What the care home does well

It encourages the residents to make decisions and supports them to take control of their lives. It provides a home for five people to develop skills and live full productive lives. Health and safety to ensure the home is a safe place to live. It helps to create a pleasant atmosphere making it a nice place to live. It has a well established staff team who provide a consistent level of care to the residents.

What has improved since the last inspection?

The record keeping in the home is being improved. The manager said that the home has developed good relationships with their neighbours making it a friendly place to live.

What the care home could do better:

Ensure that all recordings related to the residents meet the guidelines of confidentiality and data protection. Residents care plans should not be written in the first person unless the residents are involved in writing them. All residents` care plans must be kept up to date.

Date of last inspection Brief Description of the Service: 47 Doublegate Green is registered to provide residential, personal and social care for 5 people under 65 years of age who have learning disabilities and may also have physical disabilities. The home is a purpose built bungalow with five single bedrooms. It is situated close to Ripon City centre and provides good access to the citys services and amenities. The home is part of the United Response organisation. The responsible individual is Ms Naomi Atkinson. At present the home does not have a registered manager but the acting manager Mrs Stephanie Thornton will be applying for registration. This was an unannounced inspection carried out as part of the inspection process on 26th July 2005. The acting manager was not available for the first hour of the inspection but there were three members of staff and 4 residents in the home. All of them were spoken to and care practices were observed. It was very clear that the staff were aware of their duties and that the care of the residents was their priority. The inspection also involved a check on the requirements from the previous inspection, a tour of the premises, and a check on some of the records. The inspection took 8.5 hours which includes preparation. The home was clean, well decorated and furnished, and there was a pleasant atmosphere. The residents were supported to take control of their lives and the staff were encouraging and helpful. The four residents all communicate in special ways but they were able to make their wishes known to the staff. They were clearly happy living in the home. The fifth resident had gone to his day service but it was reported that he is always telling people that ‘he loves it here’. The inspection showed that the home was well organised and that the staff were aware of their duties, and the residents were well cared for and had full well structured lives. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2,4,5. Residents know that if they choose to live in the home that they will be well cared for. EVIDENCE: Four of the residents have lived in the home for several years but one was admitted last year and records showed that a comprehensive admission procedure was undertaken. This included assessments involving the resident, the resident’s family, other professionals, introductory visits, and a trial stay in the home. All residents have an individual contract and reasonable steps have been taken to ensure that it is explained to them. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,8,9,10. The residents’ health and personal care needs are met and they are encouraged and supported to make choices about their daily lives. This helps them to have control over their lives in the home. EVIDENCE: Care plans identify the residents’ personal and social care needs. Some plans were not up to date and some were being written in the first person and not being signed by the writer who clearly was not the resident. This gave a false impression of the facts and it was recommended that the process cease or consultation takes place with carers and other professionals. Risk assessments are included in the care plans so that both staff and residents are aware of the support required. The residents are involved in all aspects of running the home and it was clear that they valued and enjoyed this. Residents hold regular meetings to decide on menus, leisure and social activities. The daily record maintained by the staff in the home included relevant information of the residents’ day and night activities. Advice was given as to how this could be done. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,1516,17. Residents eat well and enjoy a wide range of activities both inside and out of the home and are part of the local community. EVIDENCE: Each resident had an individual programme aimed at developing their skills and staff are available to support them. The residents have very active lifestyles supported by a very committed staff team. The staff are constantly looking for new activities which will interest the residents and provide further stimulation and development. All residents use the local facilities in Ripon and more recently have been building good relations with their neighbours and invite them to various activities in the home and join in others outside. Residents are supported to maintain links with family and friends. Food is an important part of the residents lives and they help to choose the menus weekly and each resident will take part in helping with the shopping. Although the residents have few verbal skills, communication is not a great problem and lunch was a very noisy affair with a lot of interaction and enjoyment. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19,20. The residents’ physical and emotional health needs are met. EVIDENCE: Each resident is registered with a GP and specialist healthcare is organised through the practice. The home was able to demonstrate good communication and joint working with the health care professionals to benefit the residents. None of the residents self medicate. A monitored dosage system is in operation and the staff member responsible for medication was able to show the effectiveness of the system. All staff have had medication training. Staff induction and on going training includes care practice that ensures and promotes privacy and dignity. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22,23. Residents are protected from abuse, neglect, and self harm. EVIDENCE: There is a complaints procedure in various formats but it is difficult to say that residents are aware of its implications. Staff said that they need to be vigilant and observe any changes in behaviour and monitor to see if it is because residents are concerned or have a problem. The protection of vulnerable adults procedure is available in the home and training of staff has been recorded. The manager was aware of the procedure but it was recommended that it should be reinforced to staff at regular intervals to ensure they are aware of its conditions. All the residents go out regularly and meet with many people who could be advocates for them if they were not being cared for properly. The home has a robust recruitment procedure to help to protect residents. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,28,29,30. The residents live in a safe, comfortable and clean environment which meets their needs. EVIDENCE: The home was clean and well maintained and provided comfortable living accommodation for the residents. All residents had personalised their rooms to make them homely. The space provided was flexible enough to cater for a range of activities. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,35. The staff are well trained and well organised which ensures that residents feel supported and staff are aware of their duties. EVIDENCE: The staff rota is organised around the needs of the residents. Staff felt that there were enough staff on each duty to meet these needs. The three senior staff all have individual responsibilities and there were clear lines of accountability in the home which meant that everyone was clear about their role. The home has a very settled staff team and some of the staff have worked with the residents for over 15years. The staff on the day of the inspection were very enthusiastic and committed to meeting the needs of the residents. Staff training is on going and relates to the specific needs of the residents which makes staff more confident in their role. A robust recruitment procedure ensures that suitable people are employed in the home.
