inspection was to follow up on progress being made to meet the requirements set at the last key inspection on the 6th October 2009. During the inspection we, The Commission, discussed issues with the homes management, met with people living and working at the home, looked at care plans, staff training records and the complaints log. 5 care plans were looked at; 3 for long term residents, 1 for a person who had recently moved to the home and another for a person who was due to stay at the home for a period of respite. Requirement 1 Standard 9 Regulation 13(2) The home must ensure that people receive their medicine as prescribed to protect their wellbeing. There was no evidence to suggest that the people living at the home were not receiving medication as prescribed. The new head of care is now auditing Medication Administration Records on a monthly basis to monitor practice and identify any errors. THIS REQUIREMENT HAS BEEN MET. Requirement 2 Standard 16 Regulation 22 The registered person must ensure that all complaints are dealt with using the homes complaints policy. We saw evidence that one complaint had been received since the key inspection. This had been responded to by the homes general manager within the timescales set out in the homes complaints policy. THIS REQUIREMENT HAS BEEN MET. Requirement 3 Standard 31 Regulation 12. The registered person is required to ensure that care provided at the home is monitored and audited to ensure good practice is maintained. A new head of care has been appointed to the home and has been in post for 4 weeks. They have begun to review care plans with the person concerned and/or their representative. They are planning to carry out regular care plan audits. Staff have begun receiving formal supervision to identify strengths and weaknesses and to ensure that all staff are working in a consistent manner. The management of the home are planning to put in place formal care audits to ensure that any shortfalls in the service are identified and addressed without delay. Care Homes for Older People Page 4 of 10 THIS REQUIREMENT HAS BEEN PARTIALLY MET. It will be looked at again at the next inspection. Requirement 4 Standard 3 Regulation 14 (1) The registered person must ensure that anyone wishing to move to the home has their needs assessed by a suitably qualified person. Since the last inspection one new person has moved to the home. Before moving in the person was living a distance away from the home so it was impractical to carry out a pre admission assessment. They did however acquire a copy of a full needs assessment and speak with the persons family representative. Another person is due to have a respite stay at the home and a full pre admission assessment had been carried out. The home has also received a copy of a full assessment carried out by professionals outside the home. From these documents an initial care plan and risk assessments had been put in place in preparation for their stay. THIS REQUIREMENT HAS BEEN MET. Requirement 5 Standard 7 Regulation 15(1) The registered person must ensure that care plans clearly set out the needs of people and the action to be taken by staff to address needs. Care plans must be measurable to ensure that their effectiveness can be monitored. The plans must also record the wishes and preferred routines of people living at the home. 5 Care plans were looked at during this inspection. The 3 for long term residents had all been reviewed but did not give clear guidance in respect of all needs. For one person with diabetes there was no care plan in respect of this. Although the home were providing a specialist diet and monitoring blood sugar levels there was no care plan to say what the specialist diet was. There was no guidance in place to say how often or when blood sugar monitoring should occur, no information about what the persons normal level was or what to do if the level was outside the normal range. Another care plan was for someone who had been loosing weight. There was evidence that the person had been seen by their doctor but again no care plan. Staff were recording food and fluid taken but the recordings were poor. There was no care plan in place to say what the person was expected to eat and drink in a day, the records written did not give clear amounts and there were no totals in place for each day. A third care plan was for someone who had a dementia and lacked capacity to make some decisions. There was no care plan to show how the person would be assisted to make choices and no evidence that the home were working in line with the Mental Capacity Act. Decisions were being made by family members and these were being recorded. There was no evidence that the home were following the Best Interests guidance set out in the Mental Capacity Act and staff had a poor knowledge of the Act and its implications. THIS REQUIREMENT HAS NOT BEEN MET. Care Homes for Older People Page 5 of 10 Requirement 6 Standard 30 Regulation 18 (1) [a] The registered person must ensure that there is an ongoing training programme for all staff. Each person working at the home now has a personal development programme with targets for when each training module should be completed by. A training matrix has been put in place to show that all staff have completed statutory training. One new member of staff is working through an induction programme. The head of care said that they are currently looking for additional training for staff in dementia care. 7 of the 15 care staff (46 ) have completed a National Vocational Qualification in care at level 2 or above. A further two people have registered for the award. THIS REQUIREMENT HAS BEEN MET. Requirement 7 Standard 33 Regulation 24 (1) The quality assurance systems must include the formal auditing of care practices and documentation. Some auditing has been put in place and this is in the process of being more formally organised and recorded. THIS REQUIREMENT HAS BEEN PARTIALLY MET. It will be looked at again at the next inspection.
