The inspector found no outstanding requirements from the previous inspection report, but made 1 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The home has an established staff team who were keen for high standards to be maintained within the home. Residents` care plans and case notes were well documented and reflected the individual residents` needs. The staff managed daily activities and entertainments. Residents` were pleased with the choices on offer. An activities organiser has been employed and this has increased the activity time available to service users. Residents are well supported by staff. One resident had been assisted in setting up a direct payment scheme, which was used to pay directly for a carer. This carer undertakes activities on an individual basis with the resident. Resident`s comments included "I visited Bankfield on numerous occasions prior to my transition", "staff treat me well", "I do various activities both day and night dependant on my health", "the home is always fresh and clean" and "I like living in the home". The relatives contacted confirmed that they were welcomed into the home and they were satisfied with the overall care provided. Other comments included "We are very satisfied with the care, love and attention that our relative receives" and "the service users are well provided for". Staff spoken to commented, "I like working here" and "I like to support the service users".

What has improved since the last inspection?

So that prospective residents and relatives can have relevant information about the home a statement of purpose and function and service users guide has been produced. The healthcare needs of the residents are now kept up to date to ensure all relevant medical needs are met. To ensure that residents are cared for by appropriately qualified staff, 50% of the support staff team are now qualified to NVQ level II or above. The homes policy on confidentiality, which details what, should or not be discussed with others, had been updated to include guidance to staff on when a confidence may be shared. Additional space has been considered to accommodate a room for visitors to see residents in private without having to use their bedrooms. Satisfaction surveys for residents and relatives are now being used to obtain views of the service provided.

What the care home could do better:

To ensure that suitably qualified staff looks after residents, all staff must receive training in Adult protection from Abuse. To ensure that all views are taken into account with regard to future planning of the home the quality assurance process should be developed and shared with residents and others interested parties. For the protection and promotion of the welfare of the residents the accident book information should be kept in line with the Data Protection Act 1998.

