This inspection was carried out on 8th December 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 3 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

There is a relaxed and supportive atmosphere in the house. Residents are being supported towards a more independent lifestyle, and to make their own choices. Residents are being supported to enjoy their leisure time and make use of local community facilities. A balanced diet is being provided in cooperation with residents. The staff team is experienced and receives appropriate training, and are supported in their work with residents by team meetings, handovers and individual supervision. Staff have a good rapport with residents, and encourage them to develop their self confidence and independence skills.

What has improved since the last inspection?

What the care home could do better:

Care plans need to indicate what the resident`s goals and aspirations are. Residents` wishes with regard to ageing, illness and death should be recorded. Adult protection procedures should be updated to better protect residents. The provider must ensure all parts of the home are in a good state of decoration, and that lockable spaces and enough electrical sockets are being provided in bedrooms. All policies and procedures need to be signed, dated, monitored and reviewed by the registered manager. Regular safety checks and risk assessments need to be carried out. The views of residents, their family, friends and representatives on the service provided need to be sought.

Date of last inspection 15th July 2005 Brief Description of the Service: Aldyn is a care home registered to accommodate up to 12 service users in the category of younger adults (18-65) with a mental illness, up to 3 of whom can be over the age of 65. The registered provider is Mr Babajide Folarin Ademujimi, who is also the registered manager. The premises are sited near Bognor Regis town centre, with access to bus and train services, shopping and other community facilities. At the time of this inspection, all residents accommodated were male. The inspection was arranged to follow up previous requirements and recommendations which had been made. The inspection lasted 4 hours. The inspector spoke with four residents, two members of staff, and a visiting professional. A partial tour of the premises was made, and records and care plans were sampled. The inspector would like to thank everyone who contributed to the inspection. What the service does well: What has improved since the last inspection? Residents are becoming more confident and taking more initiative in the house – for example, in re-arranging the television room and choosing Christmas decorations. Residents are socialising more with their peers inside and outside of the home, and making more use of local facilities. Regular orders from a local greengrocer are ensuring that more fresh food is available. Improvements to the premises since the previous inspection have included the provision of some new bedroom furniture and beds, some redecoration of bedrooms, and the redecoration and refurbishment of a bathroom. Staffing levels have been increased by the addition of a full time cleaner, thus allowing care staff to be spending more one to one time with residents. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2, 4 Arrangements are in places for admission to the home which include preadmission assessment and visits and overnight stays. EVIDENCE: One set of admission records was sampled. The records indicated that the resident had stayed overnight on separate occasions for one, two or three nights before moving in properly. Pre admission information, assessments and risk assessments were seen on the admission records sampled. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 9 Residents are being supported towards a more independent lifestyle. Care plans need to indicate what the resident’s goals and aspirations are. EVIDENCE: Three sets of care plans and care records were sampled, which indicated that plans of care and the review of these are being provided. It was recommended at the last inspection visit that the individual service user plan include how residents will be assisted to meet their aspirations and achieve their goals. While discussion with staff indicated that residents are being supported to develop their confidence and independence, the individual service user plans seen did not indicate what the residents’ goals and aspirations were. Examples were provided of how staff are ensuring that residents’ independence is being promoted, and how residents are exercising more choice in the menus provided, choosing furnishing, decoration and carpets for their bedrooms, and Christmas decorations and planning. Registered manager Mr Ademujimi said that residents had said they’d like a shower room, and at the time of the inspection there was a plan to convert a bathroom into a shower room. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 16, 17 Residents are being supported to enjoy their leisure time and make use of local community facilities. A balanced diet is being provided in co-operation with residents. EVIDENCE: Three residents were attending a local day centre on the day of the inspection, and records seen indicated how staff had supported residents to visit the centre and develop a pattern of attendance. One recent development has been that a number of residents have been going to a monthly disco which is giving some of them the opportunity to meet up with old friends, and staff are providing transport for this. Mr Ademujimi said that staff had arranged for more games and puzzles to be bought for the house. Residents are accessing local facilities such as the library, café, pubs, shopping, and going out for walks. Staff said they