If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service The Paddocks was purpose-built in the 1970s. All bedrooms and communal areas are on the ground floor, centred on a dining room and adjoining lounge area. There are two other sitting areas, close to bedrooms, plus an activities room. In addition there is Annual Service Review Page 2 of 6 0 8 0 1 2 0 0 9 an attractive enclosed garden with summerhouse. Bedrooms have vanity units but toilets and bathrooms are shared. One room is reserved for short stays. The home is one of a large number of homes provided by the Orders of St John Care Trust in Wiltshire and in three other counties. It is situated in its own grounds, off one of the main roads into Trowbridge. There are good bus and rail links to neighbouring towns, and the home has its own car park. The registered manager, Anna Lovesey, is supported by a team of care leaders and an administrator. Care workers are provided throughout the day and night. There are housekeeping and kitchen staff, and the home has its own handyperson. Weekly fee levels range between £430 and £585 [as at January 2009], according to assessed dependency. Where people are placed and part-funded by the local authority, the contracted fees are slightly less. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information we have received about the service, or asked for from it, since the previous annual service review. This included: The Annual Quality Assurance Assessment [AQAA] that the service sent to us. This is a self-assessment that focuses on how well outcomes are being met for people that use the service, and how the service intends to develop. It also contains some statistical information. Surveys returned to us by four people who live at The Paddocks, and from a community nurse and GP. Notifications the service has sent to us about significant events affecting people living in the home. The report of our previous key inspection carried out on 8th January 2009. Our inspection activity at other services provided by The Orders of St John Care Trust in Wiltshire. What has this told us about the service? The service sent us their annual quality assurance assessment [AQAA] when we asked for it. The AQAA was very detailed and gave all the required information. It showed that the management maintained a focus on how the service is provided and how it may be further developed. They take account of feedback that they obtain directly from the people that live in the home. There is evidence they identify and work with peoples diverse needs. The provider maintains oversight of the service by routine audit and monitoring. The AQAA gave examples of how changes have been made within the home in response to gaining views and opinions from people who live there. For example, as a result of the homes annual quality survey, there have been changes made to the variety and timing of meals, and further attention to ways of providing a good dining experience. Their survey also showed some dissatisfaction with aspects of the laundry arrangements in the home, which again were receiving attention. The AQAA records environmental enhancements that have been made to maximise peoples options for use of shared space. For example, small sitting areas have been developed, so people can find more privacy outside their own room if they wish, or more choice of areas for reception of visitors. Residents have been directly involved in choosing colour schemes and soft furnishings. Evidence has been provided of increased staffing levels in the home, partly by increasing care support so that care staff can work more closely with the activities coordinator. The description of activities provision in the AQAA demonstrates an imaginative and individualised approach, based on person-centred principles and using Annual Service Review Page 4 of 6 life history information and inputs from peoples relatives and friends. All staff undertake training in dementia care and in equality and diversity awareness. There has also been a programme of work and training with a local hospice to enhance the provision of end of life care. A community nurse responding to our survey indicated that the home always seeks and acts on advice to meet peoples health care needs and improve wellbeing. A GP responded that they usually do. The GP expressed an observation that the home faced increasing dependency levels, particularly in regard to dementia, which increased pressures on staff to care for people who are close to needing nursing home placements. Both the GP and community nurse saw the home as respectful of peoples privacy and dignity, and supporting them to live the life they choose. Notifications of incidents and events have shown us that the home responds promptly to matters arising. Other professionals are consulted as appropriate. New risk assessments, and safeguarding referrals, are put in place when indicated. The Orders of St John Care Trust have introduced changes to their care planning systems and related documentation. This has resulted in a consistent approach to assessment and review, within and between homes in the Trust, with a noticeable emphasis on personcentred care. Four people who live at The Paddocks responded to our postal survey, with assistance from close relatives in some cases. Two indicated that the staff were always available when needed, two said they usually were. All respondents said they received the care and support they needed and all said the home was always fresh and clean. One person wrote additionally about excellent staff, and another wrote the manager and staff are very able and very willing to help me. A third persons comment was excellent staff, all very kind and caring. One person living in the home wrote, I think its great, I can do as much for myself as I want, Im very happy and wouldnt change it at all. A relative added that the home was a very welcoming place to visit. The community nurse observed that the home stimulates people and involves them in regular activities. The service has shown since the previous inspection that matters arising have been managed well. They share openly with us and have shown us the home continues to provide good outcomes for the people that live there. There has been clear commitment to further improve peoples quality of life year on year. What are we going to do as a result of this annual service review? Subject to any changes in registration and inspection practice following implementation of the Health and Social Care Act 2008, our current plan is to do a key inspection by 7th January 2012. However, we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
