Conditions of registration: 1. Age range 55yrs Date of last inspection 24/05/05 Brief Description of the Service: Nutley Lodge is a detached, extended property, situated in the village of Elburton. It is set in its own grounds with easy access for service users. The Home is registered to provide accommodation and personal care for up to 25 service users over the age of 55 for reason of old age, physical disability and dementia. The home is registered for the categories OP (Older Person), PDE (Physical Disability Elderly) and DEE (Dementia Elderly).The home is owned by Mr Jeremy and Mrs Lynwen Woodcock and the registered manager is Mrs Allison Thompson. The home has 23 single bedrooms and 1 double bedroom, 14 of the single bedrooms and the double bedroom have en-suite toilet facilities. On the ground floor there are two lounge rooms and a large dining room with a further seating area in the entrance lobby. A shaft lift and a stair lift provide access to the first floor. There is a call bell system throughout the home. Service users are enabled to access any health or social care services they require and various social activities are arranged by the home. This is an overview of what the inspector found during the inspection. The inspection was unannounced and took place in the afternoon on 21st December 2005. As the key standards had been assessed at the previous inspection in May 2005, the focus of this inspection was to consult with residents and review the care planning processes. The inspector spoke to 12 residents as well as 6 visitors, toured the building and examined care records and other documents relating to fire safety issues. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3, 4, 5 Thorough and comprehensive systems for admission allow residents and their relatives to be confident that their needs can be met. EVIDENCE: A newly admitted resident and her family confirmed how the Registered Manager had undertaken a thorough assessment of her needs as well as fully explained the services provided at the home. They said that felt very confident at their pre-admission visit that the home would be appropriate and have since been very happy with the care provided. The relatives said that they can visit freely and have been made very welcome by the care staff. Care staff confirmed that training relating to the care needs of older people with dementia is being organised for the spring and would be undertaken by the majority of the staff once their medication training has been completed. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 Residents’ health, personal and social care needs are being met and residents are treated respectfully. Practices relating to medication administration protect the residents from risk. EVIDENCE: Residents said they could not be treated better and described living in the home as “lovely”. The six visitors spoken to by the inspector said they were very happy with the care their relatives received and described the home as “one happy family”. One resident described how her health had improved since admission and that she had started to gain weight as the food was “excellent”. Those care plans sampled detailed residents’ care needs and had been regularly reviewed and updated to ensure they reflected current care needs. Records of accidents are recorded and care staff confirmed that these are reviewed to determine whether a residents’ health or safety are at risk. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13, 15 Residents can be confident that their relationships with family and friends are respected. Meals are nutritious and varied. EVIDENCE: The six visitors spoken to by the inspector said that they have been made very welcome and can visit the home at any reasonable time. They said they were being kept informed of the welfare of their relatives. Residents said that the meals were “excellent” and they were offered a choice everyday. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 Complaints and suggestions from residents, relatives or other visitors to the home, are treated seriously. Service users are listened to and issues resolved promptly. EVIDENCE: Residents and visitors said that the Registered Manager and the care staff were very approachable and those residents that were able confirmed that they were confident any issues of concern would be listened to and dealt with. The home has received no complaints since the last inspection. Although staff were able to describe the actions they would take should an issue of abuse be suspected, they had received no formal training. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 24, 25, 26 The residents live in a pleasant, well-maintained home that is comfortable and warm and which provides sufficient facilities to meet their needs. EVIDENCE: All communal rooms and bedrooms were pleasantly decorated and furnished and residents said that it is always very clean. The main hallway, stairs and landing carpets have been replaced since the last inspection. Radiators were guarded and, where a risk had been identified, hot water temperature controlled to protect residents from scalds. The home was found to be very clean and tidy and the visitors said that they have found it to be clean and free from offensive odours whenever they visit. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for the following standard(s): 27 Residents are cared for by caring staff in sufficient numbers to meet the needs of those currently living in the home. EVIDENCE: Residents described the staff as very kind and caring and confirmed they responded promptly to requests for assistance indicating that there are sufficient staff to meet the needs of those currently living in the home. Many of the care staff had been employed at the home for many years and staff turnover was low thus providing a settled environment for the residents. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 36, 37, 38 Residents live in a well managed home. The owners, manager and staff strive to provide a stimulating, safe environment that respects and protects residents’ rights. EVIDENCE: Residents said that they feel safe and secure in their home and that the owners and Registered Manager are very approachable and available most days. Staff confirmed that they receive regular supervision with the Registered Manager, both individually and in small groups to discuss their training and development needs and to ensure consistency with the care of the residents. The fire log book detailed equipment testing to ensure that it is functioning correctly and staff training to ensure care staff are aware of their responsibilities.