arranged for residents interested to attend a weekly lunch event in the local community, but that after a few weeks residents no longer wished to attend. Aldyn Care Home DS0000043596.V261708.R01.S.doc Version 5.0 Page 11 During this visit it was evident that residents are taking more initiative than previously towards their communal living. For example, residents had rearranged the television room, and were taking responsibility for housework and shopping. Residents have also been involved in choosing Christmas decorations. On the day of the inspection, the lunch provided was individual pies, fresh mashed potatoes and vegetables, which were carefully prepared and much enjoyed by the residents. The inspector discussed with the cook how residents are supported to eat a healthy diet, and was advised that there are regular orders from a local greengrocer, and that fruit is always on the table. The inspector was advised that after one resident had developed diabetes, other residents decided together to cut out puddings other than yoghurts and occasional treats. Residents are involved in choosing the menus, and where a resident does not like what’s on the menu the cook plans to provide an alternative in advance. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 21 It is recommended that policies and procedures with regard to the ageing, illness and death of a service user be developed. EVIDENCE: Two residents are presently responsible for their own medication, and a lockable place for these medicines in bedrooms is now provided. It was recommended at the previous inspection that a policy and procedure be provided to ensure that residents’ wishes are recorded in the event of them becoming seriously ill or dying. Mr Ademujimi said this had not been commenced. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 It is recommended that adult protection procedures should include guidance for staff on the referral of incidents to the police, local authority adult duty team, non-contamination of evidence, and the provider’s responsibility to refer staff to the Protection of Vulnerable Adults register (in accord with guidance provided by the Department of Health). EVIDENCE: Mr Ademujimi advised that adult protection procedures have not been revised as recommended at the previous inspection. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 26 It is required that the registered person ensure that all parts of the home are kept reasonably decorated. EVIDENCE: Improvements to the premises since the previous inspection have included the provision of some new bedroom furniture and beds, some redecoration of bedrooms, and the redecoration and refurbishment of a bathroom. On the day of the inspection work was taking on the redecoration and refurbishment of a further bathroom. The inspector visited three bedrooms and found that some of the furniture and flooring remains in need of replacement, and advises that the provider should ensure that lockable spaces and enough electrical sockets are being provided in bedrooms. It was also noted that a window frame near the back door was in need of replacement or repair. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 36 The staff team is experienced and receives appropriate training, and are supported in their work with residents by staff meetings, handovers and individual supervision. Staff have a good rapport with residents. EVIDENCE: It was the view of a community psychiatric nurse who talked to the inspector during this visit that the manager and staff at Aldyn were professional in their approach and dealings with the local mental health team. Mr Ademujimi advised the inspector that staffing levels have been increased by the addition of a full time cleaner, thus allowing care staff to spend more one to one time with residents. On the morning of the inspection, the staff complement was two care staff, the registered manager, and the cook. Staffing levels allowed one member of staff to be out shopping with a resident on the morning concerned. Staffing is also being provided for individual activities such as accompanying a resident once a month to visit his mother in London. Staff interviewed said they benefited from the training and individual supervision time provided. Staff said that recent training undertaken had included administration of medication, health and hygiene, food safety, and understanding psychosis. Staff said they also found handovers and staff meetings helpful. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39, 40, 42 It is a requirement that a system for seeking the views of residents on the service be put in place. It is a requirement that the registered person shall ensure that unnecessary risks to the health and safety of service users are identified and so far as possible eliminated. EVIDENCE: The registered manager Mr Ademujimi is presently undertaking the National Vocational Qualification in care Registered Manager award. Staff interviewed said that during supervision meetings Mr Ademujimi provides helpful advice and support, and highlights for staff what training might be the most useful for them. Mr Ademujimi said that no surveys to seek the views on the service provided of residents or their families, friends or representatives had been carried out. Mr Ademujimi said that improvements to the service suggested by residents were usually taken up, such as the proposal to turn a bathroom into a shower room. There was therefore no evidence that the provider has sought to assess the safety of the home and identify environmental risks as had been made a requirement at the previous inspection. Discussion with Mr Ademujimi indicated that although recommended at the previous inspection, policies and procedures have not been signed, dated, monitored and reviewed.
