What the care home does well

The residents said they were always told what was going on and were given chance to have their say, which made them feel that it was their home. The home is clearly run for the residents who were encouraged to make choices about their daily lives both in the home and for activities outside the home. This made them feel in control of their lives. Residents said that all the staff were very helpful and they were able to talk about any problems with them or the manager. The home has been awarded the RNIB Accredited Scheme for each of the last 7 years. This ensures that the environment is suitable for the residents and that specific training for staff is provided. There is a nice atmosphere in the home and staff and residents work together very well. Residents said that they ` love living at Otley Rd`.

What has improved since the last inspection?

The RNIB Accreditation work continues, demonstrating constant awareness and improvement in all areas. The staffing levels have been increased to meet the specialist needs of a resident. The deputy manager has completed the Registered Managers Award and all staff are engaged in NVQ training.

Registered for 13 Service Users with Learning Disabilities, some or all of whom may also have a Physical Disability and/or Sensory Impairment. 19th April 2005 Date of last inspection Brief Description of the Service: 14 Otley Road is registered to provide residential personal and social care for 13 adults aged 18 to 65 years who have learning disabilities and associated sensory impairment. The home is situated in a residential area approximately 1 mile from Harrogate town centre and provides good access to the towns services and amenities. The property is a large converted manor house with a large accessible garden. The upper floors are accessible to service users by vertical passenger lift if required. The registered provider is Foresight Residential Ltd and the responsible person is Mr S Coldwell. The registered manager is Ms Carol Sowerby. This was an unannounced inspection carried out as part of the inspection process on 20th December 2005. The home was ready for Christmas and some of the residents were preparing to go home and two had already gone. The manager Ms Carol Sowerby was available as were six members of the staff including the cook and the cleaner. It was also possible to speak to nine residents and one carer. As all the key standards had been met at the previous inspections this inspection concentrated on conversations with the residents and staff and observation of their interactions, and some of the care practices. The inspector also had lunch with the residents which was a very pleasant experience. It was clear that the home was well managed, that the residents are encouraged and supported to take control of their lives, and that the staff are well trained and aware of their duties. The home has a relaxed atmosphere but the staff have a professional approach to caring. The residents all said that they were very happy living at Otley Rd. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 The residents health and personal care needs are met and they are encouraged and supported to make choices about their daily lives. This helps them to have control over their lives in the home. EVIDENCE: The residents were aware that their care plans and risk assessments were written to help the staff provide the level of care they required and that they could change them as required. It was clear that the staff were also aware of the individual needs of the residents and how the care needed to be provided. All the residents were assisted and encouraged to do as much for themselves as possible and the interactions between staff and residents was relaxed and professional. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17. Residents eat well and enjoy a wide range of activities both inside and out of the home and are part of the local community. EVIDENCE: Many residents go home regularly to their families and there were only going to be four residents in the home for Christmas but they had planned their day. Last year they went out for lunch but had not enjoyed it so this year they were staying at home and had arranged the menu, who would be on duty, and how they wanted to spend the day. They were really looking forward to it. The carer spoken to said she was very happy with the home, the care it provided, and the good communication links it developed. Each resident had an individual programme with short and long term goals aimed at developing their skills. The staff are constantly looking for new activities which will interest the residents and provide further stimulation and development. All residents use the local facilities in Harrogate e.g. theatre, pubs, and clubs. Otley Road 14 DS0000061602.V273816.R01.S.doc Version 5.0 Page 11 They meet with residents from other homes, and also have friends who visit and take them out and spoke enthusiastically about these trips. Lunch was served during the inspection and a variety of well prepared dishes were available for residents to choose. The home has a cook who regularly talks to the residents and prepares their favourite meals. Residents said that they really enjoyed the meals and this was witnessed, and experienced during the inspection. Some residents also help in the kitchen as part of their programmed activities. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20. The residents physical and emotional health needs are met. EVIDENCE: The residents said that the support they received from the staff was how they had agreed it in their care plans. They said that the care staff and manager were very supportive and that if they had any problems they could talk to them and were confident that they would help. All staff have had medication training and two staff always deal with the administration. The pharmacist also visits to check the storage and provides a report. A safe system with good record keeping is in place. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22, 23. Residents are protected from abuse neglect and self harm. EVIDENCE: The home has a detailed complaints procedure, in suitable formats, but residents have not used it but felt safe knowing that it was there. They also said they could talk to the manager and staff and knew that they would take them seriously. The vulnerable adults procedure is available in the home and training of staff has been recorded. Staff were aware of the procedure and as with the complaints procedure above would take all reports seriously and deal with it properly. Residents said they felt safe in the knowledge that everyone knew about it. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,30. The home is clean, and well decorated and furnished and provides a comfortable place to live. EVIDENCE: 14 Otley Road is decorated with the age and needs of service users in mind. Lighting and colour contrasting have been given high priority to help residents use the home with greater independence and safety. The home meets the RNIB guidelines for mobility with different areas marked with contrasting tactile surfaces and colours again to help independence and safety. Residents are encouraged to choose their own decorations and furniture in their rooms and to provide their own personal items which makes it their own private space. The communal rooms are well furnished and decorated and residents said they were happy to bring people into the home. The cleaner said she is employed only for the communal areas as the residents take responsibility for their own Otley Road 14 DS0000061602.V273816.R01.S.doc Version 5.0 Page 15 rooms with the support of the care staff. This encourages independence and a feeling of ownership, and residents said that they were proud of their rooms and the home. The home was clean and hygienic and free from offensive odours. There is an infection control policy to alert staff and ensure good hygiene practices. The home has a planned maintenance programme to ensure that it is kept safe and comfortable. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,35. The staff are well trained and well supported by the manager and this ensures that residents feel supported and staff are aware of their duties. A robust recruitment procedure offers protection for the residents in the home. EVIDENCE: The home has a settled staff team. They work at times suitable for the residents who all felt supported by them. Staff also felt that there were enough staff on each duty to meet the needs of the residents. It was clear that the staff knew the residents well and that they got on well together. Informal chat was interspersed with good care practices and a professional approach. Staff said they were well supported by the manager and worked well as a team. A part time member of staff said he attended the staff meetings and training, and enjoyed the support of the staff team. There is clear leadership and guidance from the manager which ensures consistent quality care for the residents. This means that the health, safety and welfare of the residents is promoted at all times. EVIDENCE: The manager believes that the residents should be involved in the running of the home so keeps them informed and asks for their opinions. The residents said this gives them trust in the manager and staff. The residents do not have formal meetings as it is considered too stressful for some. They all eat together and this provides a relaxed informal atmosphere where all residents can contribute. This was witnessed during the inspection and the amount of information shared and the level of interaction between staff and residents showed how much control the residents have over their lives.
