What the care home does well

The care plans provide a clear and easy to read description of care needs ensuring that all staff have the information they need to fully support the residents. The residents describe the care staff as very kind and caring and living in the home as "lovely". Staff work closely with other health care professionals who provide specialist advice such as dementia care and mobility difficulties.

What has improved since the last inspection?

Mr Woodcock and Mrs Thompson have addressed the issues raised at the last inspection relating to the environment, documentation and staff training: a lock has been fitted to the ground floor bathroom; hot water temperature control valves are fitted to hot water outlets identified as posing a risk to residents; the residents` contract has been updated; a risk assessment relating to the control of Legionnaires Disease has been completed and staff training in the safe administration of medication has been arranged.

What the care home could do better:

The Quality Assurance process should be extended to ensure that information relating to the quality of the services provided can be assessed and an action plan developed ensure continual improvement. Training should be provided relating to dementia care and adult protection to ensure that the services provided are based on current good practice.

Age range 55yrs Date of last inspection 8/03/05 Brief Description of the Service: Nutley Lodge is a detached, extended property, situated in the village of Elburton. It is set in its own grounds with easy access for service users. The Home is registered to provide accommodation and personal care for up to 25 service users over the age of 55 for reason of old age, physical disability and dementia. The home is registered for the categories OP (Older Person), PDE (Physical Disability Elderly) and DEE (Dementia Elderly).The home is owned Mr Jeremy and Mrs Lynwen Woodcock and the registered manager is Mrs Allison Thompson. The home has 23 single bedrooms and 1 double bedroom, 14 of the single bedrooms and the double bedroom have en-suite toilet facilities. On the ground floor there are two lounge rooms and a large dining room with a further seating area in the entrance lobby. A shaft lift and a stair lift provide access to the first floor. There is a call bell system throughout the home. Service users are enabled to access any health or social care services they require and various social activities are arranged by the home. The garden is attractive, spacious and has physical disability access. The inspection was announced and took place on 24th May 2005 over a 5-hour period. The focus of the inspection was to consult with residents, review the care planning processes and the progress with the requirements and recommendations made at the last inspection. Mr Woodcock, the owner, and Mrs Thompson, the Registered Manager, were present and they and their staff team assisted the inspector throughout the inspection. The inspector spoke to 17 residents as well as 2 visitors, toured the building and examined care records and other documents. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 5 Thorough and comprehensive systems for admission allow service users and their relatives to be confident that their needs can be met. EVIDENCE: Assessments were undertaken to identify residents’ needs prior to admission and these were available for 2 newly admitted residents: these residents confirmed that they and their families had been able to visit the home before making a decision to move in. Care management assessments and hospital discharge assessments were also available. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10 and 11 Residents’ health, personal and social care needs are being met and residents are treated respectfully. Practices relating to medication administration protect the residents from risk. EVIDENCE: Residents said they could not be treated better and described living in the home as lovely. Two visitors said they were very happy with the care their relatives received: one described the care staff as an extended family. Care plans detailed residents’ care needs: these plans were reviewed regularly and included assessments relating to pressure area care and moving and handling. The District Nurse, Community Psychiatric Nurse and the Continence Advisor provided specialist advice. Residents with terminal illnesses could remain in the care home as long as their needs could continue to be met. Mrs Thompson said that she and her staff had been very well supported by the District Nursing Service whilst they were caring for two terminally ill residents. Medication administration records, including the controlled drug register, were well maintained and medication was stored safely. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 Social activities provide daily interest for the residents and meals are nutritious and varied. EVIDENCE: Due to memory loss many of the residents spoken to were unable to recall the activities provided, however a record had been maintained of residents’ involvement in a variety of activities facilitated by the staff, including table top games, music and walks. Themed events were organised periodically reflecting the time of the year. Relatives said that they were always made welcome and could visit at anytime. Residents described the food as very good and those without memory loss said that there was always an alternative to the main menu. A supper of cheese and biscuits or sandwiches was offered. Special diets were catered for, including diabetic diets and pureed food. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18 Complaints and suggestions from residents, relatives or other visitors to the home, are treated seriously. Service users are listened to and issues resolved promptly. EVIDENCE: Residents said that the Registered Manager was very approachable and those that were able confirmed that they were confident any issues of concern would be listened to and dealt with. The home has received no complaints since the last inspection. Although staff were able to described the actions they would take should an issue of abuse be suspected, they had received no formal training. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 24, 25 and 26 The service users live in a pleasant, well-maintained home that is comfortable and warm and which provides sufficient facilities to meet their needs. EVIDENCE: All communal rooms and bedrooms were pleasantly decorated and furnished and residents said that it is always very clean. Mr Woodcock, the owner, maintains a schedule of redecoration and refurbishment to ensure the home remains in a good state of repair: the main hallway, stairs and landing carpets are due to be replaced next week. A risk assessment relating to the control of Legionnaires Disease was available and regular water temperature monitoring was taking place to ensure the control measures were effective. Radiators were guarded and, where a risk had been identified, hot water temperature controlled to protect residents from scalds. The hot water from bedroom sinks was assessed at the time of the monthly care plan reviews to ensure individual residents were not as risk. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29 and 30 Residents are cared for by caring staff in sufficient numbers to meet the needs of those currently living in the home. Recruitment processes protect vulnerable residents. EVIDENCE: Residents described the staff as very kind and caring and confirmed they responded promptly to requests for assistance. Many of the care staff had been employed at the home for many years and staff turnover was low thus providing a settled environment for the residents. Care staff are supported by catering, laundry and domestic staff. A training programme detailed the ongoing training undertaken by staff ensuring that they have the skills to care for older people: training in the safe administration of medication had been arranged for the following week and dementia care training for later in the year. All staff had recently undertaken first aid training in April and May. Fifty percent of staff were trained to NVQ level 2. Staff recruitment practices ensure the relevant checks were made prior to employment. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 34, 35, 36, 37, 38 Service users live in a well managed home. The owners, manager and staff strive to provide a stimulating, safe environment that respects and protects service users’ rights. EVIDENCE: Residents said that they feel safe and secure in their home. Mr Woodcock was available each day and Mrs Thompson meets with service users, relatives and staff on a daily basis. Residents, relatives and friends are invited to join the “Friends of Nutley Lodge” association and meetings are arranged monthly and minutes are recorded. A written survey has been sent to each service user to be completed by them or, if they are unable, their relatives, seeking their views about the home and the services it provides: these should be extended to include the ability to comment directly of the quality of the services provided to enable Mrs Thompson to ensure the high standards are maintained and actively improved upon. Nutley Lodge D52-D04 S3484 Nutley Lodge V214569 240505 Stage 4.doc Version 1.20 Page 14 Mrs Thompson supervises the staff team: as much of this supervision has been spontaneous and in response to the changing needs of the residents it has not always been recorded. Daily handover meetings between staff take place and Mrs Thompson uses this opportunity to provide teaching sessions relating to the care needs of the residents such as managing a resident’s anxiety and the care of residents with diabetes. Money was held for safe keeping for a number of residents and this was checked and records were found to be accurate. Mr Woodcock audits the records monthly to ensure accuracy. Written policies addressed health and safety matters. The fire log detailed equipment testing and staff training. Mrs Thompson and a senior member of staff are trained as fire safety trainers and moving and handling trainers ensuring that they can respond immediately to staff’s training needs.
