What the care home does well The service had acted positively on the previous requirements relating to care plans, medication, training and supervision. What the care home could do better: The manager needs to be registered. The controlled medication cupboard needs to be fixed in such a way as to meet the specification of medication storage. Care Homes for Older People Page 3 of 8 What we found: Following examination of the information available to us, we undertook a risk assessment. This assessment indicated that a Random Inspection could be undertaken rather than a Key inspection. It was recognised that the rating does not change following a Random inspection. On examination of 3 care plans we found evidence that the care plans had been reviewed, and the information within the risk assessments had been increased. The care plans had been audited in February 2010 and the shortfalls highlighted and recorded. These remained outstanding, as we found similar issues within the care plans. The manager agreed to raise this with the staff and the person who undertook the audit. Also to ensure in future that the issues raised will be acted upon and signed off. On examining the medication we found that the record keeping had improved and the quantity was now recorded. All medication which had been administred had been signed for on the medication administration record. On examining the storage of the medication, within the treatment room on the ground floor, we asked to see the controlled drug records and storage. The drugs were located in a cupboard, but not the controlled drug cupboard. On questioning why the drugs were not in the controlled drugs cupboard, we were informed that they could not locate the key. Several keys were tried without any success. The manager advised that she had ordered a new controlled drug cupboard. On examination of the medication in the treatment room on the first floor, we found that the controlled drug cupboard had not been fitted as detailed in the manufactures specification. It had not been dead bolted but loosely screwed into the wall. The fitting instuctions were located on top of the cupboard. These were given to the manager. It was discussed that the chains on medication trolleys were always difficult to attached. It was identified that the security of the treatment rooms consisted on a 3 lever lock on one door and a Yale type lock on the other. Should the service increase the security on the doors to a 5 lever anti saw locks, then the chains on the trolleys could be removed. The service now receives confirmation that all checks have been undertaken on the staff, which the agency provide. The manager was not registered with the Commission.She advised that she was to apply for registration. We were shown evidence that staff training had occurred and a training matrix was now in operation. Supervision had commenced and future dates were planned. However the information contained within the records was minimal. It was discussed with the manager that she should consider all aspects of practice, as described in Standard 36. Ways of monitoring staff practice was also discussed.
