This inspection was carried out on 12th September 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Provides a good quality of care to the service users. There is a good team spirit and staff spoken to were enthusiastic about the service that they provide. The registered manager has an open-door policy and service users and staff have access to a member of the management team at anytime.

What has improved since the last inspection?

One member of the management team now visits all prospective service users to ensure that the person can be accommodated within the home and that the staff team have the skills required to care for people admitted. Activities available to service users have increased giving a wider choice of activities within the home and outings into the community. Activities available each month are displayed within the home.

What the care home could do better:

Service users considered that they were provided with good care, accommodation and food and could not suggest any improvements to the service that they received at the present time.

It is registered for 34 older people, however, due to the number of double rooms being used as single rooms a maximum of 31 service users can be accommodated at any one time. North Yorkshire County Council owns the home and Jennifer Beard manages it. It is located approximately half a mile from the centre of Malton and service users can access the shops, the post office and other amenities easily. The home is an adapted property and is set in its own grounds. The grounds are well maintained and accessible to the service users. There is also a passenger lift. A copy of the service users guide to the home is given to prospective service users and a visit is made to the home before a decision is made to move in for a trial period. A copy of the latest Commission for Social Care Inspection Report is available for prospective service users and relatives to read. The weekly fee on the date of the inspection is £368.90. Additional charges are made of £6 for hairdressing and £8 for chiropody. The evidence used to inform this report was obtained from the information documented in previous inspection reports, a pre-inspection questionnaire completed by the registered manager of the service, a site visit, completed survey forms and discussions with 11 service users, the registered manager and four members of staff. Prior to the inspection ten survey forms were sent to service users and seven were returned, six surveys were sent to GPs and five returned and five were sent to care managers and one was returned. All service users surveyed considered that they received good care. This unannounced inspection took place on the 12th September 2006, commencing at 9.30am. A number of records were inspected including service users’ assessments, care plans, medication and health and safety information. A tour of the home was made and staff were observed interacting with a number of service users. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 Quality in this outcome group is good. This judgement has been made using available evidence including a site visit. The assessments prior to admission are comprehensive and provide informed decisions about moving into the home. EVIDENCE: All prospective service users files examined included a comprehensive care management assessment prior to consideration for admission to the home. A member of the management team visits prospective service users in their own home or in hospital to ensure that the facilities of the home and the skills of the staff meet the needs of people wishing to be accommodated. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 Quality in this outcome group is good. This judgement has been made using available evidence including a site visit. The staff have a good understanding of the service users’ support needs. This is evident from the positive relationships that have been formed between the staff and service users. EVIDENCE: All service users have a plan of care. These are reviewed as necessary and at least once a month. All files seen had a care plan and regular reviews. All service users spoken to stated that they received the necessary care from staff if they were unable to take care of all their personal care needs. Service users are registered with a GP. Access to specialist medical services is by referral from the GP. The district nurse visits as required. A chiropodist visits every six weeks. Appointments are made as necessary for opticians and dentists. The records inspected showed that service users receive appropriate medical care. Service users spoken to confirmed this. Comments from GPs who visit the home included “Excellent home, well run by good people” and “Excellent care”. The monitored dosage system for medication was checked. The records and medication were in good order and up to date. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 Quality in this outcome group is good. This judgement has been made using available evidence including a site visit. The meals in the home are good offering both choice and variety and catering for special dietary needs. Social activities are planned and organised. EVIDENCE: Activities are arranged in the home and include quizzes, craft sessions, bingo, chair exercises, puzzles and board games. Entertainers visit the home on occasions. Outings are arranged into the community. Service users commented how they enjoyed the outings. One service user stating “We are going to Whitby this month for fish and chips”. One service user attends chapel twice each week. Members of three different churches visit the home regularly to see service users and to take services for those service users who wish to attend. There is a five-week rotating menu that provides suitable nutrition for service users. The menus rotate three times and then a new menu is introduced. There are alternatives available if service users do not wish to eat the set meals for the day. Diets are catered for. Service users spoken to stated that the food was very good and that they could always have an alternative to the dishes displayed on the weekly menu if they wished. There are satisfactory abuse and complaints policies and staff are aware of what action to take if a complaint or suspected abuse situation arises. EVIDENCE: One complaint has been made since the last inspection. This was investigated and completed within the 28 day time scale and was upheld. The member of staff concerned was dealt with appropriately. Staff are aware of what action to take in the event of a suspected abuse situation. The standard of the environment within the home is good providing comfortable communal rooms and individual bedrooms for service users. EVIDENCE: Service users stated that they were happy with their individual bedroom accommodation. Some service users stating that they preferred to spend the majority of the time in their own rooms. There are a number of small lounges which give service users choice of where to spend their time. The cubicle toilets are being replaced by private facilities and workmen were converting the first set of toilets on the day of the inspection. The home is clean, hygienic and pleasant. The service users receive a good standard of care from the well-prepared and enthusiastic staff. EVIDENCE: The staff on duty at anytime of the day or night appear to meet the needs of the present service users. Service users spoken to considered that they received the appropriate care that they required. There is a staff training policy and staff have undertaken training in induction and foundation training, NVQ Level 2 in care, food hygiene, back care, dementia awareness, first aid, fire prevention and promoting independence. The employment records on the staff files examined were in order and up to date. The management of the home is satisfactory overall and appropriate records are maintained and kept up to date. EVIDENCE: The registered manager is experienced in the care and management of older people and has a National Vocational qualification in management at Level 4 NVQ. Both staff and service users considered that the registered manager is approachable and available to discuss any issues as they arise. There is a quality assurance system that involves the home checking the service it provides against the national Minimum Standards for Older People. Other information used in this process comes from the service users meetings, a questionnaire sent to relatives and friends and management meetings. The information received is used to develop the service provided at Ashfield.
