What the care home does well

Bay Tree Lodge is a small family run home which provides a relaxed and intimate style of care in an entirely domestic setting; at the time of this inspection only four residents, with low to medium care needs, were accommodated. Residents enjoy a good quality of life and social care, in comfortable homely surroundings providing privacy, companionship and assistance when needed. Residents are satisfied with the home; comments received by the Commission included "an excellent and caring environment...always made to feel most welcome when we visit and are kept informed of (X`s) state of health".

What has improved since the last inspection?

In accordance with requirements contained in the report of the last inspection improvements have been made to arrangements for the recording of administered medication. Since the previous inspection the home has installed a stairlift, enabling service users unable to negotiate stairs to be accommodated at first floor level.

What the care home could do better:

This report contains no requirements or recommendations; from previous inspections and from this inspection it is known that the home routinely meets the National Minimum Standards and levels of resident satisfaction are high.

Either room 3 or 6 may be used as a double, provided that the registration figure is not exceeded. 25th July 2005 Date of last inspection Brief Description of the Service: Bay Tree Lodge is a detached property in a quiet residential area approximately one mile from Weymouth town centre. The home provides care and accommodation for a maximum of six older people with low care needs in four single bedrooms and one double bedroom, on ground or first floor levels. There is a stairlift, for the use of residents unable to negotiate stairs. Communal areas comprise a lounge/dining room and a smaller ’conservatory’. There is access to the attractive well-maintained garden at the rear of the house. The home is a well-established ‘family run’ care home, registered to Mr and Mrs Simpson; Mrs Simpson manages the home on a day-to-day basis. The home does not routinely provide wakeful night staff; Mr & Mrs Simpson live on the premises, are on-call throughout the night and respond to resident’s requests for assistance. This inspection was one of the two statutory inspections required in accordance with the Care Standards Act 2000. The previous inspection took place on 25 July 2005; since that inspection no complaints against the home have been received or investigated. The inspector arrived (unannounced) at 12.45pm; she spoke to all residents, and together with Mr & Mrs Simpson considered other evidence relating to the National Minimum Standards, as described in this report. Additional information used to inform the inspection process included 3 Comment Cards completed and returned to the Commission since the previous inspection; 2 cards were from health care professionals, 1 was from the relative of a resident accommodated at the home. The cards indicated high levels of satisfaction. In recognition of the circumstances and history of this small home, which is known to routinely meet almost all National Minimum Standards and to be in the continuous direct care of registered providers Mr & Mrs Simpson, this inspection was brief in duration and focussed on discussion with residents and assessment of only the most essential standards. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 The home does not provider Intermediate Care so Standard 6 does not apply. Prior to admission, the needs of each proposed resident are assessed to ensure the home will be properly able to meet them. EVIDENCE: The records of a recently admitted resident included details of pre-admission assessment carried out by the manager when the prospective resident visited Bay Tree Lodge. The assessment records identified the needs of the prospective resident and enabled the manager to determine that the home is able to properly meet them. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 (Standards 7, 8 & 10 were assessed and found met at the last inspection) Medicines prescribed by doctors are safely stored and carefully administered to residents by staff, unless the resident has chosen to store and administer their own medicines, in accordance with risk assessment. EVIDENCE: Residents wishing to do so can manage their own prescribed medicines, although most prefer this to be done by the staff. Medicine administration records (MARs) have been improved since the last inspection in accordance with requirements made in the report; they properly indicate that medicines are accurately administered as directed by the prescriber. Staff involved in handling medicines receive related training. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28 & 30 Evidence for Standard 29 was not assessed; since 2004 no new staff have been employed at Bay Tree Lodge. The home employs enough staff to meet the needs of residents and to ensure their safety and comfort and the good condition of the premises. Staff receive appropriate training and are competent to carry out their jobs. EVIDENCE: At the time of inspection there were sufficient staff on duty to properly meet the needs of residents; there are usually at least 2 staff on duty during each day. On occasion, this may reduce to one, particularly when the second staff member is accompanying a resident outside the home e.g. to visit friends or go shopping. The home does not routinely provide wakeful night staff; Mr & Mrs Simpson live on the premises, are on-call throughout the night and respond to resident’s requests for assistance. This circumstance is kept under continuous review by the home; Mr & Mrs Simpson stated that in the event of increased dependency wakeful night staff would be provided. At all times there are staff trained in First Aid and health-care in the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 & 35 (Standard 38 was assessed and found met at the last inspection) Staff have good leadership from Mr & Mrs Simpson. Residents are satisfied with the home and feel staff care for them well and put them at their ease. With the exception of the personal allowance of one resident, the home does not manage the finances of residents. EVIDENCE: The Registered Manager is Mrs Simpson, who together with her husband has owned the home since 1999. Mr Simpson has obtained the Registered Manager’s Award in care. With the exception of the personal allowance of one resident (for which all transactions are confirmed by receipt), the home does not manage the finances of residents.
