What the care home does well

John Street provides a relaxed, friendly, homely environment for the service users with a good quality of care. Staff understand the service users needs and interacted well with individuals and worked well as a team. The home is continuously maintained to a high standard and the cleanliness throughout was excellent. Service users were very happy and well looked after.

What has improved since the last inspection?

The home continually maintains a high standard of care and environmental standards. The domestic hours that had been increased at the last inspection have now been filled and there is a domestic allocated to each bungalow.

What the care home could do better:

The manager and staff are always looking at ways to improve the home for the service users. They are still looking at ways to improve the security of the grounds by providing some better quality fencing.

That one service user over the age of 65 be allowed to reside at the home. If this service user leaves, the condition will be removed. Date of last inspection Brief Description of the Service: John Street is a care home for adults aged 18 - 65 with learning disabilities; it consists of three bungalows with 5 service users in each. Each bungalow has its own kitchen and laundry facilities with dining and lounge areas in each. Assisted baths are in each bungalow. There are patio areas and adequate parking facilities. John Street is in Thurcroft, which has shops, pubs, and restaurants walking distance from the home. This was the homes second inspection in the year 2005/2006 and took place over one day the inspection was on 7th March 2006 at 16.30 and finished at 19.00. 9 Service users, 5 staff and the acting manager were spoken to. The inspector observed staff and practices including meals and interaction with service users. The remaining key standards were assessed during this inspection. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 Service users are able to make decisions about their lives. EVIDENCE: Service users are supported to make decisions this was evident from observing staff with service users and speaking to the service users. Service users ability to make decisions is well documented in the plans of care to ensure their needs are met. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 & 15 Service users access the local community and their rights are respected. EVIDENCE: Staff support service users to participate in the local community using the local facilities including shops, hairdressers, pubs, outreach centres and clubs so that service users are part of the local community. Staff support service users to have relationships with family and friends many service users have regular contact with family and friends. Staff spoken to were aware that relationships had to be appropriate to protect service users as due to their learning disabilities not all service users are able to make an informed decision. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20 Medication policies and procedures protect the service users. EVIDENCE: The home has a good medication policy and procedure in place. Medication was signed for on arrival, administration and disposal. The home has a contract for the disposal of medication and these are stored and documented correctly. Nursing staff give medication but all nursing assistants have received accredited medication training, as occasionally they will give medication on day trips. The home ensures there are regular reviews of medication for all service users to meet the needs of the service users. Some medication is taken to the day centres the home needs to ensure there are policies drawn up with correct procedures to follow and risk assessments are carried out. The day centre will also need to have a Medication record sheet to complete to protect service users. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. Service users benefit from clarity of staff roles and responsibilities. Service users are supported by competent and qualified staff. Service users are supported by an effective staff team. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34 & 35. The home has an effective staff team that are competent and qualified. EVIDENCE: Staff are qualified and competent to meet service users needs. All mandatory training is carried out yearly for all staff and was up to date. Induction training is in place for all new starters and meets the required standards. Staff were currently completing adult protection updates and dementia training. All nursing assistants have achieved NVQ level 3 or above apart from the new starters who are waiting to commence the training to ensure they are qualified to meet the needs of the service users. A selection of personal files were looked at they all contained all the required information and checks to ensure the service users are protected. Service users benefit from competent and accountable management of the service. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39 & 42. The home is well run with health and safety promoted and maintained. EVIDENCE: The registered manager is competent and qualified to run the home, she is currently on sick leave but due to return on 1st March 2006. The Deputy Manager has been acting manager during her sick leave she is also competent and qualifies to run the home all staff have been very supportive during this period and the service users benefit from a well run home. The home has got good quality monitoring systems based on seeking views of service users and Regulation 26 visits are carried out to ensure service users views underpin development in the home. The home has a good health and safety policy and staff have received health and safety training to ensure the safety of the service users. John Street Nursing Home DS0000003095.V273762.R01.S.doc Version 5.0 Page 15 The maintenance records for electrical safety, Pat testing, legionella, fire checks and safe environment including equipment and machinery were all available at the time of the inspection up to date ensuring the safety of the service users. The water checks appeared to be out of date however since the inspection the acting manager has informed the inspector they have been carried out but records had been sent to head office these are now at the home.