The home is registered for a maximum of 6 service users in the category LD (Learning disability) 8th November 2005 Date of last inspection Brief Description of the Service: 2a Bankfield Road is a care home providing 24-hour personal care for 6 adults with learning disabilities. The home is owned and managed by Community Integrated Care, a not for profit organisation. The home is located off Liverpool Road in Widnes, on a bus route within easy reach of Widnes Town Centre and Hough Green station. The home is a purpose built dormer bungalow with only staff facilities upstairs. Residents accommodation is on the ground floor and comprises six single bedrooms, two assisted bathrooms, a separate assisted toilet, a large lounge / dining room, a kitchen and utility room. There are no en-suite facilities. There is a small parking area at the front and a garden with patio at the back. The staff team consists of the acting manager who is supported by eight support staff, an activities organiser and a housekeeper. The fees at Bankfield Road are £57.45 paid by the service users and £322.55 paid by Social Services or other sources. Optional extras include personal items, transport costs, other activities and hairdressing. An unannounced visit took place on 9 January 2007 and lasted five and a half hours. Maureen Brown carried out the visit. Feedback was carried out with the acting manager at the end of the visit. This visit was just one part of the inspection. Before the visit the home was also asked to complete a questionnaire to provide up to date information about them. Questionnaires were also made available for service users, relatives, staff and other professionals to find out their views. Other information since the last key inspection was also reviewed. During the visit various records were looked at and a tour of the home was undertaken. A number of service users and staff were also spoken with and they gave their views about the service. All the key standards were assessed and all were met. All previous requirements had been met. The overall quality rating for Bankfield Road is excellent. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1&2 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Good information is provided for residents to make a decision about moving into the home. EVIDENCE: A statement of purpose and function and residents’ guide had been produced following a previous requirement. This was available in standard and large print and was written in plain English and available in audio format. Within the Service Users Guide it states “service users have a right to be respected and treated as an individual and to expect fair treatment and personal planning for the future.” Residents confirmed “I visited Bankfield on numerous occasions prior to my transition” A copy of the latest inspection report was available and staff knew where it was and its content. The care needs assessment was seen in service users files. This contained all the information required to ensure that the home can meet service users needs. Included was Next Of Kin information, personal support and care needs and medical information. This is completed initially and used in conjunction with the Social Services information. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 9 & 10 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The residents’ health, personal and social care needs are met by the staff team who enable them to maintain their privacy and dignity. EVIDENCE: Both service user files viewed had a complete needs assessment and care plan. Copies of the service users guide, summary of abilities and activities of daily living were also available. All these were clearly detailed and were up to date. Social services reviews were up to date. The care plan files contained many photographs of service users and pictures of the equipment they used for example hoists, cups, bedrooms etc. Each service user had a communication dictionary, which stated, “when I do” ………”it means”…”and you should ……”. This was a very useful tool for the staff team. In a separate file the daily records and personal activity programmes were seen. There was a good record of daily activities, which were well recorded and appropriate to each service user. Service users confirmed “I do various activities both day and night dependant on my health”, “I will let the staff know what I want to do by eliminating other choices when asked by staff” and “I am involved in all aspects of the home and duties”. Risk assessments were available for both service users. A range of activities were noted such as moving and handling, dysphasia, choking, hot water temperatures, incontinence needs and radiator temperatures. These had been reviewed in November 2006. A previous recommendation regarding the confidentiality policy had now been met. The guidance to staff now includes information on when a confidence may be shared. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16 & 17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ were able to take part in a range of activities. Personal and family relationships were encouraged by the home and the staff team supported people with this. EVIDENCE: Service users are engaged in a range of educational and occupational pursuits. One service user is the chairwoman of Social Inclusion and is support by staff in this work. Also one service user attends college for an arts and craft course. At this time the acting manager stated there were very few suitable courses available but she monitored the situation and advised service users when appropriate courses were available. Service users freely use the local and wider community. The acting manager stated that this was a very important area for development. She has invited neighbours to the home to meet the service users and some of the neighbours now speak to the service users and staff when they meet. The acting manager Bankfield Road Care Home DS0000005142.V322865.R01.S.doc Version 5.2 Page 12 is also going to plan a Bar-B-Que in the summer and invite family, friends and the neighbours to further develop local relationships. Some service users go out with family members. Relationships are developed as per individual preference. It was noted that family and friends are welcome at the home and are actively encouraged by the staff and manager to visit. Daily routines are appropriate. Service users complete household tasks such as meals, shopping, cleaning and washing with support as necessary. Service users like to listen to CDs, watch TV, Videos and DVDs, go to bingo and go into town. The home provides a six-week menu. A good variety of meats, cheese, fish, fresh vegetables and fruit were included and this provides a well balanced diet. Service users are assisted in choosing the meals they would like. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 & 20 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The residents’ health, personal and social care needs are met by the staff team who enable them to maintain their privacy and dignity. EVIDENCE: Personal care and support are provided in service users own rooms or bathroom. Support is given in line with service user plan details. Residents confirmed, “I am treated very well and enjoy living here and l like all the staff who support me”. Healthcare information is written on separate sheets. All service users had received a medication review and healthcare reviews within the last year. The sheets recorded optical, dental, GP, chiropody and continence assessment dates. These were all within last year. The medication system was seen. Medication Administration Record sheets were appropriately completed. The medication is a monitored dosage system provided by Boots chemist. The medication is locked in a cabinet, which is stored in the kitchen. The acting manager stated that they had a good relationship with the pharmacist. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents and relatives were satisfied with the support they received from the manager and staff. Clear policies and procedures were in place to ensure that residents were protected from abuse, neglect and selfharm EVIDENCE: The complaints procedure was appropriate and a copy was available with the Service Users Guide and Statement of Purpose and Function. All service users had a copy. No complaints have been received by CSCI or the home since the last visit. One relative stated that they were not aware of the complaints procedure. The home has signed up to the Halton Borough Council “No Secrets” policy. They also have policies on abuse and suspected abuse; abuse guidelines; adult protection POVA guidance; whistle blowing and harassment and bullying procedures. Four of eleven staff are trained in Protection Of Vulnerable Adults. A requirement was made that all staff must receive Abuse Awareness training. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24 & 30 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The home provides a clean and comfortable environment for the people to live in. EVIDENCE: The overall state of repair of the home both internally and externally is very good. The communal areas and some bedrooms were seen on this visit. The home was noted to be clean and odour free which all the service users confirmed and stated “My home is always clean and tidy” in the questionnaires received. The bedrooms seen had been redecorated since the last visit and were to individual’s personal taste and personal possessions were in evidence. Planned work for the forthcoming months includes new curtains; new driveway; refurbishment of the kitchen; decoration of corridors, sleep-in room, office, bathrooms, lounge and dining room. Since the last visit a vegetable plot has been developed in the garden. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34, 35 & 36 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users are protected by the homes recruitment policy and practices as they are consistently followed. EVIDENCE: The staff team comprises of the acting manager, 8 support staff, activities worker and a housekeeper. The rota showed appropriate levels of staff on duty. Four of the eight support staff have NVQ level II or above, which means that 50 of staff are qualified. Three staff files were examined. All staff had pre-recruitment information available. All staff had undertaken Criminal Record Bureau and identity checks carried out. The staff files were well presented and subdivided into areas making it easy to find all the relevant information. A good range of mandatory training is available. Moving and handling, first aid, food hygiene, fire prevention and health and safety have been undertaken. Some staff have completed Abuse Awareness training. Most staff had undertaken some specialist training. Staff appraisals had been completed during the last year. Staff supervision on day-to-day basis was good and formal supervision was undertaken with records kept. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The health, safety and welfare of the service users are protected. The views of service users are obtained and influence the running of the home. EVIDENCE: The Registered Manager has left and the post is currently being advertised. The deputy has been acting manager for three months. She has NVQ level III in care and is working towards NVQ IV in management. She has worked at the home for two years in a managerial role and prior to this was a support worker for two years. The Fire book was seen and weekly fire tests were carried out. Also emergency lights, fire evacuation tests, fire system servicing and fire training for staff were all up to date. The Gas safety certificate, Portable Appliance Test and Electrical wiring safety certificates were all appropriate. A recommendation was made with regard to the Data Protection Act 1998. The home has a policy on quality assurance and used business excellence for service users and family surveys. This was then published as a company document with all CIC homes included into one. The service users are involved in the six monthly quality monitoring forms where possible and this is split into people we support, staff team and environment. It was recommended that the system be revised to be individual to the home and use other stakeholders as part of the quality assurance process. Key success indicators are measured and monitored on a regular basis. The monthly core-monitoring tool is used and REACH standards, which apply to, supported living services. Also Regulation 26 notices are regularly completed each month and include information on discussions with service users. Resident and staff meetings are held jointly every six weeks. The last meeting was on 10 December 2006 and issues discussed included Care Plans, washing, toiletries, hairdressing, activities, cleaning, staff issues and confidentiality.
