The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

What has improved since the last inspection?

Service users are now provided with terms and conditions as they move into Beechwood House. Radiator covers are provided throughout most of the building and remaining radiators in communal areas have been measured. Fresh fruit is available on request and a selection of chopped fruit are offered to service users during afternoon tea break Handrails have been provided to enable access in better weather to the garden for residents. Since the last inspection an activities coordinator has been appointed and the opportunity for outings and the range of activities in house has improved A new lift has been installed in the home

What the care home could do better:

There are insufficient bathrooms in use in the home to meet the needs of residents. The bathroom on the first floor currently being used for storage must be cleared and suitable facilities provided to enable service users to access bathing facilities in order to comply with National Minimum Standards. There have been some staff changes recently resulting in the loss of two senior members of staff and two care staff in addition to the recent resignation of the activities coordinator. Staff changes should be monitored to ensure the best interests of service users are maintained at all times and the management team must work together to promote staff morale . Comments from comment cards: "I would like more activities" "I would like to go out occasionally, perhaps a disabled bus to take us to the sea front for an ice cream, trips to the Red Lion for a meal occasionally would be nice" " I am concerned about members of staff leaving who have been at Beechwood for a lot of years. Staff morale seems very low." " Beechwood House is very dated. Bathrooms and toilets are very shabby. Wheelchair access is limited." "It would be nice to have a few more outings although I appreciate staffing difficulties with this."

