Conditions of registration: The total number of registered placements will be 21 (twenty one). Mental Disorder (MD) age range 40 years and over Date of last inspection 26/1/05 Brief Description of the Service: Alban House provides accommodation and varying degrees of care and support for 21 service users over the age of 40 years. The home promotes independence, choice and respect. There is a family and informal atmosphere in the home. The property is situated in the residential area of the coastal town of Ilfracombe. It is close to the facilities of the town centre and is easily accessed to all areas by local transport. The accommodation is provided on four levels and is unusual in that it is a large terraced house, which was formerly 3 separate houses converted to make 1. The home is in the process of continually being updated and refurbished by the owners. There are a variety of private rooms and communal areas. All of the bedrooms are single, some with ensuite facilities. A shaft lift provides access to all areas. The home will accommodate service users pets where possible. This is an overview of what the inspector found during the inspection. The inspection took six hours over one day. Records, and policies and procedures were examined. Nine residents gave their views of the home to the inspector. Comment cards were received from four relatives and five residents. Three staff, and the owners were spoken to during the day. A tour of the premises took place to monitor progress with the refurbishment of the home. Nine residents were spoken to during the inspection, and their comments included: “The staff and [the owners] are all very kind”, “they’re all really lovely”, “Very kind and considerate people”, “I’m quite contented here”, “This is the best place I’ve lived in”. Comments received from relatives in comment cards included: Alban House has “a warm caring environment”, “a friendly homely atmosphere”, “I cannot thanks the owners and staff for all the care they give.” The people living at Alban House told the inspector that they did not like the term ‘service user’ and that they preferred to be referred to as ‘residents’. This term is used throughout the report. What the service does well:
Alban House has a friendly, homely atmosphere. There is a stable team of staff that are responsive to the changing needs of their residents. They make good use of health and social care resources available in the community to ensure that residents receive the best care available. Residents are treated with dignity and respect. They are actively encouraged to voice concerns about their care, safe in the knowledge that their views will be respected and properly investigated. Residents live in comfortable accommodation that is suitable for their needs and is maintained to a good standard. Major renovation work to the home continues and is being done in a way that ensures inconvenience to residents is kept to a minimum. Robust recruitment procedures and opportunities for training and development ensure that suitably qualified and experienced staff care for the residents at Alban House.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,6 Residents needs are well known, and met by competent carers. EVIDENCE: Three care files were inspected. A summary of the care management assessment and care plan had been obtained prior to each individual’s admission to the home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 Alban House staff are responsive to the changing needs of their residents. They make good use of health and social care resources available to them in the community to ensure that residents receive the best care available. Residents are treated with dignity and respect. EVIDENCE: Care plans were well-maintained, accessible to residents and/or where applicable their next of kin. Relatives in comment cards verified that they were kept fully informed about their family member’s care. Care plans inspected had been regularly reviewed. The inspector tracked the care for three residents and saw evidence of good outcomes for the people concerned. For example, one person’s care plan clearly identified risks with regard to mental health issues, and set out a strategy for minimising those risks. Records for that person demonstrated that the home had good relationships with other social and healthcare professionals such as the consultant psychiatrist, speech therapist, district nurses and community psychiatric nurses. The inspector observed staff to be kind and respectful towards residents. During a conversation, a resident told the inspector that they had been on a

Five residents in comment cards, and nine others during the inspection, verified that they felt well cared for and that they were treated with respect. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 Residents are able to voice concerns about their care, safe in the knowledge that their views will be respected and properly investigated. Policies, procedures and training are in place to protect residents from abuse. However, the manager needs to ensure that all of the staff are made aware of the adult protection procedures during the induction training. EVIDENCE: The home has a complaints procedure that residents and relatives were aware of. Pre-inspection information verified that 3 complaints had been investigated in the last 12 months. A record of complaints had been kept by the home, which demonstrated that the issues had been properly investigated and appropriate action taken. No complaints were received during the inspection. Two out of three staff were not aware of adult protection procedures. They did verify, however, that the induction training had covered the issue of abuse and were shown the “No Secrets” video. Induction training records demonstrated the issue of abuse had been covered. The manager verified that no referrals had been made to the Protection of Vulnerable Adults list. Nine residents were spoken to during the inspection, and a further five in comment cards felt that the staff treated them well. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25,26 Residents live in comfortable accommodation that is suitable for their needs and is maintained to a good standard. Major renovation work to the home continues and is being done in a way that ensures inconvenience to residents is kept to a minimum. EVIDENCE: The inspector toured the premises, and was shown areas such as the laundry and a disabled bathroom, which have been completely refurbished. Bedrooms that had been renovated had new carpet, furniture and fittings. Domestic staff are employed for twenty five hours per week. The home was clean and odour free. A passenger lift provides access to all floors, and was in full working order. Nine residents told the inspector that they had been encouraged to personalise their bedroom, and had done so with their own furniture, photographs and ornaments. One person said “I chose the colours for my room from a chart” and “everything gets fixed really quickly if needed by [the handyman]”. Since the last inspection, the registered persons had contacted the Fire Safety officer for advice. The inspector was shown a letter from the
Alban House D54-D07 S22139 Alban House V232506 170805 Stage 4.doc Version 1.40 Page 14 Fire Safety Officer about self-closing devices for fire doors, and went on to observe that the recommendations had been implemented throughout the home. The total communal space available provides at least 4.1m² for each service user. The accommodation provided comfortable, homely fittings and fixtures. Eight bedrooms have ensuite facilities. Pre-inspection information verified that a further ensuite and wc had been fitted since the last inspection. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29 Resident’s needs are well met by a stable team of staff. Robust recruitment procedures and opportunities for training and development ensure that suitably qualified and experienced staff care for the residents at Alban House. EVIDENCE: Staff were observed to be relaxed and unhurried as they went about caring for the people living at Alban House. The duty roster for the week beginning 15th August 2005 was inspected and demonstrated that there were sufficient numbers of staff on duty. Four relatives in comment cards verified that their opinion there was always sufficient numbers of staff on duty. Nine residents were spoken to during the inspection, and their comments included: “The staff and [the owners] are all very kind”, “they’re all really lovely”, “Very kind and considerate people”, “I’m quite contented here”, “This is the best place I’ve lived in”. Four personnel files were inspected, and found to contain written references, including CRB and POVA checks and other information required by law. According to the pre-inspection questionnaire, 50 of the care staff had attained an NVQ care qualification. Three staff were interviewed and told the inspector that they were actively encouraged to undertake training to develop their knowledge and skills.
