What the care home does well

The home has an in-depth care planning system with resident and/ or family involvement wherever possible. All healthcare needs are met, with an up to date record showing all professional healthcare visits and external appointments. Efforts are made to ensure that a nutritious and varied menu is provided, with a choice at each meal.

What has improved since the last inspection?

A small amount of internal decoration has been completed but other than this there have been no major changes.

What the care home could do better:

Discussions with the supervisor and staff confirmed that they are continually striving to improve the level of care and support given to the residents. Care is taken to ensure the running of the home is always in the best interests of the people living there.

The service must at all times employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. A maximum of forty older people (OP40) may be accommodated of whom nine may have dementia (DE(E)9). The staffing levels for the home must meet the Residential Forum Care Staffing Formula for Older Adults by 1st April 2004. 4th October 2005 Date of last inspection Brief Description of the Service: Inglewood is a care home providing accommodation and care for up to 40 older people, 9 of whom have dementia. The home is operated by Cumbria Care, an internal business unit of Cumbria County council, and is situated close to the centre of Wigton. The home is divided into four living areas or units, each having a lounge/dining room with small kitchen area. There are bedrooms, toilets and bathrooms nearby. There are also other lounges and sitting areas that can be used by service users who want a quiet area, and a lounge that is used by those who wish to smoke. There is a large room used to provide day care for older people from the local community. This room can also be used by service users during the evening and weekends for special events. There is a passenger lift, handrails, grab rails and assisted bathrooms and toilets to help people in their day-to-day lives. There are pleasant garden areas including a central courtyard and a secure garden area accessed from the unit that accommodates people with dementia. This was an unannounced inspection of the home that took place over the morning of the 22 March 2006. It was the second inspection of the annual cycle and the standards not assessed on this occasion were inspected and met during the previous visit that took place in October 2005. During the inspection, time was spent speaking with the supervisor on duty, staff and residents. Documentation relating to care of the residents was examined and some parts of the home were looked at. What the service does well: What has improved since the last inspection? No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 4 & 5 The Statement of Purpose is informative and provides prospective service users and their families with information about the home. The admission procedure to the home is satisfactory and ensures service users needs can be met. EVIDENCE: The Statement of Purpose was available in the home for prospective residents and/or their families to read. It contained important information about how the home operated, as well as the services and facilities provided. All residents have a contract with the Social Services Department. Prospective residents and their families are invited and encouraged to visit Inglewood prior to admission. This gives opportunity to meet the manager, staff and the other people living in the home, to view the facilities on offer and to ensure all assessed needs can be met. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 & 15 Mealtimes and social activities are both well managed and help to create a varied and positive day for those living in the home. EVIDENCE: There is an activities organiser who works with the residents on various projects, all of which are recorded in the activities file. The inspector was able to observe her working with a small group of residents talking about cooking and baking both of which they enjoyed whilst living in their own homes. Outings and larger group activities are also organised. Some residents are able to attend the Church next door to the home and visiting clergy give communion to those wishing to receive it. Discussions with the cook on duty and inspection of the menus evidenced that a variety of nutritious meals are provided with a choice at each mealtime. All the residents who spoke with the inspector said they enjoyed their food and that there was always plenty. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 & 18 Service users benefit from the availability of an advocacy service should this ever be required. Staff have a knowledge of abuse issues, which protects those living in the home. EVIDENCE: An advocacy service would be provided for any resident who has no family or friends to assist them in managing their affairs. Currently, all those living in the home have family members who are available to assist them with personal and financial matters. All residents are given the opportunity to take part in the election process if they wish. The home uses the corporate adult protection policy that includes whistle blowing. This subject is covered in NVQ training and the staff induction programme. There is a copy of Cumbria’s policy for Protection of Vulnerable Adults in the home that is made available for staff to read or consult if they wish. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 21, 22, 23, 24 & 25 The home offers comfortable and safe accommodation and most areas are well maintained. This contributes to a good quality of life for the residents EVIDENCE: Inglewood is divided into four units each with its own dining and lounge areas. There are other sitting areas around the home that allow residents to see their visitors in private if they wish. There is also a designated smoking lounge for those wishing to use it. There are seven bedrooms with en-suite toilet facilities and communal toilets throughout for the residents to use. There is a total of five bathrooms and a shower room, all of which are suitable to meet the assessed needs. The home provides hoists and other specialist equipment in order that the independence of the residents can be maintained as long as possible. All the bathing and toilet facilities are suitable for those who may have a physical disability and there are handrails on the corridors to assist with movement around the home. All those inspected were nicely decorated, clean and, on the whole, bright. Residents have “ made them their own” with small items of furniture, pictures and ornaments. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28, 29 & 30 Staff have completed the required training and provide a skilled and experienced workforce. EVIDENCE: Cumbria Care has a full recruitment and selection process that is used throughout the organisation. This ensures that no member of staff starts work until all the required checks are completed. The inspector was able to examine a sample of personnel files and found them to be up to date and in order. An ongoing staff training programme is in place covering all the statutory training required, plus other subjects such as medication, infection control and emergency planning. Almost half of the staff is already qualified to NVQ level two with one other currently working towards the award. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 33, 34, 35, 36 & 37 Regular monitoring by the registered manager ensures the home is run in the best interests of the residents. Financial procedures are in place to protect the residents from any financial abuse. EVIDENCE: Although the manager was away from the home on the day of the inspection, discussions with the supervisor and staff on duty evidenced that he is highly motivated and that the home is run in the best interests of the residents and their families. Residents who spoke with the inspector were complementary of his caring attitude and said he was approachable and easy to talk to. Management supervision motivates the staff to undertake training, which assists in the improvement of their skills and knowledge. Financial policies and procedures are in place and are suitable to safeguard those living in Inglewood.
