This inspection was carried out on 12th July 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The staff offer excellent support to the ladies and gentlemen who live at Ferncliffe to enable them to pursue the work, leisure and educational activities of their choice. The flexibility needed to react to changing needs was seen to be in place and each person`s current needs was reflected in their up to date plan of care. The importance of individual choice was respected and communication between service users and staff was seen to be friendly and respectful.

What has improved since the last inspection?

The quality assurance questionnaire for relatives and other visitors has now been developed and distributed. The organisation has now reviewed the policies and procedures and the monthly visits by the registered provider, or their representative, are being undertaken regularly.

What the care home could do better:

It would be good if the staff without a qualification in care could undertake and achieve the NVQ in care qualification.

Date of last inspection 4 November 2004 Brief Description of the Service: Ferncliffe is a residential home providing care and accommodation for up to eight men and women. The home is owned and managed by Mayfield Trust, formerly Calderdale Mencap. There is good access to all local amenities and bus routes. The service users are encouraged and supported to take part in work, social, educational and recreational activities according to their own wishes. The atmosphere is relaxed and comfortable and interaction between service users and staff creates a positive, family feel to the home. This was a very positive inspection with no statutory requirements or good practice recommendations arising. Everyone who commented confirmed that they were happy and well cared for at Ferncliffe. The meals and social activities within the home are all enjoyed. Some service users had recently been on holiday to Cyprus, accompanied by staff. They showed me the photographs and said how much they had enjoyed themselves. Thanks go to the service users, staff and manager for their warm welcome and hospitality during the inspection. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 8 & 9 The individual plans of care are updated regularly and reflect the service users needs and how they wish to be supported. The preferences and wishes of the people who live at Ferncliffe are the predominant influence on the way their home is run. People take agreed risks within a risk management framework. EVIDENCE: The personal plans of care and support which were examined were seen to be well set out and covered all the necessary areas. They had each been developed with the involvement of the service user. Although some care plan reviews had been unavoidably delayed there was evidence to show the review process was back in track. Each service user’s key worker ensures the plans and records are updated regularly. From discussions and observation it was clear that the ladies and gentlemen who live at Ferncliffe are involved in the decision making processes in their home. They also take part in the recruitment and selection of staff. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15 & 17 The people who live at Ferncliffe are supported to access work, leisure and educational activities according to their choice. They use the local services and participate wherever appropriate. The mealtimes are unhurried and enjoyed by service users. EVIDENCE: Written evidence showing how staff support service users to take up work, leisure and educational opportunities was confirmed through discussions with the service users and staff members. The changing needs of service users were also explored, especially with reference to their work. The service users choose the meals they prefer and can participate in the preparation of food and drinks if they wish. Evidence of nutritional and dietary assessment was seen. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 The way each service user is supported was seen to be suitable for their needs and preferences. Individual choices were seen to be respected and conformed with. EVIDENCE: The routines of daily life were seen to be flexible and individual choice was observed to be a priority. Any equipment and adaptations needed to promote and maintain independence are in place. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. JUDGEMENT – we looked at outcomes for standard(s) 23 All the evidence shows that the service users at Ferncliffe are protected from any form of abuse and potential harm. EVIDENCE: Protection of vulnerable adults training had been arranged for the staff. The training is to be offered on several occasions to ensure all staff can attend. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29 & 30 The home’s staff and manager react appropriately to ensure suitable equipment is in place to meet service users changing needs. Infection control measures and laundry facilities are of the necessary standard. EVIDENCE: A new lifting aid has recently been purchased to assist a service user to rise from the floor should they fall and be unable to stand up independently. Two staff members are currently undertaking a training course on the control of infection in residential care homes. The laundry facilities and other infection control measures were seen to meet the standard. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32 & 35 The staff training provided was seen to be of an acceptable standard. The communication and interaction between service users and staff is very effective. EVIDENCE: From a total of ten support staff one has achieved NVQ level 3 in care and four have NVQ 2 in care. A further three staff members are about to start the qualification. Staff were in the process of taking medication administration refresher training. Three new staff were booked to take First Aid training and the remaining staff are to take refresher training to update their skills. Observation and discussion confirmed that the staff at Ferncliffe are well acquainted with the health and social care needs of the service users. They were seen to be good and effective communicators and very keen to support service users to make the most of the opportunities available to them. JUDGEMENT – we looked at outcomes for standard(s) 39, 40, 41 & 43 All the elements assessed here refer to the systems which maintain the quality and safety of services. They were all found to be satisfactory. EVIDENCE: The regular visits to Ferncliffe by a senior manager within the organisation are now being carried out. A quality assurance questionnaire to gain the views of relatives, friends and other visitors has been developed. It is being used with the limited number of people who visit the home. The policies and procedures have been reviewed and evidence was seen of reviews carried out at the end of 2004 and early 2005. The policies and procedures are developed centrally and two service users are involved in this process. All confidential records were seen to be securely stored.
