Conditions of registration: Date of last inspection 10th May 2005 Brief Description of the Service: 2A Bankfield Road is a care home providing 24-hour personal care for 6 adults with learning disabilities. The home is owned and managed by Community Integrated Care, a not for profit organisation. It is located off Liverpool Road in Widnes, on a bus route within easy reach of Widnes Town Centre and Hough Green station. There is a small parking area at the front and a garden with patio at the back. The home is a purpose built dormer bungalow with staff facilities only upstairs. Residents accommodation is on the ground floor and comprises six single bedrooms, two assisted bathrooms, a separate assisted toilet, a large lounge / dining room, a kitchen and utility room. This is an overview of what the inspector found during the inspection. This inspection was carried out during the morning and took four hours. An hour was spent planning the inspection by reviewing previous inspection reports and the service history. It included a full tour of the premises, inspection of records and discussions with three residents’, the deputy manager and other staff on duty. Eighteen out of forty-three standards were assessed and most of these were met at this time. One resident, four relatives’, one GP and one health and social care professional comment cards were received. Feedback from this inspection was given to the deputy manager at the end of this inspection. What the service does well: What has improved since the last inspection?
Monthly reviews of residents’ risk assessments and care plans are now being carried out. The garden area is being maintained to an acceptable standard for residents to enjoy. Records that were difficult to locate during the previous inspection had been reorganised and this had significantly improved the ease of use.

The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 The home did not provide the residents with the statement of purpose and function and residents’ guide, therefore full written information regarding the home was not available. EVIDENCE: The deputy manager said the home’s “welcome to Bankfield House” booklet was shown to prospective residents. This was confirmed with relatives. Within the policy file, policies on philosophy of the home, client’s charter and mission statement were seen. A statement of purpose and function or residents’ guide was not available. This information is required to enable prospective residents and their families make an informed choice about whether the home is suitable for their needs. A previous requirement had been made regarding this and although some work had been completed the document was still in draft format and not available. See requirement No. 1. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7 & 10 The residents’ health, personal and social care needs are met by the staff team who enable them to maintain their privacy and dignity. EVIDENCE: A sample of two residents’ files were seen. These were presented in individual ring binders and contained all the information required to adequately care for the individual residents. Included were copies of the essential life plan, “my health” document, risk assessments, personal activity programme and residents’ evaluation sheets. The care plans were written in consultation with the residents and their families and were based on their assessed needs and risks. Monthly reviews were undertaken and up to date. Case notes showed that residents’ day-to-day activities were recorded. The information in the care plans was accurate, well recorded and the documents were kept safe and secure. Within the daily care notes and personal activity programmes each persons’ preferred activities were noted. These included being passively involved in

Also enjoying the community with support as required going to the shops, local cinema and eating out. It was clear the service users were given information on what was available and that they made their own decisions on what to do supported by the staff. The home’s policy on confidentiality should include details on sharing information with family, friends and professionals. It needs to be clear about what confidential information would be shared, for example if a service user made allegations of abuse. See recommendation No. 1. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 & 15 Residents were able to take part in some leisure activities, either in the home or the local community. The staff team supported residents with personal and family relationships as required. EVIDENCE: The residents’ plan reflects the range of activities undertaken which included a “lifestyle” programme which was undertaken at the local day centre. Opportunities included baking, beauty treatments, videos, DVD’s, listening to music, passive exercise and sensory stimulation. Other activities outside the home included shopping, visits to the cinema, meals out and visits to the local pub. Residents access the local community with their own adapted transport. Residents can see visitors in the combined lounge/dining area or in their own bedrooms. No separate space is available for this purpose. A recommendation in the last inspection report with regard to additional space for residents’

