The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home had an established staff team who were keen for high standards to be maintained within the home. Residents` care plans and case notes were well documented and reflected the individual residents` needs. Staff supervision was observed during this inspection and formal supervision was recorded and completed on a regular basis. The staff managed daily activities and entertainments. Residents` were pleased with the choices on offer. Residents are well supported by staff. One resident had been assisted in setting up a direct payment scheme, which was used to pay directly for a carer. This carer undertakes activities on an individual basis with the resident.

What has improved since the last inspection?

Staff had planned and booked a holiday for all the residents to go to Portugal in June. The home has maintained its good standards in respect of care of the residents` and the environment since the last inspection.

What the care home could do better:

A statement of purpose and function and residents` guide needs to be produced. (See requirement No 1). Monthly reviews of residents` risk assessments and care plans should be carried out. (See recommendation No 1). The home`s policy on confidentiality should be updated to include guidance to staff on when a confidence may be shared. (See recommendation No 2) The garden area should be maintained to an acceptable standard for residents to enjoy. (See recommendation No 3). Additional space separate form the shared lounge/dining room could be provided where residents can see their visitors. (See recommendation No 4). Satisfaction surveys for residents and relatives should be collated and the outcomes shared with residents and other interested parties. (See recommendation No 5). The annual development plan, which should be based on the annual review of residents, should be completed. (See recommendation No 6). The records that were difficult to locate and were fragmented should be reorganised for ease of use. (See recommendation No 7).

