Date of last inspection 4.11.04 Brief Description of the Service: Aldyn is a care home registered to accommodate up to 12 service users who have suffered from mental disorder, three of whom may be over the age of 65 years of age. There are conditions of registration that only service users under the age of 65 may be admitted, and that the total number of persons accommodated must not exceed 12.The registered provider is Mr. Babajide Folarin Ademujimi, who is also the registered manager.The home is situated in Bognor Regis, close to town centre shopping facilities and train and bus transport. This is an overview of what the inspector found during the inspection. The inspection took place over 4 hours on the 15th July 2005, and was arranged to follow up recommendations and requirements made at the previous inspection. The inspector interviewed four residents and two members of staff, did a partial tour of the premises, and inspected a sample of care records, policies and procedures, and safety records. A telephone discussion with the registered provider and manager Mr Ademujimi was held on the 20th July 2005, during which feedback on the inspection was given. The inspector would like to thank all residents and staff who contributed to the inspection. What the service does well: What has improved since the last inspection?
The smoking room and three bedrooms have been redecorated, and new carpets have been laid in several bedrooms. A questionnaire for seeking the views of residents on the service provided has been prepared, but not yet used. Procedures for assisting residents with their finances have been written and made available. Written consent to medication is being sought. Some new policies and procedures have been provided, which are clear and well written. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 4 Arrangements should be put in place to minimise all risks identified before a resident is admitted to the home. EVIDENCE: One set of admission records was sampled by the inspector. These indicated that an assessment of needs and risk had been carried out by qualified staff before admission. There was a lack of evidence in the records or care plans to indicate steps being taken to minimise risks identified. Registered provider and manager Mr Ademujimi discussed with the inspector steps that had been taken to minimise some of the identified risks. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6 Care plans need to better reflect residents needs and aspirations, and provide guidance for staff on how these will be achieved. Care plans also need to indicate how assessed risks will be managed by the service. EVIDENCE: Four sets of care plans were sampled by the inspector. Care plans seen were set out well, but did not indicate how residents’ social and developmental needs and aspirations were being assisted. Care records seen did not indicate that there was much in the way of one to one activities taking place with residents. Some structured activities do take place, but these tend to be activities such as assisting with tidying and cleaning bedrooms, and prompts with hygiene. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 15 Staff and residents get on well, and residents are encouraged to maintain contact with family and friends. The provider needs to review if enough outings, activities, and one to one support are being provided for residents. EVIDENCE: Residents were observed to interact well with each other and to share a good camaraderie. Residents interviewed talked of how staff assist them to maintain contact with family and friends. The statement of purpose includes a policy for visiting. Residents interviewed said there was a lack of activities and outings being provided. Staff said that trips to Butlins were being discussed with the manager. Four sets of care records for residents covering a seven day period were read by the inspector. Care records seen indicated a lack of planned one to one support activities taking place with residents. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21 Access to support and health services is being provided for residents. Lockable spaces in bedrooms should be provided for residents who administer their own medication. EVIDENCE: Care records seen and interviews with residents indicated that residents are accessing mental health and health care services as appropriate. Records seen indicated that residents’ written consent to medication is now being sought. Staff said that one resident administers his own medication, but did not have a lockable space in his bedroom for the medication. A medication policy and procedure has been written, but not signed or dated. Procedures have been provided in the event of the death of a resident. Guidance for staff on residents suffering serious or terminal illness has not been written yet. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 23 Adult protection procedures should include more guidance for staff. EVIDENCE: A vulnerable adults policy has been written and dated, but not signed. Guidance on referring staff for inclusion of the protection of vulnerable adults register is not included, and there is no guidance on circumstances under which incidents should be referred to police, the local social services adult duty team, and CSCI. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 30 The programme of improving the decoration and furnishing of bedrooms and bathrooms should continue. EVIDENCE: The grounds and exterior of the property are being kept in good order. Improvements to the premises have included the redecoration of three bedrooms, new carpeting in some bedrooms, and redecoration of the smoking room. Rooms that were seen that have been redecorated had been completed to a good standard. Mr Ademujimi said that planned improvements included the redecoration and refurbishment of bathrooms and toilets, and that quotes had been obtained for this work. The home is clean and hygienic. JUDGEMENT – we looked at outcomes for standard(s) 35 Staff are being appropriately trained. EVIDENCE: Staff interviewed provided examples of how they are assisting residents to communicate their needs and develop their confidence and independence skills. There was evidence that relevant core skills training and National Vocational qualification training are being provided for staff. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38, 39, 42 The home has a good atmosphere, and residents find the staff and manager approachable and sympathetic. Assessed risks need to be addressed. EVIDENCE: Staff interviewed said they enjoyed working in the home. Interactions between residents and staff observed by the inspector indicate that residents find the staff and manager approachable and sympathetic. Residents indicated that there is a friendly and good atmosphere in the home. Some new policies and procedures have been provided which are dated, but not signed. These include a health and safety policy statement, and health and safety at work. It was not clear if there was a health and safety representative for the home. Records for fire tests, procedures and drills were sampled. Mr Ademujimi said that a format for questionnaires to ask residents for their views on the service

No arrangements are in place to seek the views of relatives, friends and advocates. The inspector asked the manager to review if risks highlighted in individual assessments were being properly addressed.
