Conditions of registration: One named service user with learning disabilities under the age of 65. Date of last inspection 21st February 2005 Brief Description of the Service: Braunton is a family run home providing accommodation and support for nine older people in domestic scale accommodation. Mr & Mrs Lambert, the owners, continues to be directly involved in the management of the home. Their daughter Alexandra Vickery is the registered manager and another daughter provides administrative support for the home. The management team, are supported by a small staff team. Residents are encouraged to maintain links with family and the wider community. The emphasis is on providing care within a ‘homely’ environment. This is an overview of what the inspector found during the inspection. This inspection was conducted over approximately seven hours following a satisfactory inspection in February 2005. During this time the Inspector spoke individually with the majority of the services, with the owner Mrs Lambert, registered manager Mrs Vickery two staff members, and reviewed premises and a small sample of care plans and other records. This inspection confirmed previous judgements – this home is a well run service, providing a good standard of care to older people. There are good systems in place underpinning administration and overall a satisfactory standard of record keeping. Training for staff is good. Residents were very positive about the service they receive. What the service does well: What has improved since the last inspection?
Since the last inspection, the management team have ensured that all the POVA checks required in relation to staff recruitment are carried out. All residents now have individual files to promote confidentiality. Improved facilities have been provided for the storage of medication.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,6 These standards are met. The evidence demonstrates that appropriate assessments are carried out before any offer of a place is made. EVIDENCE: Two new residents have been admitted since the last inspection. Varying levels of assessments involving multi-disciplinary professionals are undertaken prior to admission. This combined with the homes pre-admission assessment provides a baseline for the ongoing assessment during the trial period following admission. There is no intermediate care offered at this home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10 Good systems are in place to ensure that these standards are met. EVIDENCE: Assessment takes place prior to admission. The management team visit prospective residents in their own homes or hospital prior to placement. All new admissions are on a trial basis, which allows for further in-house assessment until the placement is confirmed. The home works closely with locally based multi-disciplinary professionals to ensure that health care needs are met, and where necessary involves specialist help. Residents are supported to maintain control of their medication if possible. The home is aware that the CSCI pharmacist and locally based pharmacists work together to ensure that systems operated in care homes are appropriate and safe. The local pharmacist last visited in June 2005 and left a positive report, which indicated that the recommendation s made at the previous visit has been implemented. Health and personal care needs are well organised with regard to the dignity and privacy of individual residents. Service users said that they were treated with kindness and respect. This is a small home with one main living/dining room area where the majority of people tend to congregate during the day. Some people entertain visitors in their own rooms; alternatively the conservatory area is available. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 These standards are met. EVIDENCE: Routines are flexible and the management team ensure that individual choices and preferences are taken into account. Residents also confirmed that this is the case. Visitors are welcomed and the home has well-established links with the local community. The management team are clearly aware of the individual needs and preferences of the people living here. The majority of people appear to prefer spending most of their time in the communal parts of the home however, the staff team are aware of the problems of potential isolation for those who choose to stay in their rooms for long periods. The meal provided during the inspection appeared appetising and a pleasant smell of cooking was evident during the morning. The management team ensure that staff carefully monitor likes and dislikes plus specific dietary needs. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16.18 These standards are met. Conversations with service users confirmed that the majority of people were confidant about who they could speak to if they had any concerns. The home has an appropriate complaints procedure in place and complaints are fully investigated. EVIDENCE: Feedback from residents indicate that staff are approachable, including Mrs Vickery and Mrs Lambert. Residents are mostly able to resolve things without recourse to formal complaint. The management in the home encourage residents to speak their mind and express their wishes and preferences. This would not detract from complaining formally should issues requiring this arise. There was evidence to show that correct procedures have been followed with the appointment of staff. JUDGEMENT – we looked at outcomes for standard(s) 19, 26 These standards are met. EVIDENCE: Braunton was originally the family home of Mr and Mrs Lambert and their family before it was converted into a residential home. Braunton is domestic in style and scale, and has a homely appearance. The owner ensures that the home is well maintained and meets the needs of the people living here. The home is not suitable for those who rely on wheelchairs for their mobility and the management team take this into account when new ‘long term’ admissions are considered. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29,30 There is good evidence to support these standards. EVIDENCE: The staff team are engaged in the full range of activities here, from direct care task to cooking and laundry maintenance. There are sufficient numbers of staff and at the last inspection; seven members of the team had NVQ II (or above) qualifications. The number of staff with this qualification has reduced, due to some changes to the staff group. The management team ensure that admissions are carefully selected in order to maintain the level and quality of support provided here. Staff records were sampled and were in good order. The management team are keen to ensure that staff are appropriately trained. Mrs Lambert continues to provide support as an NVQ assessor for all staff. There was evidence that staff had completed a number of training courses over the past year, these included specialist training on working with people with dementia, and focusing on meeting the needs of people as individuals (Person Centred Planning). Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,35,38 These standards are met. The views of service users and their families are sought through the use of questionnaires. All service users are supported by their families regarding the management of their finances Appropriate safeguards are in place to ensure the protection of service users. The management team ensure that the health and safety of those who live and work in the home is promoted. EVIDENCE: The management team seek the views of service users and their families, through the use of questionnaire. Since the last inspection, practice within the home has changed to ensure that where appropriate, questionnaire are given primarily to service users with the involvement of relatives as necessary.

The home does not act as appointee for any service user. It was evident that staff are aware of the homes policy in relation to involvement in individual finances and of not accepting gifts from residents. The management team confirmed that all the appropriate safeguards are in place to ensure the protection of service users.