This only applies for use by married or partnership couples sharing bedroom 10. Date of last inspection 19 April 2005 Brief Description of the Service: Beechwood House is a residential care home for older people situated in the village of Earsdon. There are two public houses in the village which is served by a local bus service. Accomodation is provided over two floors and access to the first floor is via a passenger lift. There are 27 bedrooms, none of which are en suite. Toilets are located throughout the building and commodes are provided in bedrooms overnight. There are five bathrooms three of which have assisted baths and one incorportes a roll in shower., One bathroom is used as a storage area and this must be cleared and assisted bathing facilities provided to improve choice for residents. There is a mobile hoist available in the home. Beechwood House has a large lounge and conservatory on the ground floor and two dining rooms. In addition there is a smoking area on the ground floor and a quiet lounge on the first floor. The home is pleasantly decorated and furnished throughout and there is a private garden to the rear. There are plans under consideration to extend the size of the home using existing buildings adjacent to Beechwood House in order to provide care for a further nine residents if planning is approved. This report is of an announced inspection, which took place on 4th October 2005, the inspection took place over 5 ½ hours and included a tour of the building. The inspector spoke with four visitors, ten residents and five staff during the inspection. Comment cards were returned by 15 residents and 17 relatives/carers. The inspection included an additional visit on the 13th October to discuss inspection findings and to investigate a complaint. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 & 3 Each service user has a written contract /statement of terms and conditions within the home. Needs are assessed and assurances given that needs will be met before service user’s move into Beechwood House. EVIDENCE: Discussion with the manager following the inspection confirmed that the owner has prepared written contracts and these are given to new service users as they move into Beechwood House. Care files examined confirm that assessments are done before service users move into Beechwood House. In some cases the assessment is undertaken by a care manager and supplemented by the manager’s assessment of need, in the case of privately funding individuals the assessment is undertaken by the manager Mrs Bell. Mrs Bell is an experienced practitioner and ensures that needs can be met in Beechwood House before an offer of a place is made. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7-10 Health, personal and social care needs are set out in an individual care plan for each service user Health care needs are met There is a medication policy and procedure in place and staff administering medication are provided with appropriate training. Service users are treated with respect and privacy is upheld. EVIDENCE: Four case files were examined during this inspection and the overall standard of care files was good. Care plans and daily recording sheets provide evidence that health and personal care needs are being addressed. Family members spoken to during this inspection expressed concern that their relative had been sent to hospital unaccompanied following an accident which took place over a weekend, subsequent discussion with the deputy and the manager indicates that an action plan is now in place to prevent this occurring again. Senior staff have agreed to participate in an on call rota for out of hours emergencies and this will be monitored to ensure that out of hours service provision is managed effectively. On the day of inspection the optician was in the home testing the Beechwood House B53-B03 S61572 Beechwood House V240088 041005 Stage 4.doc Version 1.40 Page 10 vision of a number of residents and records confirm that GP, District Nurse, Chiropodist and other health care professionals are involved in the primary care of residents in Beechwood House. An assessment has been requested and CPN involvement arranged for one resident whose behaviour has changed and this is recorded in the care files, an action plan is to be developed relating to this resident whose first language is not English in order that effective communication can continue to be facilitated between him and the staff and fellow residents. Relatives consulted expressed concern that hospital appointment letters had not been opened on behalf of one resident and this system has been amended to ensure that private post is forwarded to residents unopened however medical appointments will be actioned if residents and family members agree to staff opening specific items of mail. There is a medication policy and procedure in place at Beechwood House and checks undertaken of controlled medication during the inspection confirmed that records are accurately maintained and medicines are stored appropriately. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12-15 Service users have expressed concern that there is not a range of suitable activities available at Beechwood House. Contacts with the local community and with friends and families are encouraged. Service users are enabled to exercise choices within their lives There is a varied balanced diet provided at Beechwood House. EVIDENCE: An activities coordinator has been appointed at Beechwood House and the range of activities available has improved since her appointment. Trips to Boundary Mills and walks around Earsdon village have been introduced and a theatre trip is planned. Arts and craft activities have been introduced and armchair exercise and games provided in house. Unfortunately the activities coordinator has very recently resigned from her post in order to take up a post elsewhere and the post of activities coordinator will be advertised and another coordinator appointed. In the meantime service users must be consulted as to the range of activities they wish to engage in. Visitors were observed during the day and conversations with service users and their visitors confirmed that visitors are welcomed to Beechwood House. Beechwood House B53-B03 S61572 Beechwood House V240088 041005 Stage 4.doc Version 1.40 Page 12 Residents in Beechwood House have a pet cat living in the home, who provides companionship and affection to a number of residents. Meals are provided on the premises and are cooked fresh daily, on the day of inspection a choice of sausage casserole or steak pie was provided with potatoes, vegetables and a dessert of trifle or banana custard. The food was hot and portions of an ample size and served according to individual preference. Breakfast is offered at times to suit individual tastes and a choice of cooked breakfast or cereal and toast is on offer. Choices are offered at tea time and supper time and tea and coffee is offered during the day and evening. Fresh fruit is available on request and in the afternoon portions of fruit and cream are offered Kitchen staff are experienced and have received appropriate training in nutrition and food hygiene. Special dietary requirements may be catered for including diabetic diets and low fat options for people with high cholesterol levels. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 &18 There is a complaints policy at Beechwood House; which is to be reviewed and all comments and complaints recorded. Service users are protected from abuse. EVIDENCE: There is a complaints policy at Beechwood House and since the last inspection no complaints have been recorded; however following the inspection an anonymous complaint was received which indicated that a verbal complaint had been made but not recorded, this was discussed with the manager. Visitors spoken to on the day of inspection indicated that they had spoken to the deputy manager to complain about the treatment of their relative following a fall however this had not been recorded in the complaints book. The manager agreed to review the complaints procedure and gave assurances that all complaints whether verbal or written will be recorded and responded to within timescales indicated. Discussion also took place of the importance of recording positive comments from residents and visitors and of giving positive feedback to staff of comments made. Residents spoken to stated that they would speak to the manager if they had a complaint. The deputy manager and manager both demonstrated an understanding of the protection of vulnerable adults; staff spoken to during the inspection were aware of the rights of individuals and the vulnerability of the people in their care. POVA training has been provided to management and further POVA training is planned for staff members. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 ,21 & 26 Service users live in a safe well maintained environment The home provides sufficient lavatories but has less bathing facilities than recommended The home is clean pleasant and hygienic EVIDENCE: Radiator covers have now been fitted in the majority of the home and remaining radiators have been measured to comply with this requirement. The door to the conservatory has been replaced Handrails have been fitted to the garden to enable residents to go outside. The lift has been replaced and is now in full working order. The home is clean and décor throughout the building is pleasant and the home is odour free. Two bathrooms provide assisted bathing but on the first floor a bathroom is used as a storage area. In order to provide greater choice for service users this bathroom should be cleared and suitable assisted bathing facilities provided. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29,30. Service users needs are currently met by numbers and skill mix of staff however staff losses must be monitored Service users are protected by the homes recruitment policy and practices Staff are trained and competent to do their jobs EVIDENCE: The manager and deputy manager have a familial connection and the newly appointed senior care officer is the manager’s daughter and the deputy manager’s partner. The manager is aware of the implications for the management of the home in that family connections may potentially compromise practice and has an action plan in place whereby in the event of any disciplinary issues arising within the home an external manager will be brought in to manage the conflict and to investigate accordingly. The management of the home should be monitored to ensure that relationships are maintained in the best interests of service users. The management team have developed plans to introduce a system of a duty senior on call rota to ensure that events which arise out of hours are effectively managed by the senior team. Four staff members have been recruited from Beechwood House to a new care home, which is due to open nearby and this will have implications for staffing levels and qualified staff numbers at Beechwood House. The manager has recruited staff to fill the vacancies using the local press and job centre and is waiting for references and Criminal Record Bureau checks to Beechwood House B53-B03 S61572 Beechwood House V240088 041005 Stage 4.doc Version 1.40 Page 17 be returned for external recruits. One senior care post is to be filled by an existing member of staff. There is a commitment to training and development within Beechwood House. The majority of care staff are qualified to NVQ level 2 or above and there are a number of staff who are pursuing further NVQ level awards. The deputy is engaged in NVQ level 4 and is keen to access any further training, which may be available. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,35 & 38 The home is run in the best interests of service users Service users financial interests are safeguarded The health, safety & welfare of service users and staff are promoted and protected. EVIDENCE: The manager is an experienced and competent practitioner who is qualified to NVQ level 4 and has achieved the Registered Managers Award. There is a commitment in Beechwood House to training and development for staff and an awareness that recent staff changes will inevitably have an impact on service users and the staff team. The manager is committed to developing new staff to meet the needs of service users. Policies relating to the financial interests of service users are in place and records relating to individuals financial transactions maintained. Portable appliance testing is overdue however the owner has arranged for an electrician to undertake this on a contractual basis. Fire records confirm that equipment checks are up to date and testing is undertaken on a regular basis. The lift has been replaced and maintenance contract provided.