Relatives commented that the only private space that could be used during visits was the residents’ bedrooms. During a tour of the home some residents were seen spending time alone in their rooms whilst others were enjoying social contact with others in the lounge/dining area. See recommendation No. 2. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 20 Residents received support from the staff for personal care in their preferred way. Administration and control of medications were appropriate for the needs of the residents. EVIDENCE: Visits to health professionals were recorded in the care plans, including visits to GP’s, opticians, chiropodists and dentists. These were not up to date. The deputy manager said a carer always attended appointments with residents. In the kitchen a locked steel cupboard was available for the secure storage of medication. A monitored dosage system was used and all medication was stored appropriately. The medication administration sheets were signed by the staff and were up to date. Controlled drug storage was available but not required at the time of this inspection. Weekly checks on the medication were recorded on the medication administration record sheets. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 Clear policies and procedure were in place to ensure that residents’ views were listened to and acted upon. EVIDENCE: The home’s policy on complaints was seen and also the full complaints procedure produced by Community Integrated Care (CIC). The complaints procedure included timescales for response to the complainant and to whom you can complain to. Residents said that they would speak to the staff if they had a complaint. Staff confirmed that they were aware of the procedure and would pass concerns to the manager. Relatives confirmed that they were aware of the complaints procedure. The home or the Commission had not received any complaints since the previous inspection and a complaint book and relevant paperwork was available in the event of a complaint being received. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 & 30 The home provided a clean and comfortable environment for the residents to live in. EVIDENCE: The home was furnished in a domestic style with additional equipment such as bars in toilets and bathrooms, rails along corridors and moving and handling equipment provided to meet each person’s needs. The décor in bedrooms was to resident’s preference and the residents and staff confirmed this. The home was clean, tidy and free from unpleasant smells. The laundry room was clean and tidy and cleaning materials were stored in a locked cupboard. The deputy manager said that Control of Substances Hazardous to Health data sheets were available, and staff confirmed they were aware of the data sheets. From the previous recommendation the need for weeding the garden and cutting the lawns had now been completed and a new gardening contract was in place.

Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 & 35 The manager provides clear leadership. Records were well maintained. EVIDENCE: At the time of this inspection the agreed staffing levels were met. Three staff were on duty at the time of this inspection and the deputy said that a member of staff sleeps-in on the premises each night. The manager, deputy and housekeeper support the care staff. The staff team was well established. They had a range of experience and this was complimented by mandatory courses undertaken, such as moving and handling and food hygiene. Other relevant courses were also available, such as Adult protection and Cerebral Palsy. Three out of ten staff had completed NVQ level II and two staff were working towards this award. The deputy was undertaking NVQ level III and the manager was working towards the Registered Mangers Award. See requirement No. 3. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 39, 40, 41 & 42 Residents’ views were used in the planning for the home. Decisions are influenced by the information obtained from the completed satisfaction surveys and from conversations with residents. Arrangements are in place to minimise the risk so that the safety and welfare of the residents is promoted. EVIDENCE: The manager had given satisfaction surveys to residents’ and relatives. These details were currently being examined and the manager said that this information would be used to improve the service provided. This was a previous recommendation in the last inspection report and remains outstanding. This process should continue and the outcomes shared with residents’ and other interested parties. Bankfield Road Care Home DS0000005142.V263782.R01.S.doc Version 5.0 Page 18 A previous recommendation was made with regard to the annual development plan, which should be based on the annual review of residents’ needs. The Manager said this was still in the process of being reviewed. Records seen were kept in good order. These were in line with the Data Protection Act 1998. The deputy manager said that residents had access to information stored about them. Residents said they were aware of information kept about them. All the policies and procedures seen were appropriate to the service and up to date. Policies were produced by Community Integrated Care and were reviewed on an annual basis. Safe working practices included visits from the fire safety officer and the environmental health officer, which had been completed satisfactorily. The gas safety, electrical safety and insurance certificates were in place and up to date. Fire alarm tests were being undertaken on a weekly basis and records kept. Emergency lighting tests were also being carried out on a monthly basis with records kept. All staff had received fire awareness training in September and October 2005. See recommendation No. 3.
