What the care home does well

The staff are enthusiastic and committed to ensuring the best care is provided for the patients in their care. The environment is comfortable, homely and clean. The extensive gardens are well maintained and easily accessible to patients. The plans of care include sufficient detail and are organised in such a way that staff can deliver the care required by each individual client. The patients spoken to had high praise for the home and the staff. The staff observed during the inspection were polite and considerate to patients.

What has improved since the last inspection?

Risk assessments for radiators not covered or guaranteed low surface temperature are completed and reassessed when a new patient moves into a room. A quality assurance system has been introduced.

The house has been modernised to provide a care home for a maximum of 60 persons over the age of 65 years. The home is registered to provide care for persons requiring nursing and/or personal care with physical frailty/illness or disability.The accommodation comprises of 38 single bedrooms and 11 double bedrooms, all bedrooms have en-suite WC and wash hand basin. The communal rooms provide one large lounge with sun lounge attached, one smaller lounge on the 1st floor, one dining room and a chapel. Cann house has 9 acres of attractive grounds with lawns, woodlands and a walled garden.The home is owned and administered by Premiere Health limited. The announced inspection took place over 6 1/2 hours between 10am and 4.30pm and was conducted with the matron, deputy matron and one of the providers. The inspector spoke to 8 patients and observed several others taking part in a variety of activities in a group and individually. A number of completed comment cards were received from relatives /representatives prior to the inspection. One of the purposes of this inspection was to follow up recommendations made following the last inspection. A tour of the premises took place and a variety of records were inspected. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3 &5 Comprehensive pre admission assessments are made and therefore people are admitted to the home appropriately. Clear information and opportunities to visit the home are given to people to help them to decide if they want to live at Cann House. EVIDENCE: The matron said that prospective patients and/or their representatives are able to visit the home when they like to look around. Individual records examined had evidence of a pre admission assessment being carried out. The ‘resident enquiry’ form has a space ‘date visit arranged’ to be completed. The Statement of Purpose examined was informative and easy to read with nice coloured pictures of aspects of the home. The one kept in the entrance foyer was up to date. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 9 & 10 The medication at the home is generally well managed, promoting good health. Interactions between staff and clients promotes and protects patients privacy and dignity. . EVIDENCE: The deputy matron demonstrated the homes medicines management procedures, they are in line with laid down legislation. The drug fridge temperatures should be checked and recorded daily. Data sheets for all medications should be available and dated when received. Service User comment cards completed (5) and patients spoken to indicated that patients feel well cared for and safe and have their privacy respected. Plans of care identify the preferred name a patient likes to be known as. Staff were observed knocking on patients doors before entering the room. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13 & 15 The staff have a good understanding of the patients cultural, social and recreational needs and the importance of maintaining contact with family and friends and engage the patients in a variety of ways. The meals in the home are good offering both choice and variety and cater for special dietary needs. EVIDENCE: Care plans examined contain information about interests and hobbies. An organist/singer was entertaining several patients with ‘old time’ music during the inspection. Completed Service User comment cards and patients spoken to had high praise for the home and one described it as ‘like a lovely dream’. A tour of the home saw patients engaged in a variety of activities independently and with the support of the staff. Menus provide prior to the inspection show that a balanced diet with plenty of variety is offered, with alternatives to the menu always available. Two of the 5 completed Service User comment cards said they like ‘some’ of the food the rest said they like it. One of the completed relatives/visitors comment cards said that the meals are sometimes not hot enough. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. JUDGEMENT – we looked at outcomes for standard(s) 16 The home has a satisfactory complaints system. EVIDENCE: The complaints log examined had details of complaints recieved and the actions taken to resolve the matter. Two minor complaints have been made since the last inspection and were satisfactorily resolved. The complaints procedure is displayed within the home and is in the Service Users guide a copy of which is given to everybody on admission. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,23,24,25 and 26. The standard of the environment within the home and the grounds is excellent providing clients with attractive and homely place to live. The overall quality of the furniture and fittings is good providing a safe environment. The home is clean and hygienic. EVIDENCE: A tour of the home confirmed that all of the communal facilities (ground floor lounge with sun room, first floor lounge, dining room and chapel) and the extensive gardens are accessible to clients. The fresh decoration and subtle colours combine to make the rooms light and airy. Patients rooms seen were comfortable, clean and contained many of their own possessions. Entries are dated and signed. Domestic staff were seen cleaning the home during the inspection and all areas were clean and tidy. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 and 30. Clients needs are met by a satisfactory mix of nurses, carers and ancillary staff who are trained and competent to do their job. The homes recruitment policy and practice protect the clients EVIDENCE: The duty rota provided with the pre inspection questionnaire details the number of staff on duty and during the inspection people looked well cared for. The pre inspection questionnaire indicates that staff turnover is low. Agency staff are used on occasions. Three of the four completed relatives/visitors comment cards returned prior to the inspection said that they feel that there are ‘sufficient staff on duty at all times’. The matron explained the recruitment procedures used in the home and 2 randomly selected personnel records examined confirmed that 2 written references and a Criminal Records Bureau check are received prior to employment. The deputy matron takes responsibility for training and records examined were up to date. Certificates for training undertaken were seen in the staff files. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 and 38. The systems in place in the home promote and protect the health and safety and welfare of the clients and staff. EVIDENCE: Staff meetings are held and minutes taken, the inspector viewed minutes from recent meetings held. The clients, relatives /friends and staff are asked about aspects of the homes performance. Responses received from a recent questionnaire, given to patients and relatives/representatives, were examined and the action proposed by the matron/provider to resolve any issues that arose was detailed on the response. A variety of up to date maintenance certificates were examined that included the passenger lift and hoists. The fire- log book was examined and was up to date.
