This inspection was carried out on 5th January 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home had a friendly and welcoming atmosphere, was clean and tidy and free from any unpleasant odours. The service users were jovial and all stated that they had a wonderful Christmas due to the efforts made by the staff. One service user stated, " The staff are excellent they really look after you, I`ve settled here it`s a fantastic place." Another service user stated " This is a home from home, the staff are very good, I would give this home top marks." All the comments received by residents were positive. The home operates comments and suggestion card system, the manager then reviews these and "good ideas" are implemented. Residents were encouraged to continue with their hobbies, some of the residents who were interested in gardening had planted winter flowers within the patio area.

What has improved since the last inspection?

A mobile phone has been made available for service users, which allows them to receive personal calls in the lounges or in their own bedrooms. The registered manager monitors the dependency levels of service users every two weeks to ensure that there are enough staff available to meet the needs of service users. New staff does not start work without the necessary pre employment checks being carried out.

What the care home could do better:

Storage within a narrow corridors leading to the day unit must cease. Heating to this corridor must be implemented. The storage of controlled medication must comply with current legislation.

It is run by North Yorkshire County Care Services, which is a North Yorkshire County Council business unit. Mrs Whitelock is the registered manager. The home is located close to the centre of Tad caster and service users can access the community amenities. The home was built approximately 30 years ago. It is a modern two storey building which is situated on a site shared with a day centre for up to 12 persons that is also managed by the homes manager. All bedrooms are ground floor and single occupancy. Bedrooms are arranged on four wings, some of which have a secure inner courtyards and a garden patio area for service users use. The unannounced inspection took place over 3.5 hours and was carried out by one inspector, following half a day’s preparation. The registered manager was not present at the time of the inspection, Barbara West, relief team manager assisted in the process. A tour of the premises was completed, which included communal areas, laundry, kitchen, offices and treatment room. A number of service users and staff were spoken with. The view of a visiting general practitioner was also sought. Some records were inspected, including staff rotas, staff files, menus and kitchen records. Residents personal allowance accounts were also checked and found to be correct. The inspection concluded with a feedback session, involving Barbara West relief team manager with Denise Rouse, Regulatory Inspector. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 9 10 Service users rights were upheld, however the method of storage for controlled medication was incorrect and may place staff at risk. EVIDENCE: Service users were seen to be dressed appropriately. A mobile phone was available for service users to use and receive private incoming calls. Telephone lines could be fitted within service users bedrooms at their own expense. Mail was given to service users unopened. Service users were seen socializing within several lounge areas and spending time within their own bedrooms. Staff addressed service users by their preferred name and knocked upon bedroom doors before entering. A visiting professional came to see a service user; the consultation took place privately, in the service users chosen environment. The registered manager must ensure that the home complies with current legislation pertaining to the storage of controlled medications. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 14 15 Service users control their own lives and receive a wholesome appealing well balanced diet in pleasant surroundings. EVIDENCE: There were a variety of lounge areas for service users, all having televisions and sterios. Service users were seen making choices about where they would like to go within the home once they were out of bed. Quiet areas were also available for service users who preferred to sit on their own and relax. The main lounge offered a bar facility. A very jovial atmosphere was evident in this lounge with service users sitting together and really enjoying their conversations with each other and staff. The kitchen was inspected; the meal being prepared looked appetizing .The vegetables were being steamed to ensure nutrition was not lost. A choice of meals were available. Menus were in place upon the tables within the dining areas. Service users could eat within their bedrooms if they wished. The chef prepared food for service users with diabetes. There were no service users requiring puree food. Meals were also provided for four service users within a day unit at the home. Food and fridge temperatures were recorded. The kitchen and preparation areas were clean and tidy, apart from the potato rumbler and surrounding floor, which required cleaning. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 19 25 Service users live in a home that was clean and well maintained, however one corridor was very cold, with stored items impeding access; this may place staff and service users at risk. EVIDENCE: The home was warm, clean and tidy in most areas. The communal lounges were spacious and well decorated. However one smaller lounge had a carpet which was thread bare by the fire exit .The relief team manager stated that this was going to be replaced as there was a fault with the carpet. The gardens were tidy and there was a small-enclosed patio area with planters filled with winter flowers. The service users who liked gardening had planted these. The corridor leading to the day unit was narrow and very cold; the temperature was 11.8 degrees centigrade. Staff commented that they had to wear cardigans or coats to take service users through this area. There were a number of items being stored in this corridor, which could impede the escape route for service users and staff. Three wheelchairs were also placed in the corridor from the service users occupying the day unit. The relief team manager and a carer immediately rectified the situation by removing the stored items from the corridor. Storage of items must not occur within this corridor. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 27 28 29 Service users needs are met by trained and competent staff who commence work after all necessary pre employment checks have been completed. EVIDENCE: The home manager monitors the dependency level of the service users every two weeks. This ensures that the needs of the service users can be met. Service users needs are met by a trained workforce; approximately 75 of the staff hold a National Qualification In Care at level 2 or 3. New staff have two written references on file. The Criminal Records Bureau and Protection of Vulnerable Adults checks had been completed. New employees did not commence work until this information had been received by the registered manager. The policy pertaining to the recruitment of staff was not current, it was last updated in 1997, and this requires reviewing and updating. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 31 35 The registered manager ensures that the home is managed well and service users are not placed at risk. EVIDENCE: Service users and staff were complimentary about the home manager, they stated, “ She was very approachable and would listen to their ideas comments or concerns.” A service user stated “ if I had a complaint I would speak to the manager and I know she would sort it out for me.” The Manager is currently undertaking her National Vocational Qualification in Management level 4 and the Registered Managers award. The relief team manager also confirmed that the home was run in a professional manner, by an experienced manager who was good at her job and approachable. Receipts for transactions and purchases were kept at the back of each individual’s statement. Money was stored in individually named plastic wallets. Items received by service users and their relatives were signed for as being received. The administrator was operating a good recording system of personal allowance transactions within the home.
