What the care home does well

Worsley Road provides a very supportive family home for the one person who is considered very much a part of the family. The home is maintained to an exceptionally high standard and the person who lives at the home is very happy, enjoying an active lifestyle appropriate for his age.

What has improved since the last inspection?

There were no requirements or recommendations made following the previous inspection in January 2006. Since then there has been a new condensing boiler and the residents bedroom has been redecorated and new furniture provided.

What the care home could do better:

The service provided is individual and exceptional. There are no areas that the inspector feels could or should be improved.

There is no expectation that the home would remain registered if this person moved to alternative accommodation and the home does not intend to admit new people. The home is a pleasant, well-maintained, domestic family house situated on the outskirts of Newport and close to all amenities. The person who lives at the home has his own bedroom and free use of all the space in the home and garden. This inspection was the first ‘Key Inspection’ for 15a Worsley Road a ‘Key Inspection’ being part of the new inspection programme, which measures the service against the key National Minimum Standards. The service was telephoned prior to the inspectors visit to arrange a suitable time for the inspector to visit when the person who lives at the home and the manager would be available. The visit to the service, was conducted by one inspector over one day lasting a total of two hours, where in addition to any paperwork that required reviewing the inspector met with the manager, her husband and the person who lives at the home. Communal areas of the home were viewed. Due to the size and nature of the home a pre-inspection questionnaire was not sent to the home. The inspector telephoned the care manager of the person who lives at the home prior to the visit. What the service does well: What has improved since the last inspection? JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area has not been rated. This judgement has been made using available evidence including a visit to this service. No standards were assessed as the service does not intend to admit any new people and the one person living at the home has done so for many years. EVIDENCE: The manager confirmed that the home does not intend to admit any new people. The service exists solely to continue to provide a home for one person who has lived there since being a child and would cease to exist as a registered home should he move to alternative accommodation. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 8 and 9. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The person who lives at the home is supported to make appropriate decisions, take reasonable risks and lead an active lifestyle appropriate for his age and interests. EVIDENCE: The person who lives at the home has a social services care manager who confirmed that she has recently conducted a review of the placement and that there were no areas of concern. The person who lives at the home is aware that records have to be maintained and reviews undertaken however he dislikes these and will avoid involvement wherever possible. The manager encourages choices and responsibility for decisions. The manager spoke knowledgeably of the ways this can be done and the progress that has been made in this area. Discussions with the person who lives at the home confirmed that he is able to make decisions for himself and is aware of the Worsley Road (15a) DS0000012594.V334900.R01.S.doc Version 5.2 Page 10 consequences of these decisions. The home does not restrict his rights and the manager confirmed she would discuss any concerns with the care manager. The person who lives at the home does not have a benefits appointee however the manager does provide some advice to support him to manage his personal finances. The manager maintains appropriate records in respect of finances however the person who lives at the home dislikes having to sign for his weekly spending money as he is aware that other people do not have to do this. It is suggested that the manager continue to maintain the records and inform the care manager that these will no longer be signed by the person who lives at the home as he is unhappy doing this. The person who lives at the home confirmed that he is able to make his preferences known and that he is involved in decisions about the home. During the inspection it was evident that he felt comfortable with the manager and her husband and is able to make any suggestions or requests as they occur. The policies and procedures within the home are specific to the needs of the service. The manager clearly understands the needs and capabilities of the person who lives at the home. He is encouraged to consider the implications of his actions and to understand any risks involved. He works daily and is able to use public transport to get around the Island. The home has an appropriate procedure in place should there be any unexplained absences or he does not return home at the time expected. The manager has undertaken risk assessment training. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The person who lives at the home has chosen an active lifestyle appropriate for his age. EVIDENCE: The manager is particularly good at encouraging social skills. The manager and her family include the person who lives at the home in all their activities but also appreciate the need for him to have his own interests. The person who lives at the home discussed his work and leisure activities with the inspector and is supported when necessary by the manager, her husband or other family members. The person who lives at the home has friends his own age with which he works and spends leisure time. On the day of the inspectors visit he was going to the cinema with his friends after work to see a film they had Worsley Road (15a) DS0000012594.V334900.R01.S.doc Version 5.2 Page 12 chosen. He also enjoys a number of sporting activities. It was clear that the service user has control over what he does and how he spends his time. The person who lives at the home is capable of caring for himself in many ways but needs reminders to undertake domestic and self care activities. The manager continues to encourage him to develop independence skills although he is reluctant in some areas. The person who lives at the home was very complimentary about the food provided at the home and stated that he is involved in meal choices. The manager and her husband have food hygiene certificates. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 and 20. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Health and personal care needs are appropriately met. EVIDENCE: The person who lives at the home is independent in all areas of personal care requiring only reminders and encouragement. He has a daily routine for getting up, baths, mealtimes and bedtimes. Verbal support is provided tactfully by the manager and she reported that suggestions are appropriately accepted. The person who lives at the home is registered with a local GP in Newport who he sees yearly to renew relevant benefits certificates. He is happy for one of the proprietors to accompany him for medical appointments and will tell them if he has any health concerns. He is generally fit and healthy with no regular medication prescribed. Appropriate records are kept when short-term courses of medication are prescribed. The manager maintains a record of all visits to health professionals and advice/action taken. The manager has undertaken medication training. This judgement has been made using available evidence including a visit to this service. The person who lives at the home is able to complain and is protected from abuse. EVIDENCE: There have been no complaints about this service. The person who lives at the home was clear that he had no complaints and confirmed that he was aware of how to make one. He is also aware of his care managers phone number and could also make a complaint via his care manager or his work placement who would ensure that appropriate action was taken to resolve any complaints. The manager was aware of the need for him to feel he could make complaints and would pass on complaints either to his care manager for investigation or to the Commission. The home has a copy of the Isle of Wight adult protection policy and procedure with the manager and her daughters having received adult protection training. The manager was clear about her responsibilities in connection with adult protection and the action she should take should she have any concerns. Discussions with the manager and her husband showed they had a good understanding of abuse issues. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. This judgement has been made using available evidence including a visit to this service. The accommodation provided is to an exceptionally high standard and meets everyone’s needs appropriately. EVIDENCE: The home is an exceptionally well maintained domestic family home and provides suitable and comfortable accommodation with access to all parts of the home and garden. The home is conveniently located on the outskirts of Newport within walking distance of the town, shops, work placement and is on a bus route. Since the previous inspection the bedroom of the person who lives at the home has been redecorated, with a new carpet, new furniture and television. The room’s occupant was involved in this process. A certificate for maintenance from an external contractor was seen during the inspection. A new condensing boiler has recently been fitted. The home has a lounge and dining room/conservatory. In addition there is a safe garden that is used by all family members. The shared space provides plenty of room for everyone. On the day of the unannounced inspection the home was very clean, warm, tidy and well maintained. There were no visible hazards. The person who lives at the home is encouraged to take responsibility for placing his dirty clothes in the washing machine but the manager does the laundry. The manager has undertaken training in connection with her work and is fully aware of infection control procedures should these be necessary. EVIDENCE: The home does not employ staff and therefore the standards within this section are not applicable. The extended family provide care and support if the proprietors are away and are appropriately trained via their domiciliary care and social services employment. This judgement has been made using available evidence including a visit to this service. The management arrangements of the home are appropriate to ensure the service user’s needs are met. EVIDENCE: The manager has an NVQ level 3 in care. The manager has opportunities to undertake core and additional training via her domiciliary care work, and has attended study days for epilepsy, adult/child protection, medication, Introduction to Learning Disabilities and moving people safely, violence and aggression and mental health awareness. Since the previous inspection in January 2006 she has also undertaken first aid, medications, skills for care induction, vulnerable adults, medication and risk assessment training. The manager’s husband has a food hygiene certificate. The manager also works as a domiciliary care worker for adults and children with mental health and learning disabilities. The manager and her family have extensive knowledge of the person who lives at the home as a result of caring for him since he was a child and is well regarded for the care that is given to him. The home is a family home and the atmosphere is one of warmth and caring with each member sharing views and desires. The manager shares her role with her husband, but takes most of the responsibility for the paperwork. During the inspection the interaction between the person who lives at the home and the manager/her husband was positive and he clearly felt able to state his views and opinions. Although there are no formal quality assurance processes in place there are reviews of the placement by Social Services. This gives everyone an opportunity to voice his or her views. These reviews are detailed and written records maintained. There were no concerns in respect of health and safety identified during the inspectors visit.
