This inspection was carried out on 29th September 2005.

What the care home does well

John Street provides a relaxed, friendly, homely environment for the service users with a good quality of care. Staff understand the service users needs and interacted well with individuals and worked well as a team. The home is continuously maintained to a high standard and the cleanliness throughout was excellent. Service users were very happy and well looked after. Many activities and outing are arranged for the service users, which are varied and meet their needs. Many questionnaires were returned by service users and relatives and all gave positive feedback, stating the home provides a good quality of care by a dedicated staff team ensuring needs of service users are met.

What has improved since the last inspection?

There is a new day centre project opened in Thurcroft near to John Street that the service users have started using. The project arranges drop in secessions so service users can go along at any time during the secession for a drink and a chat this is well used by the service user at John Street and enjoyed. The domestic hours have been increased and there is currently a vacancy for 16 hours when this is filled each bungalow will have its own domestic.

What the care home could do better:

The home has recently had some garden furniture stolen the staff are currently looking at ways to improve security to try to reduce the risk of it happening again.

If this service user leaves, the condition will be removed. 29th November 2004 Date of last inspection Brief Description of the Service: John Street is a care home for adults aged 18 – 65 with learning disabilities, it consists of three bungalows with 5 service users in each. Each bungalow has its own kitchen and laundry facilities with dining and lounge areas in each. Assisted baths are in each bungalow. There are patio areas and adequate parking facilities. John Street is in Thurcroft, which has shops, pubs, and restaurants walking distance from the home. This was the homes first inspection in the year 2005/2006 and took place over one day the inspection was on 29th September 2005 at 10.25 and finished at 13.45. 5 Service users and 4 staff were spoken to. A tour of the buildings took place, observing staff and practices including activities, meals and interaction with service users. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All service users have their needs fully assessed prior to admission. EVIDENCE: All service users have an assessment undertaken by the manager or appropriately trained member of staff prior to admission and are only offered a trail placement if they feel that all their needs can be met at John Street. For service users referred through the care management a health and social service assessment is also obtained to ensure that the needs of the individual can be met. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All service users have a plan of care reflecting their changing needs and personal goals, including risk assessments. EVIDENCE: Various care plans were seen they were very good explained each individual’s need in detail in order that staff are able to meet there needs. All care plans are reviewed at least every six months. The staff confirmed that the service users and relatives were involved in the implementing and reviews of the plans if they wished. Each service user has individual risk assessments these were good and covered all areas of possible risks and hazards. It was evident from the risk assessments and talking to staff that service users were supported to be able to take responsible risks. Staff are fully trained to ensure they understand all risks associated with each individual service user. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Appropriate activities are provided for service users and their rights are respected. Service users are offered a healthy diet. EVIDENCE: Each service users has a weekly programme attending various day centres and places of work to meet their needs. There is a new day centre project opened in Thurcroft near to John Street that the service users have started using. The project arranges drop in secessions so service users can go along at any time during the secession for a drink and a chat this is well used by the service user at John Street and enjoyed. The daily routines of the home promote individual choice on the day of the inspection the service users in one bungalow had a day of from the various day centres and were all having a very relaxed morning, which was their choice. They were given breakfast in bed, baths, massages, facials, nails and hair done John Street Nursing Home DS0000003095.V250562.R01.S.doc Version 5.0 Page 11 and many did not come into the lounge until 11.30. The service users had enjoyed the relaxing morning and being pampered. All service users who wanted to go on holiday has had a holiday this year a further break was organised the same week as the inspection 5 service users were going to Blackpool, seeing a show and the lights. One service users had also had a special holiday this year organised for his 70th Birthday, which he thoroughly enjoyed. Holidays are organised with service users in order that their choices are considered. Each bungalow has its own menu with choices drawn up with service users input to meet their needs. The menu was varied, nutritious and balanced and met the dietary requirements of individual service users. Meals were also very flexible to fit in with the lifestyle of the service users meeting their needs. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Service users receive personal support in a way they prefer and healthcare needs of the service users are assessed and met. EVIDENCE: It was evident from the plans of care and talking to staff that service users health is monitored and potential complications and problems are identified and dealt with at an early stage to prevent further problems. Staff seek help and advice from various health care professionals this was documented in the plans of care in order to meet the needs of service users. Personal support was given sensitively it was flexible maintaining privacy, dignity and independence; this was evident during the inspection. Service users choices were very evident in the way they were dressed in appropriate clothes for their age wearing jewellery and makeup, which reflected their personality. JUDGEMENT – we looked at outcomes for the following standard(s): Service users views are listened to and are protected form abuse. EVIDENCE: The home has a clear and accessible complaints procedure in easy to read format for the service users. The home has received no complaints in the last 12 months. The home has an adult protection policy all staff are aware of the procedure to follow should an allegation be made. All staff have also attended training on protection of vulnerable adults in order to be aware of different types of abuse and protect service users. JUDGEMENT – we looked at outcomes for the following standard(s): Service users live in a clean safe environment. EVIDENCE: The home is accessible, safe and well maintained and provides a homely friendly atmosphere for the service users. Many communal areas and bedrooms have been re-decorated since the last inspection the decorators were in the home on the day of the inspection painting one of the kitchens. The standard of cleanliness observed throughout the home was excellent providing a clean safe environment for service users to live in.
