If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service The Malthouse is a 17th Century building with attractive gardens situated on the outskirts of Gillingham, which has been considerably extended to provide residential care for 33 older people. The home is located approximately half a mile from Gillingham town centre in a quiet lane. The home has recently come under new ownership and the Registered Providers are Malthouse Healthcare Ltd. The accommodation is arranged over three floors with a passenger lift serving the first Annual Service Review Page 2 of 6 floor and a stair lift providing access to the three second floor rooms. There are 31 bedrooms, 4 of which are suitable for a couple, friends or relatives wishing to share. Bedrooms are of good size and all but one has en-suite facilities. In addition to personal care and support, the service provided includes all meals, laundering of clothes etc., and all housekeeping; a range of social and leisure activities are also provided, and the manager and staff provide help to make and attend healthcare appointments, where this is necessary. In the grounds of the home five lodges have been built offering an independent lifestyle. On the ground floor there are three apartments also offering an independent lifestyle. The lodges and apartments do not form part of the registered premises. The home has achieved the Investors in People Award. At the time of the last Key inspection the weekly fees ranged from £625 to £715. Additional charges are made for hairdressing, chiropody, newspapers and personal shopping. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received and requested since the last key inspection which took place on This included: The annual quality assurance assessment (AQAA) sent out by us, was completed and returned to CQC in a timely manner. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys completed by people living at the home and other interested parties. Regulation 37 notifications informing us of significant events in the home have been received throughout the year. We reviewed the previous key inspection report. We took account of what other people have told us about the service and any relevant information received from other organisations was considered. What has this told us about the service? The home sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and very detailed and gave us all the information we asked for. It is clear that since the home changed ownership last year that several changes have been implemented and some are ongoing. Some examples of developments within the home are: new social activities organiser, introduction of a key worker system, residents surveys, regular reviews with residents and their relatives and full time maintenance person. A comments and suggestion box has been installed and there are plans to improve the homes brochure, pre-admission forms and the website. There are also plans to refurbish parts of the home and grounds. The Acting Manager says in the AQAA that continuity of long-term objectives has been hindered by managerial changes and there has been inconsistency of implementation, however staff have a positive attitude and work well as a team. The AQAA also tells us that each person now has their own key worker who helps to ensure that each person has their needs met in a way that suits them to ensure that peoples care continues to be planned in an individual and coordinated way. A care plan co-ordinator has been appointed and care plans are reviewed and updated monthly and as necessary. The AQAA tells us the home continues to provide regular activities for residents including a mobile library and gentle exercises. Annual Service Review Page 4 of 6 The AQAA tells us that 16 of the 34 care staff have achieved a National Vocational Qualification (NVQ) in care at Level 2 or above. There are plans to increase the level of staff training over the coming year. There has been a high turnover of staff since the last inspection, 21 staff have left, and this is of concern. The Commission has not received any other information from other sources about this service. We have not made any visits to the home since the last key inspection. The home continues to provide a high standard of care and however there is evidence to indicate that outcomes for people who use the service may have changed since the last inspection of the service, because of the change in management and the high turnover of staff. What are we going to do as a result of this annual service review? As there is no registered manager in post, and because of organisational changes within the home and the high turnover of staff the next inspection will be brought forward.
