Conditions of registration: Date of last inspection 2.12.04 Brief Description of the Service: Perran Bay provides care for up to 34 elderly service users. The home provides spacious accommodation adjacent to the main street of Perranporth, with a variety of shops, which are readily accessible. There are uninterrupted views of Perranporth beach on one side of the home including from the main lounges. The accomodation is provided over three floors. There is a lift to the first floor. The home also has two pleasant gardens, and there is adequate parking for visitors. The registered provider is Cornwall Old Peoples Housing Society which is a registered charity. This is an overview of what the inspector found during the inspection. This inspection took place over eight and a quarter hours. The inspection was carried out on an unannounced basis. The primary focus was regarding requirements from the previous inspection and other standards not inspected at that time. The inspector was able to speak to many service users, and some of the staff on duty. The inspector examined the medication system, staff and care records, and inspected the building. A new manager has been recruited who appears to be professional and enthusiastic. She has improved some of the management and care systems in the home. A quality assurance system has been set up to ascertain levels of satisfaction of care given. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3, 4, The registered persons provide suitable information to enable prospective service users to make an informed choice regarding moving into the home. Service users are assessed appropriately before admission is arranged. There are appropriate links with external professionals so service users know the home can meet their needs. EVIDENCE: The inspector viewed a suitable statement of purpose. A copy of this is available in the hallway. Service users had a copy of a pre admission assessment on their files. Copies of assessments by social services and the healthcare trust are obtained where possible. The registered manager said good links have been developed between the home and external professionals such as GP’s, district nurses and community psychiatric nurses. Service users also have access to dentists, chiropodists etc.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10, 11 The registered persons provide suitable care plans for service users. There are appropriate links with external agencies so service users health care needs can be met. The home has a satisfactory medication system. The measures ensure service users health and personal care needs are met appropriately. Appropriate measures are in place so service users are treated with respect and dignity. The home has an appropriate policy so service users are treated with appropriate care when they are dying. EVIDENCE: The inspector viewed satisfactory care plans for each service user. Appropriate accident and daily records are maintained. The registered manager said suitable links are maintained with health professionals. She said no service users currently have pressure sores. Two staff have completed training courses in tissue viability. Hearing and sight tests are arranged regularly for service users. The inspector spoke to a visiting district nurse who said she was happy with the care provided. The medication system was also inspected and this was satisfactory. The registered manager said staff who administer medication have received training from the pharmacist, and medication is administered appropriately. The registered manager said service users privacy is respected, and they are treated with respect and dignity. For example staff always knock on service users doors and personal care is carried out in private. Bedroom doors all have locks, and service users have a lockable box in their bedrooms for valuables. Service users were very positive about how staff work with them, and felt they were always treated with respect and dignity. The inspector viewed a suitable policy regarding death and dying. One member of staff has received training regarding palliative care, and a further two staff will be trained shortly. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15 Routines in the home are individualised so the home does not operate along institutionalised lines. Suitable activities are on offer so there are appropriate opportunities for entertainment and stimulation. Meals are to a high standard so service users can have a balanced and wholesome diet. EVIDENCE: Service users said routines are individualised; they are supported to get up and go to bed when they wish. The registered manager said a varied range of activities is offered such as bingo, exercise and arts/crafts. Other activities are also arranged on an ad hoc basis. There have been recent visits by singers, a story teller and the local museum. External trips are also arranged. An activities book is maintained. There are also regular residents meetings. The registered manager said visiting times are open. The inspector shared a meal with service users. This was to a high standard. Service users said the standard of catering was very high and a choice of meal was offered. The registered manager said the four weekly menu is regularly reviewed for example at resident’s meetings. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18 The registered persons have developed satisfactory policies and procedures regarding complaints and adult protection. This assists the process of ensuring service users rights are protected. EVIDENCE: The inspector viewed appropriate complaints and adult protection policies. Copies of the policies are displayed in the hallway. A comments book is also available in the hallway where people can enter any concerns. The registered manager said two complaints had been received. After discussion the inspector concluded these appeared to have been dealt with appropriately. The registered manager said issues regarding the prevention of abuse are covered in the staff induction. Some staff have attended adult protection training. The registered provider is currently investigating an alleged theft of money from a service user. The registered provider will inform the Commission regarding the outcome of this investigation. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19-26 The registered persons provide a suitable environment to meet the needs of service users. The building is homely, well maintained and clean. One bedroom, due to its location, would benefit from an en-suite facility- and the registered persons are considering this option. EVIDENCE: The inspector inspected the building. Service users said they found the home pleasant and homely. Bedrooms are suitable to meet service users needs. The carpet in the hallway needs to be cleaned or replaced. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27-30 Staffing levels are appropriate to meet the needs of the service user group, so service users have adequate support to meet their care needs. An appropriate recruitment, induction and training programme was outlined by the registered manager. However documentation must be improved so service users can be assured staff are appropriately recruited and trained to carry out their jobs. EVIDENCE: The registered manager said the following staffing is provided. At least: • 4 staff from 8-2 • 3 staff from 2-6 • 2 staff from 6-10 • 2 waking night staff and 1 sleep in member of staff • Kitchen and cleaning staff The inspector viewed staff rotas and staffing appeared to be satisfactory. Staff appear to have regular days off and the rota appeared to be fair and equitable. The registered manager outlined an appropriate recruitment procedure. The inspector examined staff files. Some gaps in information required by regulation was noted (e.g. staff must provide proof of identity). A copy of a staff induction checklist was viewed. Although it is evident staff receive an induction, the checklist is required to be more comprehensive. The registered manager said staff received appropriate training. The manager said the majority of staff either had or were enrolled on at least NVQ 2, and had received other training required by regulation (e.g. fire, infection control,

However records of training kept are not satisfactory, and the registered manager could not evidence training had been received. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 36, 38 The registered manager has suitable skills, experience and knowledge to manage the home and there appears to be a positive working culture. The registered persons have a satisfactory quality assurance and day to day supervision systems. These measures ensure the home is well managed, and run by competent staff. Some improvements are required regarding health and safety so service users can be assured they live in a safe environment. Policies must be more accessible to staff. EVIDENCE: The registered manager said she has received her NVQ 4 in care and has completed her registered manager’s award. She appears to have suitable experience and knowledge to manage a care home. For example until recently was Assistant Manager at a similar care home. Staff and resident meetings are held regularly, and suitable day to day supervision systems are in place. For example there is always a senior member of staff on duty. However no formal one to one supervision system is currently in place. The registered manager has set up a quality assurance system. There is evidence this has been implemented for example service users, staff, and visitors have been consulted regarding their views of care. Record keeping regarding care and the running of the home is satisfactory. However, although the registered persons have developed suitable policies and procedures, a condensed file containing policies required by regulation must be made more readily available to staff-e.g. in the staff office. Health and safety precautions are generally satisfactory. An external contractor tested the water in regard to the Legionella, and the test results were satisfactory. However the registered persons should liaise with the Environmental Health Officer to check no other precautions are necessary. Emergency lighting and fire alarms must be tested at a frequency recommended by the Fire Officer. The Registered Provider should make arrangements for an assessment of the premises by a suitably qualified person, such as an Occupational Therapist. The carpet in the hallway should be cleaned or replaced.