The home is owned and managed by Community Integrated Care, a not for profit organisation. It is located off Liverpool Road in Widnes, on a bus route within easy reach of Widnes Town Centre and Hough Green station. There is a small parking area at the front and a garden with patio at the back. The home is a purpose built dormer bungalow with staff facilities only upstairs. Residents accommodation is on the ground floor and comprises six single bedrooms, two assisted bathrooms, a separate assisted toilet, a large lounge / dining room, a kitchen and utility room. There are no en-suite facilities. This inspection was carried out during the morning and took three hours forty minutes. The inspector spent an hour and a half planning the inspection by reviewing previous inspection reports and the service history. The inspection included a full tour of the premises, inspection of records and discussions with five residents’, the deputy manager and two care assistants. Twenty out of forty-three standards were assessed and most of these were met at this time. Feedback from this inspection was given to the deputy manager at the end of this inspection. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2 & 3 The home did not provide the residents with the statement of purpose and function and residents’ guide, therefore full written information regarding the home was not available. Full assessments of needs are carried out to ensure that the home can meet the residents’ needs. EVIDENCE: The manager said the home’s “welcome to Bankfield House” booklet was shown to prospective residents. Within the policy file, policies on philosophy of the home, client’s charter and mission statement were seen. A statement of purpose and function or residents’ guide was not available. This information is required to enable prospective residents and their families make an informed choice about whether the home is suitable for their needs. (See requirement No 1). The manager said each service user is assessed prior to admission and this information forms the basis of the person’s plan of care. Records showed that residents were assessed prior to admission and most residents had lived at the home for many years. The process of assessment had changed over the years and new resident said he was happy with the assessment process undertaken and that he visited the home prior to admission. This assessment is used as the basis for the care plan and ensures that the resident’s needs could be met by the home. Staff confirmed that residents’ needs were assessed prior to admission. It was seen that each service user had access to GP’s, chiropodists, opticians, dentists and other professionals. This information was in the residents’ plan of care. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 9 & 10 The residents’ health, personal and social care needs are met by the staff team who enable them to maintain their privacy and dignity. EVIDENCE: A sample of two residents’ files were seen. These were presented in individual ring binders and contained a copy of the terms and conditions of residence, basic information, review of care plan by social services, care plan sheets, essential life plan, “my health” document, visiting professionals sheets, risk assessments, personal activity programme, body map and residents’ evaluation sheets. The care plans were written in consultation with the residents and their families and were based on their assessed needs and risks. These should be reviewed on a monthly basis, however, both care plans seen had not been completed within the last month. (See recommendation No 1). Case notes showed that residents’ day-to-day activities were recorded. Staff stated that all residents’ had been involved in choosing the décor of the shared areas. One of the residents had purchased through the “direct payment scheme” a carer, who worked for the resident and assisted the resident in accessing activities within the local community. Information in the care plans was accurate and well recorded and the documents were kept safe and secure. The home’s policy on confidentiality should include the registered provider’s policy on sharing information with family, friends and professionals. It needs to be clear about what confidential information would be shared, for example if a service user made allegations of abuse. (See recommendation No 2). Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 14 & 16 Residents were able to take part in some leisure activities, either in the home or the local community. The staff team supported residents’ with personal and family relationships as required. EVIDENCE: The residents’ plan reflects the range of activities undertaken which included a lifestyle programme of mainly daily living activities, such as baking, beauty treatments, videos, DVD’s, music and individual time with staff members. Activities outside the home included shopping, visits to cinema, bowling, meals out and visits to the local pub. On the day of this inspection one service user was attending the local college during the afternoon. Daily routines of residents were clearly documented in the care plans seen and the plans were well written. The deputy manager said all the residents’ were going on holiday to Portugal in June. Residents confirmed that they had been involved in choosing the holiday destination and were looking forward to going away for the week. Residents could see visitors within their own room or in the shared living areas. Staff said that one of the ways they assisted residents was by taking them to see family members. The kitchen was clean and tidy. During a tour of the home residents were seen spending time alone in their rooms whilst others were enjoying social contact with others in the lounge/dining area. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19 & 20 Residents received support from the staff for personal care in their preferred way. Administration and control of medications were appropriate for the needs of the residents. EVIDENCE: One care plan seen described how the resident preferred to be supported with all areas of daily living throughout the day. Preferred moving and handling techniques and personal care choices were described. Residents were dressed differently according to their own choice. Personal support was provided in private and the residents had a designated key worker. All aids and adaptations required for the residents were provided as needed. During a tour of the home it was seen that various types of equipment were available for residents support and use, such as tracking equipment in bedrooms, raised toilet seats and electric wheelchairs. In the kitchen a locked steel cupboard was available for the secure storage of medication. A monitored dosage system was used and all medication was stored appropriately. The medication administration sheets were signed by the staff and were up to date. Weekly checks on the medication were recorded on the medication administration record sheets. Visits to health professionals were recorded in the care plans, including visits to GP’s, opticians, chiropodists and dentists. These were up to date. The deputy manager said a carer always attended appointments with residents. A previous recommendation made about obtaining consent to the receipt of medication for the residents from relatives or advocates had been acted on. The deputy manager explained that a document had been produced and sent out to relatives, however, only a couple of responses had been received. The manager said that she would contact the people involved who had not responded to this request. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 23 Clear policies and procedures were in place to ensure that residents were protected from abuse and neglect. EVIDENCE: The home’s Protection of Vulnerable Adults Policy was seen and this was consistent with the “No Secrets” guidance from the Department of Health. A copy of Halton’s Local Authority Adult Protection policy was available within the home and accessible to staff. The home’s whistle blowing policy was seen. The deputy said that staff had undertaken training on Adult Protection, which was covered by a video, talks and the staff reading the Protection of Vulnerable Adults policy and guidance. Staff confirmed that they had received this training, and were able to demonstrate what to do if there was a suspicion of abuse. This was also covered in the induction process. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24 & 30 The home provided a clean and comfortable environment for the residents to live in. EVIDENCE: The home was furnished in a domestic style with additional equipment such as bars in toilets and bathrooms, rails along corridors and moving and handling equipment provided to meet each person’s needs. The décor in bedrooms was to resident’s preference and the residents and staff confirmed this. Random samples of hot water temperatures were taken during this inspection and the temperature was slightly below the recommended guidance of 43 degrees centigrade. The staff explained that this was due to the hot water being off during the afternoon. Staff said the settings could be over-ridden if required but that most of the hot water was needed during the morning and evening periods. A weekly health and safety check was carried out and records of this confirmed acceptable hot water temperatures around the home. The home was clean, tidy and free from unpleasant smells. This area was clean and tidy. Cleaning materials were stored in a locked cupboard. The deputy manager said that Control of Substances Hazardous to Health data sheets were available, and staff confirmed they were aware of the data sheets. Patio tubs had been planted to provide for summer colour in the garden. The garden area in general was in need of weeding and the lawns mowing. The deputy manager said the home was awaiting a new tender for garden work. (See recommendation No 3). Residents can see visitors in the combined lounge/dining area or in their own bedrooms. No separate space is available for this purpose. A recommendation in the last inspection report with regard to additional space for residents’ visitors was discussed with the deputy manager who said that plans were being drawn up with regard to erecting a conservatory. (See recommendation No 4). Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32 &36 The manager provides clear leadership. Records were well maintained. Staff received support to enable them to meet residents’ needs. EVIDENCE: At the time of this inspection the agreed staffing levels were met. The staff team was well established. They had a range of experience and this was complimented by mandatory courses undertaken, such as moving and handling and food hygiene, NVQ training in Care and other relevant courses. Observed day-to-day supervision of staff was good and the staff team confirmed that the management team supported them. The deputy said that formal supervision was conducted on a regular basis and records kept. Examination of the records confirmed this practice was taking place. JUDGEMENT – we looked at outcomes for standard(s) 39 &41 Residents’ views were used in the planning for the home. Decisions are influenced by the information obtained from the completed satisfaction surveys and from conversations with residents. EVIDENCE: The manager had given satisfaction surveys to residents’ and relatives. These details were currently being examined and the manager said that this information would be used to improve the service provided. This was a previous recommendation in the last inspection report and remains outstanding. It is also recommended that this process continue and the outcomes shared with residents’ and other interested parties. (See recommendation No 5). A previous recommendation was made with regard to the annual development plan, which should be based on the annual review of residents’ needs. Records seen were kept in good order. These were in line with the Data Protection Act 1998. The deputy manager said that residents had access to information stored about them. Residents said they were aware of information kept about them. From records seen some information was difficult to locate and was fragmented across several files. It is recommended that this information be reorganised for ease of use. (See recommendation No 7).
