This inspection was carried out on 22nd February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

One of the strengths of Cherry Garth is the admissions process. The management team are very clear about the service they can provide. This means the service users admitted to the home know their needs will be met. The staff are fully involved in the care planning and reviewing process and the service users spoken with said that the staff were `wonderful`. The service users are encouraged to keep their own routines and when necessary they are helped to remain part of the local community.

What has improved since the last inspection?

The registered manager has ensured that all the staff that handle the medication have completed the safe handling of Medication course. The staff have also updated their knowledge of the medication policy. The environmental issues identified at the last inspection are also in the process of being dealt with.

What the care home could do better:

Overall the service provided at Cherry Garth is of a good standard. However, the registered manager should now look to the detail of the service provided to ensure the standards remain as they are now. To this end one minor improvement was identified. The Promotion of Independence Plans developed by the key workers are not currently signed by the service users. As these concern the service users then they should be signed by the service user concerned after discussing them with the key worker.

The category for Physical Disability (PD) only applies to the named service users. 19th May 2005 Date of last inspection Brief Description of the Service: Cherry Garth is a purpose built property standing in its own well-maintained grounds. It is situated a few minutes walk from the centre of Thirsk. The building occupies two floors and provides for a maximum of 48 service users. There are 40 single rooms and 4 shared bedrooms, none of which has an ensuite facility. There are communal bathrooms and toilets provided on each floor. There is a passenger lift. Communal rooms are located throughout the premises. This inspection including preparation time took place over 7 hours. A tour of the building was conducted. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 The service users can be assured that their care needs will be met EVIDENCE: The service user files seen contained copies of an assessment by a care manager. The registered manager confirmed that they do visit all prospective service uses before their admission. A copy of the assessment is kept in the service users file. Service users spoken with confirmed they had seen someone from the home prior to their admission. Intermediate care is not provided. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 8 The health and personal care needs of the service users are met. EVIDENCE: The service user files seen contained a Promotion of Independence Plan. This document was pertinent to the individual concerned and those service users spoken with were aware of their plans. However, the service user had not signed the plans. Evidence was seen to show that the plans are reviewed at monthly intervals, by the key worker and with the service user. The service users files contained information about their health needs and details of GP visits and district nurses are recorded. The service users spoken with said that they could see their GP on request and that any visits made either to the home or the surgery happened in private. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): The service users can exercise choice and control over their lives. EVIDENCE: The service user files contained information about their personal preferences as to rising and retiring times, where they like to take their meals and what they like to be called. The service users spoken with said that whilst they were considerate of the routine within the home they could follow their own routines. The service users were seen to be going out in to the community freely. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Service users were confident that any complaints or concerns they had could be raised and acted upon. EVIDENCE: North Yorkshire Social Services has a detailed complaints policy. This policy is provided to all the service users and is displayed around the home. There have been no complaints made to the CSCI in the last 12 months. Several complaints had been made directly to the home and evidence was available to show that these had been dealt with appropriately. Service users spoken with all said that if they had any problems there was someone within the home they would go to. The staff had also received several commendations for the care and support they had given. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26 The home was clean and suitable laundry facilities are provided. EVIDENCE: All communal areas of the home seen were clean and odour free only one bedroom was found to have an unpleasant odour and this was pointed out to the registered manager during the inspection. The laundry is separate from the communal areas of the building and has impermeable flooring. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): There is sufficient staff to meet the needs of the service users. EVIDENCE: The staffing levels at Cherry Garth have not altered since they were registered with the Commission. The registered manager is more prudent about the level of care they can provide when considering new admissions to the home. This means that the staff have the time to ensure they can meet the social and personal care needs of the service users. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, and 35. The service users live in a well managed home where the service is regularly checked to ensure the standards don’t drop. EVIDENCE: The registered manager has achieved the registered managers award and also has a National Vocational qualification level 4 in management. She is responsible only for Cherry Garth and receives monthly supervision from her line manager. Service users spoken with said that she was approachable and if they had any problems they would not hesitate in going to see her. Staff also said that they could speak to her at any time and when on duty she was always available to them. North Yorkshire Social Services has introduced a quality assurance programme. This information is then used to develop an action plan to improve services.
