Conditions of registration: Date of last inspection 28.10.04 Brief Description of the Service: 14 Otley Road is registered to provide residential personal and social care for 13 younger adults under the age of 65 years who have learning disabilities and associated sensory impairment. The home is situated in a residential area approximately 1 mile from Harrogate town centre and provides good access to the town’s services and amenities. The property is a large converted manor house with a large accessible garden. The upper floors are accessible to service users by vertical passenger lift if required. The registered provider is Foresight Residential Ltd and the responsible person is Mr S Coldwell. This is an overview of what the inspector found during the inspection. This was an unannounced inspection carried out as part of the inspection process on 19th April 2005. The manager Ms Carol Sowerby and her deputy Mrs Erica Kubu were available to assist with the inspection. It was possible during the inspection to speak to five members of staff and 10 of the residents.. It was also possible to spend the lunchtime with the staff and residents and also witness some of the activities in the home. The inspection also involved a tour of the premises and a check on the records kept by the home. At present there are 11 residents and two vacancies in the home. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2, 5. All residents individual aspirations and needs are assessed. EVIDENCE: No new residents have been admitted recently but the evidence from previous admissions and the individual assessments that currently take place indicates that prospective residents will be fully assessed. All residents have an individual contract and reasonable steps have been taken to ensure that it is explained to them. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,9,10. The residents health and personal care needs are met and they are encouraged and supported to make choices about their daily lives. This helps them to have control over their lives in the home. EVIDENCE: Comprehensive assessments and care plans identify their personal and social care needs which are updated regularly between the resident and the key worker. Risk assessments are included in the care plans so that both staff and residents are aware of the support required. The staff have monthly care plan meetings at which residents changing needs are discussed. This ensures that staff are kept up to date and that a consistent approach can be achieved. Staff unable to attend these meetings must read the minutes and sign to record that they have done so at the earliest convenience. There were many examples of residents making decisions in the home and about their lives and these included, choice of daily activity programmes, leisure activities, and menu planning. The residents were pleased to discuss these and they were aware that some required support and others could be more independent in different activities.

Residents were aware that records were confidential and could only be shown to others with their permission. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15,16,17. Residents eat well and enjoy a wide range of activities both inside and out of the home and are part of the local community. EVIDENCE: Each resident had an individual programme with short and long term goals aimed at developing their skills. The staff are constantly looking for new activities which will interest the residents and provide further stimulation and development. All residents use the local facilities in Harrogate eg theatre, pubs, and clubs. They meet with residents from other homes, and also have friends who visit and take them out and spoke enthusiastically about these trips. Many residents go home regularly to their families. Lunch was served during the inspection and a variety of well prepared dishes were available for residents to choose. The home has a cook who regularly talks to the residents and prepares their favourite meals. Residents said that they really enjoyed the meals and this was witnessed during the inspection. Some residents also help in the kitchen as part of their programmed activities.

The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19. The residents physical and emotional health needs are met. EVIDENCE: The care plans were comprehensive documents agreed with the residents and in conversation both residents and staff found them very useful for providing the appropriate levels of care. . Key workers meet regularly with their resident to make any changes and to set short term goals. These are then presented to the staff at care plan meetings to make everyone aware of the changes and achieve a consistent approach. The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22,23. Residents are protected from abuse neglect and self harm. EVIDENCE: The home has a detailed complaints procedure, in suitable formats, but residents have not used it but felt safe knowing that it was there. They also said they could talk to the manager and staff and knew that they would take them seriously. The vulnerable adults procedure is available in the home and training of staff has been recorded. Staff were aware of the procedure and as with the complaints procedure above would take all reports seriously and deal with it properly. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,27,29,30. The home is clean, and well decorated and furnished and provides a comfortable place to live. EVIDENCE: 14 Otley Road is decorated with the age and needs of service users in mind. Lighting and colour contrasting have been given high priority to help residents use the home with greater independence and safety. The home meets the RNIB guidelines for mobility with different areas marked with contrasting tactile surfaces and colours again to help independence and safety. Residents are encouraged to choose their own decorations and furniture in their rooms and to provide their own personal items which makes it their own private space. The communal rooms are well furnished and decorated and residents said they were happy to bring people into the home. The home was clean and hygienic and free from offensive odours. There is an infection control policy to alert staff and ensure good hygiene practices.

Service users are supported by competent and qualified staff. Service users are supported by an effective staff team. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,35,36. The staff are well trained and well supported by the manager and this ensures that residents feel supported and staff are aware of their duties. A robust recruitment procedure offers protection for the residents in the home. EVIDENCE: The home has a settled staff team. They work at times suitable for the residents who all felt supported by them. Staff also felt that there were enough staff on each duty to meet the needs of the residents. A key worker system operates in the home. Residents felt that this was helpful to have someone to work closely with. Staff also felt that it helped the residents especially with communicating their care needs. One also said that it helps with little things like making sure they got the deodorant they liked rather than all having the same. Four staff files were checked and records showed that the recruitment procedure was robust and offered protection to the residents. All staff were doing NVQ level 2 and all statutory training was up to date. This provides protection for the residents

Service users benefit from the ethos, leadership and management approach of the home. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37,38, 39,42. There is clear leadership and guidance from the manager which ensures consistent quality care for the residents. This means that the health, safety and welfare of the residents is promoted at all times. EVIDENCE: The manager believes that the residents should be involved in the running of the home so keeps them informed and asks for their opinions. The residents said this gives them trust in the manager and staff. The residents do not have formal meetings as it is considered too stressful for some. They all eat together and this provides a relaxed informal atmosphere where all residents can contribute. This was witnessed during the inspection and the amount of information shared and the level of interaction between staff and residents showed how much control the residents have over their lives. With the homes own quality assurance and the RNIB Accredited Scheme there are sufficient consultation processes and residents were happy that their views were listened to regarding the development of the home.
