What the care home does well Staff are provided with good opportunities to attend training relevant to their roles. They have excellent levels of NVQ level 2 qualified staff and can access e-learning courses to develop new skills and competencies. People told us that they had enjoyed their lunch, and they said food was always very nice. What the care home could do better: The manager of the home has worked at the home since August 2008. She has not applied to be the registered manager. This was discussed the registered provider and the manager. Staffing levels at the home could be better as two staff was insufficient to meet the social, emotional, and personal needs of people who use the service. Our judgement is based on the layout of the building and the dependency levels of the people living at the home. Risk assessments with regard to the use of bed rails must be agreed and signed by the individual or their representative. The activity coordinator should not be used to supplement care duties such as covering for sickness and escorting people to hospital. As this means she is not able to provide social and emotional support to people who use the service. The gardens around the home were untidy and people would not be able to access them easily. The home is registered to provide accommodation for 32 older people, some with nursing needs. The home was once the old vicarage and retains many of its original features including narrow corridors and some steps along the corridors. Chair lifts are provided at these points. Accommodation is on two floors and a passenger lift is provided. There are several lounges and dining areas throughout the home. Bedrooms vary in size and some have en-suite lavatories. The home has landscaped gardens and there is a car park to the front of the property. The weekly fees were from £366 to £382. Hairdressing, manicures and newspapers were not included in the weekly fee and were charged extra. The acting manager supplied this information during the site visit on 5th May 2010. The last inspection report dated 23rd June 2009 and further information about the service can be obtained from the home. People are given full information about the home before they make a commitment to stay. The most recent inspection report, a copy of the service user guide and other information about the home was available for perusal in the main entrance. Care Homes for Older People Page 3 of 10 What we found: The adequate star rating for this service was made at the previous inspection visit on 23rd June 2009. The rating cannot be changed during a random inspection visit. However, we can still inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service. Before we carried out this inspection of the service we looked at the Annual Quality Assurance Assessment (AQAA) that was sent to us by the service. The AQAA is a selfassessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. We also looked at what the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. We contacted Rotherham Metropolitan Borough Councils contracting department, they told us that they had no major concerns about the home. They did however comment about the low occupancy levels which was a concerns to the providers. We sent 20 surveys (10 to people who use the service 5 to staff and 5 to health and social care professionals). We did not receive any back. We spoke to people who use the service and a visitor. Their views are included in this report. The visit lasted for 4.5 hours starting at 09:45 and finishing at 14:15. The manager assisted with the inspection process. We looked at procedures relating to the safety and protection of people who use the service. This included discussion about arrangements for complaints and safeguarding procedures. We looked at three staff recruitment files to assess if the right staff were employed to keep people safe, and we interviewed members of staff. We audited medication to assess if people received their medication as prescribed. We also looked at the requirements we made at our last inspection, to assess if the service had made improvements to care planning, activities and the environment. The outcome is as follows: We looked at two care plans and found they had information to enable staff to deliver the care people need. However risk assessments and agreements in relation to the use of bed rails had not been fully completed. The manager told us that they were looking to develop a new format for care plans. Staff were receiving training to enable them to develop the new care plans for people who use the service. Since the previous inspection they have employed an activity coordinator, however the staff member is often deployed to escort people to hospital, which distracts from providing activities to people using the service. A visitor told us that there appears to be very little stimulation for people living at the home. They told us that people were not involved in outings and there was no programme of entertainment. We looked around the building and found that there had been some refurbishment of the building, including decoration and replacement of some carpets. The manager told us that Care Homes for Older People Page 4 of 10 they were continuing with a rolling programme of refurbishment. The staff recruitment files were organised and easily audited. They included the required documents which confirmed staff were correctly employed. Members of staff told us that they had undertaken training which included moving and handling, and safeguarding adults training. The AQAA told us they have excellent levels of NVQ level 2 qualified staff. We looked at the medication administration (MAR) records and they were accurately completed, which confirms people receive their medication as prescribed. They had robust procedures, which ensured staff were given support and training. Their competencies were regularly checked and the local pharmacist also undertakes audits. This ensures good practice is followed regarding management of medicines. We looked at the complaints records and discussed the procedure with the manager. She was able to confirm that timescales for responding to complaints were followed. They have procedures to ensure people were protected from abuse and the manager was able to demonstrate a good understanding of the procedures. The AQAA told us that there had been two referrals made to safeguarding adults. Staff regularly received training to ensure they knew what action they needed to take to keep people safe from harm, which included training in Safeguarding of Vulnerable Adults (SOVA), Mental Capacity Act (MCA) and the Deprivation of Liberty (DOL) procedures.
