This inspection was carried out on 20th September 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home benefits from a well-established staff team some who have worked at the home for some time. Service users spoken with felt that the staff have a good relationship with them and work hard to improve their quality of life. The home provides in house activities that are varied.

What has improved since the last inspection?

The home has fitted pre-set values to all outlets for the safety of the service users in the home.

What the care home could do better:

The Registered person must make arrangements for the safe storage and safe keeping of medication stored into the home. Medicines in the custody of the home are handled according to the requirements of the Medicines Act 1968, guidelines from the Pharmaceutical Society of Great Britain and the requirement of the Misuse of Drugs Act 1971. This will ensure service users are not placed at risk.

The home is registered to provide residential accommodation and personal care for a maximum of 26 people over the age of 65 years who may also have dementia or a physical disability. Accommodation is provided over three floors, and offers 22 single bedrooms, 7 of which have en suite toilet facilities, and two double rooms, one of which has an en suite toilet facility. Stair lifts provide access to all floors. On the ground floor there is a large lounge that has patio doors opening on to the garden, and a large dining room. There is a call bell system in operation in the home. The home has a chair lift to all floors. Various activities and trips are regularly offered at the home. The garden is attractive and accessible to the service users. The visit to this care home lasted over 3 1/2 hours. A full tour of the premises took place and various records were inspected. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 The information provided in the homes Statement of Purpose and Service Users Guide will assist the service users and relatives to make an informed choice of a care home. EVIDENCE: One service user spoken with during this inspection discussed his contract in detail. This service user informed the inspector that he had a copy of his contract and that it also had the terms and conditions of occupancy on this contract. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 8/9/10/11 The home continues to provide excellent personal support for service users in the home. EVIDENCE: The file held from the District Nursing service was seen as evidence for one service user. This service user showed the inspector the file that contained information on the treatment that this service user was receiving and also information for the care staff to follow. One service user informed the inspector that he self medicates and case tracking provided evidence on his care plan that a risk assessment had been completed by the home as recommended in the last inspection report. The Controlled drug box was checked during this inspection and found to be left unlocked as was the drug cupboard. All service users spoken with confirmed that they are treated with respect and dignity. One service user confirmed that they had discussed with staff that they could remain in the home until their death. The home would provided additional services when needed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13/14/15 The service users in Chatsworth House can be confident that this home will enable them to make decisions about their own lives. EVIDENCE: Many service users spoken with during this inspection confirmed, as did evidence from fresh flowers and greeting cards in individual bedrooms, that many service users receive visits from family and friends on a regular bases. 2 service users confirmed that they often go out on their own choosing to do so when they wish. All service users spoken with confirmed that the food was excellent, plentiful and that the home offered a varied and wide choice. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16/17 Service users can be confident that their complaints or concerns will be listened to and acted upon. EVIDENCE: Several of the service users spoken with felt that if they had any concerns or complaints that they would be able to approach staff and managers. Also they were confident that the concerns and complaints raised would be listened to and acted upon. One service user confirmed he had seen a solicitor to manage his finances and personal affairs and visits this solicitor regularly. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22/23/24/25 Chatsworth House continues to maintain a suitable environment for its stated purpose. EVIDENCE: Several of the homes service users confirmed that they have the appropriate specialist equipment they may require to maintain their independence. The facilities seen by the Inspector, and confirmation by service users, leads to the conclusion that their rooms suit their individual needs. All bedrooms are safe, comfortable and had many of the service users personal possessions in. The homes kitchen door was found wedged open during this inspection, the manager stated that they are planning to replace the broken hold open door devices. A tour of the premises confirmed that the home was found to be clean and hygienic during this unannounced inspection. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 Staff training is supported and promoted enabling service users to receive the best possible service. EVIDENCE: The inspector spoke with 12 of the service users in the home during this inspection and all stated that the staff in Chatsworth House are skilled in their jobs. Evidence was seen on the staff notice board of courses book for the coming months including Manual Handling and Food Hygiene. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32/33/35 The Management team within this home is very good and ensures that records are effectively maintained. The staff team are well trained and above all service users are happy and their needs are met. EVIDENCE: All service users confirmed that the homes management team are excellent, approachable and benefits from the leadership of the manager. Many service users confirmed that they manage their own finances either with family support or via a solicitor. The money the home hold for some of the service users was checked during this inspection and was found to be correct and well recorded. The manager confirmed that all water outlets have now been fitted with safety values as required in the last inspection report.
