Chatsworth is a large detached property situated in the Mannamead area of Plymouth. The home is registered to provide residential accommodation and personal care for a maximum of 26 people over the age of 65 years who may also have dementia or a physical disability. Accommodation is provided over three floors, and offers 22 single bedrooms, 7 of which have en suite toilet facilities, and two double rooms, one of which has an en suite toilet facility. Stair lifts provide access to all floors. On the ground floor there is a large lounge that has patio doors opening on to the garden, and a large dining room. What the care home does well Surveys received from some of the people living in the home recorded, "Always staff on hand if needed" with another saying, "The food is 1st Class". One staff member wrote in their survey returned to the Commission, "Provides high levels of care" and another stating, "Carers are expected to lift residents due to lack of equipment". However the manager confirmed that the home has a hoist and would discuss this issue at the planned staff meeting due next week to ensure all staff knows how to use this equipment. The home held comprehensive and clear records for staff members employed by the home. This included obtaining all relevant information to ensure people living in the home are protected as much as possible. The medication systems are managed well and the home held records to show that ordering, storage and disposing of all medication were well documented with a clear audit trail showing administration of medication and signed by staff members were clearly documented. Care Homes for Older People Page 3 of 8 What we found: We visited Chatsworth House on the 20th May 2010 at 10:15am. Mrs. Sally Colwill, the manager of the home, was present throughout the random inspection. Neither the Registered Manager nor the owners were available for this inspection. The purpose of this inspection was to review the records and hold discussion with the last two people admitted to the home and to include a medication review of these two admissions. Also to review the records and hold discussion with the last two staff members to be appointment, review any controlled drugs held, a brief tour of the premises and to review any notification and accident recorded. Two files were examined for people living in the home. Each file held comprehensive details on each person ensuring all staff were fully aware of the needs for each person. The files examined held records for each person on any medication prescribed, mental health issues and how staff will manage personal care and health care for each person. Several people including two people living in the home and one relative were spoken too during this visit. All spoke very highly of the staff in the home and also stated that the food offered was very good. With one commenting, A1 Home! and another saying, Staff look after me well. The medication was examined for each of the last two admissions and the records held showed the home had a clear audit trail on ordering, storage and administration for each medication. Senior staff members have overall responsibility for the medication held in the home. The owner confirmed that all staff completes regular training on medication and all staff shadows other staff member until they are competent to dispense medication. The home does not currently have any controlled drugs. Several people are prescribed PRN medication and the home has clear guidance on the dispensing of this medication. We recommended to the manager that all handwritten amendments to the medication records sheets be sign and confirmed by two staff members. We found that Chatsworth House held clear records for the staff files of the last two staff employed by the home. This included a completed enhanced CRB (Criminal Record Bureau) check, a copy of the persons passport and driving license and two proofs of address including a bank statement and phone bill. Evidence on the staff members files examined showed that they had completed, or in the process of completing a staff induction programme. Other training including Liverpool Care Pathways and End of Life Care training and booked training for the future included 1st Aid, a Health and Safety and Infection Control course. Both newly employed staff members were spoken with and both agreed that the management of the home were approachable and that they had been welcomed into the staff team. One staff member confirmed they had completed and Induction programme. A brief tour of the premises and discussion with some people living in the home, a relative and staff confirmed that the home is regularly updated and maintained to a high standard. The manager confirmed that many parts of the home are upgraded when needed by the owners. Care Homes for Older People Page 4 of 8 The accident book was examined and showed that all accidents recorded were also followed up in individuals daily records with medical intervention if required either by the GP or local hospital.
